I have a diagnosis of two auto-immune diseases; Ankylosing Spondylitis (AS) and Inflammatory Bowel Disease (IBD). In this video I talk about a new medication I have just started; Sulfasalazine. Sulfasalazine is a Disease Modifying Anti-Rheumatic Drug (DMARD). It’s the first DMARD I have ever been on to treat my auto-immune conditions.

DISCLAIMER: I am not a DR or a qualified health professional. I am making these videos as a patient blogging about my experience of living with these diseases in an attempt to help others living with similar diseases. I am not attempting to give medical advice - just to share my story.

Sulfasalazine a Milder DMARD than some others. Can be used alone or in combination of other DMARDs. Reduces joint pain, swelling stiffness. I’m currently taking Sulfasalazine alongside a medication which I was on beforehand called Celecoxib.

Celecoxib belongs to a different group of drugs called Non Steriodal Anti-Inflammatory Drugs (NSAIDs). I’ll link below a video where I explain how NSAIDs work in more detail.

As I said I’m still taking Celecoxib. This is because Sulfasalazine is a slow-acting drug. Patients typically start to notice a benefit after 6-8 weeks although it can take up to 6 months before people feel the full effects of the Sulfasalazine.

Sulfasalazine also belongs to a group of drugs called Sulfonamides (sulfa-drugs). These are drugs that are derived from sulfanilamide, a sulfur-containing chemical. Most sulfonamides are antibiotics, but some are prescribed for treating rheumatic conditions or IBD, such as Sulfasalazine.

There are quite a few possible side-effects that people can develop on Sulfasalazine - because of this the dosage of medication is built up over time. I took 500mg once a day for two weeks, before increasing to two 500mg tablets a day. Some people may continue to increase their dosage over time (most people wouldn’t be on more than 4000mg a day, depending on the condition they are taking it for). (1)

Side effects:
- Some people are allergic to sulfa-based drugs and may come out in a rash or have another allergic reaction. My rheumatologist advised me to stop taking the medication immediately if this happens.
- As I said Sulfasalazine is a DMARD. Part of the way that DMARDs work is by suppressing the immune system. This means that the inflammation levels in the body are lowered and inflammatory auto-immune conditions can be treated; but it can also make people more susceptible to getting ill as the immune system may not be functioning as efficiently. It can also reduce white blood cell count. Because of this, when I’m first starting this medication, I have to get regular blood tests to check inflammatory markers in the body, blood cell counts and liver and kidney function.
- One side effect that I definitely noticed when I first started taking Sulfasalazine is that I developed quite a few mouth ulcers. This was obviously a little bit sore but it seemed to begin to even out after a couple of weeks.
- Another common side effect of Sulfasalazine is nausea. Made me nauseous when I first started taking it, although no vomiting - just reduced appetite. This is slightly better now.
- Sulfasalazine can cause a fever in reaction to the drug. This is sometimes called a ‘drug fever’. I haven’t felt unwell since I have started taking Sulfasalazine, but I have had a persistent high temperature (normally sitting around 38 whenever I’ve taken it). This is something I will be asking my rheumatologist about when I next see him.
- Colour of urine - mine has become a bright yellow / orange colour. This is a normal side effect and nothing to worry about.

There are lots of possible side effects but I’m only going to talk about the ones that I have noticed myself. Medication reacts to every different person differently so please do your research before you begin a new medication and talk to your DRs if you are worried about any possible side effects.

Because Sulfasalazine is a slow acting drug, I haven’t really noticed many benefits yet - but that’s to be expected because I’ve not even been on it a month yet and it can take 6-8 weeks for most people to begin to notice a difference. I’ll try and film an update after I’ve been on it a little longer to talk about the changes I’ve noticed. At the moment I’’m just trusting in the process and taking it in good faith.

Aminosalicylates include sulfasalazine and 5-aminosalicylic acid (5-ASA).

Sulfasalazine, a sulfa drug, inhibits folic acid synthesis. As such, folic acid supplements should be taken with sulfasalazine to reduce the risk of neural tube defects.

Sulfasalazine exhibits anti-inflammatory properties when split by gut bacterium into its metabolites: sulfapyridine and 5-ASA (mesalamine).

