Rosuvastatin is a synthetic lipid-lowering medication belonging to the drug class of statins, also known as HMG-CoA reductase inhibitors.
It is one of the most potent statins used to lower cholesterol levels and prevent cardiovascular complications in susceptible individuals.
Major Indications for rosuvastatin therapy include the following.
For hyperlipidemia and mixed dyslipidemia to reduce blood cholesterol and triglyceride levels.
For the primary prevention of cardiovascular disease in high-risk patients.
And for the secondary prevention in patients with established cardiovascular disease, especially following myocardial infarction.
Rosuvastatin works by inhibiting the enzyme HMG-CoA reductase, which catalyzes the rate limiting step in cholesterol synthesis pathway.

Increased LDL receptor expression on liver cells, enhancing clearance of circulating LDL cholesterol from the blood.

Rosuvastatin is taken orally, typically in the evening or at night, because the synthesis of cholesterol is higher at night.

A small amount is excreted in urine.
Major Adverse Effects of rosuvastatin include the following.
Musculoskeletal effects such as Myalgia, or muscle pain, myopathy, and very rarely, rhabdomyolysis.

Gastrointestinal effects such as nausea, abdominal pain, or diarrhea.
Neurological effects such as headache, dizziness, or memory issues.
Hypersensitivity reactions such as rash, pruritus, or angioedema.
It is important to note that statins are usually well-tolerated by most individuals and only a minority of patients experience these adverse effects.
Contraindications for rosuvastatin therapy include the following.
Hypersensitivity to rosuvastatin or its components.
Active liver disease or unexplained persistent elevations in liver transaminases.

What is Rybelsus?


Rybelsus tablets (semaglutide) are used for adults with type 2 diabetes mellitus to control blood sugar levels. Rybelsus has also been shown in studies to help weight loss, but it is not an FDA-approved weight loss medicine. Rybelsus tablets are a once-a-day tablet that is used along with diet and exercise.

Rybelsus tablets work to lower blood sugar (glucose) levels by increasing how much insulin is released and decreasing glucagon secretion. This medicine also may slow gastric emptying after eating. Rybelsus is from the class of medicines called GLP-1 receptor agonists (glucagon-like peptide-1 receptor agonist).

Rybelsus tablets contain the same active ingredient as Wegovy injection and Ozempic injection. Wegovy is approved for weight management, and Ozempic is approved for type 2 diabetes.

Rybelsus is not for treating type 1 diabetes.


How to use Rybelsus?


Take Rybelsus exactly as prescribed by your doctor by following all directions on your prescription label and read all medication guides or instruction sheets. Rybelsus is usually started at a low dose that is gradually increased every 4 weeks to 30 days. Follow your doctor's dosing instructions very carefully.

Rybelsus tablets are taken by mouth, usually once per day.
Take Rybelsus when you first wake up and at least 30 minutes before you eat or drink anything.
Take the tablet with a small amount of water (no more than 4 ounces).
Swallow the tablet whole and do not crush, chew, or break it.
After 30 minutes of taking the tablet you may eat, drink or take other oral medicines.

Read and follow all Instructions for Use. Ask your doctor or pharmacist if you need help.

Do not use different brands of semaglutide at the same time.

Blood sugar can be affected by stress, illness, surgery, exercise, alcohol use, or skipping meals.

Low blood sugar (hypoglycemia) can make you feel very hungry, dizzy, irritable, or shaky. To quickly treat hypoglycemia, eat or drink hard candy, crackers, raisins, fruit juice, or non-diet soda. Your doctor may prescribe glucagon injection in case of severe hypoglycemia.

Tell your doctor if you have frequent symptoms of high blood sugar (hyperglycemia) such as increased thirst or urination. Ask your doctor before changing your dose or medication schedule.

Your treatment may also include diet, exercise, weight control, medical tests, and special medical care.

You may get dehydrated during prolonged illness. Call your doctor if you are sick with vomiting or diarrhea, or if you eat or drink less than usual.

Store Rybelsus in the original package at room temperature, away from moisture and heat. Keep out of the reach of children and pets.


Dosing information.


Usual Adult Dose for Diabetes Type 2:

Initial dose: 3 mg taken orally once a day for 30 days, then increase to 7 mg taken orally once a day.
Titration: If additional glycemic control is needed after receiving 7 mg/day for at least 30 days, the dose may increase to 14 mg orally once a day.
Maintenance dose: 7 to 14 mg orally once a day.
Maximum dose: 14 mg/day (taking two 7 mg tablets to achieve a 14 mg dose is not recommended).