The anti-inflammatory benefits of sulfasalazine, are chiefly derived from 5-ASA, which has fewer side effects than sulfapyridine. As such, administration of 5-ASA alone may be preferred over sulfasalazine.

5-ASA is poorly absorbed by the intestines and systemic circulation, thus most remains in the terminal ileum and colon or is passed in the stool. 5-ASA within the lumen primarily exhibits a topical effect on the colonic epithelium.

Absorbed 5-ASA is extensively metabolized to N-acetyl-5-ASA by N-acetyltransferase 1 (NAT1). N-acetyl-5-ASA then binds PPAR-gamma (peroxisome proliferator-activated receptor gamma) a nuclear hormone receptor.

Binding of N-acetyl-5-ASA induces the translocation of PPAR-gamma from the cytoplasm to the cell nucleus and a conformational change in PPAR-gamma.

This modification permits the recruitment of the co-activator, vitamin D3 receptor-interacting protein (DRIP), which interacts directly with PPAR-gamma.

Heterodimerization with the retinoid X receptor (RXR) occurs, resulting in formation of the PPAR-RXR complex, a transcriptional regulator.

The PPAR-RXR heterodimer controls transcription by binding a regulatory PPAR-gamma response element (PPRE) and modulating the expression of genes involved in the inflammation process.

PPAR-RXR downregulates the nuclear factor kappa B (NF-kappaB) and mitogen-activated protein kinase (MAPK) to reduce production of pro-inflammatory cytokines.

This complex also reduces COX-2 activity, leading to a reduction in prostaglandins involved in inflammation.

Novel agents with similar mechanisms to 5-ASA, but which target PPAR-gamma more efficiently and report a reduction of adverse events, are currently under investigation.

For example the compound, GED-0507-34, exhibits a 100- to 150-fold greater anti-inflammatory effect than 5-ASA.

A new generation of 5-ASA, balsalazide, is able to bypass the small intestine and release a high concentration of 5-ASA in the colon.

Abstract: Subcutaneous sumatriptan is an effective treatment for pain from acute migraine headache, and can be used in patients with known migraine syndrome and in patients with primary headaches when secondary causes have been excluded. In limited comparative trials, subcutaneous sumatriptan performed in a manner comparable with oral eletriptan and intravenous metoclopramide, was superior to intravenous aspirin and intramuscular trimethobenzamide-diphenhydramine, and was inferior to intravenous prochlorperazine for pain relief. The most common side effects seen with subcutaneous sumatriptan are injection site reactions and triptan sensations. As with all triptans, there is a risk of rare cardiovascular events with subcutaneous sumatriptan and its use should be limited to those without known cerebrovascular disease and limited in those with known cardiovascular risk factors and unknown disease status. In studies of patient preference and tolerability, the subcutaneous formulation has a faster time of onset and high rate of efficacy when compared with the oral formulation, but the oral formulation appears to be better tolerated. It is important to consider the needs of the patient, their past medical history, and what aspects of migraine treatment are most important to the patient when considering treatment of acute migraine or primary headache. Subcutaneous sumatriptan is a good first-line agent for the treatment of pain from acute migraine headaches and primary headaches.

We are lucky to live in a time with modern medications to treat, control, and even cure diseases or injuries. But risks come with pharmaceuticals in the form of side effects. Side effects vary from medication to medication and person to person. They can be mild like a headache or extreme like a stroke. Some side effects show up immediately and others may not manifest themselves for months or even years.

If you are a veteran who is experiencing adverse symptoms caused by medications taken to address a service-connected injury or illness, you may be eligible for additional VA disability compensation for a secondary service-connected condition. All too often, veterans applying for disability benefits overlook secondary disabilities and miss out on much-needed benefits they are entitled to.

A secondary service-connected condition is when an established medical condition that was either caused or aggravated by an incident or event during a person’s time in the military leads to a new and separate condition.

To be granted secondary-service connected benefits, you must clearly establish a link between your primary service-connected condition and your secondary condition to be granted additional disability benefits. This link is known as the nexus. Your private doctor or VA doctor will need to provide the VA with a nexus letter which is a medical opinion linking your secondary condition to your already service-connected disability. Any medical records that support a connection between those conditions are also worth submitting to the VA. The nexus between your primary condition and secondary condition must be clearly established in order to be granted service-connection on a secondary basis.


Service-connected disabilities number in the thousands and many of them are treated with both prescription and over-the-counter medications. Here are a few examples of medications that lead to secondary VA claims for adverse side effects. Antibiotics can cause kidney damage. The many different medications prescribed to treat PTSD can cause side effects that can be just as incapacitating as PTSD itself. Anxiety and cardiovascular issues are just two of the many secondary conditions that can be caused by these meds. If you developed high blood pressure during your time in service, you may have been prescribed blood pressure medication which can cause side effects like headaches or digestive issues.

The VA rates secondary disabilities the same way it rates primary disabilities using the VA’s Schedule for Rating Disabilities. The rating for your secondary disability will be determined by the VA based on the medical evidence you provide and, in some cases, by the results of a Compensation and Pension Exam and then combined with the rating for your primary disability.

The new European Academy of Neurology guidelines on the treatment of cluster headache, published in October 2023, are based on existing evidence gathered over the last 20 years of research, although this is still a field of research that needs significant development. Primary recommendations for acute cluster headache treatment are the use of 100% oxygen with a flow of at least 12 L/min over 15 min and 6mg subcutaneous sumatriptan or intranasal zolmitriptan. The use of oral agents as treatments for acute cluster headache has proved ineffective due to slow uptake being impractical for the short-lasting attacks. Verapamil is recommended as a preventive treatment in cluster headache, based on strong efficacy evidence. Corticosteroids can be used as a transitional therapy or an additive while waiting for verapamil to take effect. The use of corticosteroids in chronic cluster headache is limited by the development of severe side effects.

By using inhalers correctly and having good asthma control, we may actually reduce the need to use "steroid" tablets such as prednisolone, which contain a much higher dose than inhalers.

Have you used Symbicort Turbuhaler correctly? It may be difficult to know sometimes as the medication powder contained in the device generally doesn't have a taste. Normally if the inhaler technique is correct, the medication should be delivered to the deeper parts of the lungs where it is needed. Symbicort Turbuhaler is a dry powder inhaler (DPI), meaning that we need to empty the lungs first before inhaling forcefully and deeply in order to generate the required air flow that will carry the medication deep within the airways. Tips to prevent voice hoarseness caused by inhaler use. Many patients complain of a hoarse voice when using inhalers, especially those which contain corticosteroids (such as Symbicort, Seretide, Foster etc.). These inhaler treatments are beneficial for controlling conditions such as asthma, however a hoarse voice can develop in some patients.

Often, there are 2 causes which are often overlooked:

1. Incorrect inhaler technique - many patients were never shown the correct inhaler technique for their prescribed device. There are many inhalers available and using them correctly can be confusing. Incorrect use causes the medication to deposit at the back of the throat, which in time causes irritation and a hoarse voice. I recommend to always ask your doctor or healthcare professional for a demonstration on how to use any newly prescribed inhaler and to regularly check your technique during consultations. Breathing in the medication correctly from the inhaler is specific to each device, however can be generally classified in two broad categories: A) slow and deep breath for MDI inhalers (which produce a puff on their own) and B) strong/powerful and deep breath in for DPI inhalers (which contain a dry powder, therefore require more force to effectively draw the medication deep into the lungs).

2. Not rinsing the back of the throat after taking the dose - This is often overlooked and a very simple way to prevent inhaler-induced voice hoarseness. Especially with corticosteroid inhalers, removing any residual medication from the back of the throat and mouth is very helpful. You can do this by simply taking some water in your mouth, swishing it around, gargling for a few seconds and spitting it out. I highly recommend doing this after using your inhaler. The medication only needs to be in the airways deep in the lungs.

Are you taking too much Synthroid? Synthroid is the most commonly prescribed medication in the US which means that millions of people are taking this medicine.

Of those millions, there will always be a small percentage who are either taking too much or too little. We are going to focus on those people who are taking too much.

Symptoms of taking too much Synthroid include...

Common but less serious side effects:

Increased hair loss
Weight loss (sometimes but not always)
Tremors in the extremities
Jittery sensation
Heart palpitations

Flushing of the skin

5 big reasons why Synthroid isn't helping you feel better. Synthroid is a VERY common low thyroid medication prescribed to treat hypothyroidism but only works if used correctly.

Are you taking the thyroid medication known as Synthroid? By the way, the information we are about to talk about ALSO applies to levothyroxine!

If so, then you should be aware that there are a number of reasons why this medication may NOT be working for you in your body.

Levothyroxine and Synthroid are some of the most commonly prescribed medications in the United States with levothyroxine making the #1 spot in 2016.

This means that there are a TON of people using these medications.

The problem with that is that most people using them are not actually getting the benefit of the medication for a number of issues.

We are going to talk about 5 REASONS that many people using Synthroid are NOT feeling well (and what you can do about it).

#1. Your dose.

The first, and probably most common, issue is that most people are simply taking an insufficient or low dose. If your dose is too low then you simply won't feel any better. Thyroid medication is used to SUPPLEMENT low levels of thyroid hormone in your body. You MUST be using the right dose if you want to feel better and you are most likely underdosed if you basing your dosing off of your TSH.

#2. Absorption.

It doesn't matter if you are taking Synthroid or levothyroxine if it isn't being absorbed into your body! You can take as much medication as you want but it's ineffective unless it makes it through your intestinal lining. Many gut-related issues such as irritable bowel syndrome, the use of acid blockers, and gut inflammation can LIMIT this absorption.

If you have any of these issues be sure that you take care of them if you want your medication to be effective.

#3. Conversion issues.

Next is something called peripheral thyroid conversion. In order for your Synthroid to work, it MUST be converted into the ACTIVE thyroid hormone known as T3.

What people don't realize is that up to 20% of the population has problems with this conversion process.

#4. The wrong medication.

There are some people in which Synthroid and levothyroxine simply will NOT work. This is because they can't convert the medication or they need a medication with fewer fillers or dyes. Either way, you should understand that Synthroid may not be the answer you are looking for and there are MANY other thyroid medications available.

#5. You're taking it wrong.

Ever wonder what is actually in levothyroxine? This medication is one of the most prescribed medications in the United States and yet so many people have no idea what they are actually consuming every day.

Understanding what's in this medication is important because some of the ingredients may be causing you problems.

When it comes to thyroid medications there are two types of ingredients you should be aware of:

1. Active ingredients
And 2. Inactive ingredients

The active ingredients are usually not the problem for most thyroid patients because this component is dose dependent. You are either taking enough or you aren't.

Pregabalin, sold under the brand name Lyrica among others, is an anticonvulsant and anxiolytic medication used to treat epilepsy, neuropathic pain, fibromyalgia, restless leg syndrome, and generalized anxiety disorder. Pregabalin 75 use in epilepsy is as an add-on therapy for partial seizures. When used before surgery, pregabalin reduces pain but results in greater sedation and visual disturbances. It is taken by mouth.
Common side effects include headache, dizziness, sleepiness, confusion, trouble with memory, poor coordination, dry mouth, problem with vision, and weight gain. Serious side effects may include angioedema, drug misuse, and an increased suicide risk. When pregabalin is taken at high doses over a long period of time, addiction may occur, but if taken at usual doses the risk is low. Use during pregnancy or breastfeeding is of unclear safety. Pregabalin is a gabapentinoid and acts by inhibiting certain calcium channels.
Seizures
Lyrica 75 is useful when added to other treatments, when those other treatments are not controlling partial epilepsy. Its use alone is less effective than some other seizure medications. It is unclear how it compares to gabapentin for this use.

Neuropathic pain
The European Federation of Neurological Societies recommends lyrica 75 as a first line agent for the treatment of pain associated with diabetic neuropathy, post-herpetic neuralgia, and central neuropathic pain.

Pregabalin's use in cancer-associated neuropathic pain is controversial; though such use is common. There is no evidence for its use in the prevention of migraines and gabapentin has also been found not to be useful. Pregabalin 75 has been examined for the prevention of post-surgical chronic pain, but its utility for this purpose is controversial.

Pregabalin is generally not regarded as efficacious in the treatment of acute pain.
Anxiety disorders
Pregabalin is moderately effective and is safe for treatment of generalized anxiety disorder. It appears to have anxiolytic effects similar to benzodiazepines with less risk of dependence.

The effects of pregabalin appear after one week of use and are similar in effectiveness to lorazepam, alprazolam, and venlafaxine, but pregabalin has demonstrated superiority by producing more consistent therapeutic effects for psychosomatic anxiety symptoms.