Rybelsus side effects.


Get emergency medical help if you have signs of an allergic reaction to Rybelsus: hives, itching; dizziness, fast heartbeats; difficult breathing; swelling of your face, lips, tongue, or throat.

Call your doctor at once if you have:

vision changes;

unusual mood changes, thoughts about hurting yourself;

pounding heartbeats or fluttering in your chest;

a light-headed feeling, like you might pass out;

signs of a thyroid tumor - swelling or a lump in your neck, trouble swallowing, a hoarse voice, feeling short of breath;

symptoms of pancreatitis - severe pain in your upper stomach spreading to your back, nausea with or without vomiting, fast heart rate;

gallbladder problems - upper stomach pain, fever, clay-colored stools, jaundice (yellowing of the skin or eyes);

low blood sugar - headache, hunger, weakness, sweating, confusion, irritability, dizziness, fast heart rate, or feeling jittery;

kidney problems - swelling, urinating less, feeling tired or short of breath; or

stomach flu symptoms - stomach cramps, vomiting, loss of appetite, diarrhea (may be watery or bloody).

Common Rybelsus side effects may include:

low blood sugar (in people with type 2 diabetes);

upset stomach, heartburn, burping, gas, bloating;

nausea, vomiting, stomach pain, loss of appetite;

diarrhea, constipation;

runny nose or sore throat;

stomach flu symptoms; or

headache, dizziness, tiredness.

Salbutamol is a short-acting beta 2 adrenergic receptor agonist, used to treat various airway diseases such as asthma, chronic obstructive pulmonary disease, and to prevent exercise-induced bronchospasms.
Routes of administration include oral, subcutaneous, intravenous, and inhalational.
Out of these, the most effective one is the inhalational route since it significantly reduces the systemic distribution of the drug.
Beta 2 receptors are predominantly located in airway smooth muscle cells. In addition, they can be found in the cardiac muscle, uterine smooth muscle cells, blood vessels, skeletal muscle, and in various immune cells.
Since the drug is mainly used for airway conditions, let’s focus on its mechanism of action in bronchial smooth muscle cells.
Following administration, salbutamol binds to the beta 2 adrenergic receptors on bronchial smooth muscle cells and activates them.
Beta 2 adrenergic receptor is a G protein coupled receptor, and activation of this receptor leads to the activation of an enzyme called adenylyl cyclase, which increases the cyclic AMP levels within the cell.
Increased cyclic AMP causes reduction in intracellular calcium levels, which ultimately relaxes the smooth muscle.
Through this mechanism, salbutamol causes relaxation of smooth muscles of all airways, from the trachea to terminal bronchioles.
In addition, increased cyclic AMP levels will inhibit the release of inflammatory mediators by the mast cells, which ultimately reduces inflammation.
Salbutamol is metabolized by the liver and excreted via the kidneys.
Some common adverse effects of salbutamol include the following.
Tremors.
Palpitations, or noticeable heartbeat.
Nausea and vomiting.
Headache.
Muscle and bone pain.
And allergic reactions.
Finally, it is important to know that these adverse effects occur much less frequently when salbutamol inhalers are used.

A largescale new study offers a new approach to treating in type 2 diabetes - that puts patients in charge of their own medication. The Trimaster study is the first to allow people with type 2 diabetes to choose their own medication, after trying three different drugs in succession. It concluded the approach could be a new way of finding the best treatment.
Researchers at the University of Exeter led the Medical Research Council-funded study, which gave people with type 2 diabetes three commonly-prescribed drugs, each for a 16-week period in turn. During the study, researchers monitored the effect of each drug on the 448 patients’ glucose levels, weight and recorded the side effects. At the end of the study, now published in Nature Medicine, patients got to choose the drug that worked best for them. Their chosen drug not only lowered glucose most effectively, but also resulted in less side effects.
Lead author Dr Beverley Shields, of the University of Exeter, said: “Getting the right treatment for diabetes is fundamental to getting the best outcomes, and maintaining good quality of life. Our study is the first to invite people with type 2 diabetes to try common drugs in succession, to see which one works best for them. Interestingly, we found that the treatment people chose was usually the one which gave them best blood sugar control – even before they knew those results.”

“I was given three drugs. The first two didn’t work for me at all – in fact, one of them made me feel even worse. Thankfully, from the first moment I took the third drug, I felt different – I had more energy, and I knew it was the right drug for me. I’m still on it today and I’m able to engage in all the sports I love – it’s fantastic.”
Type 2 diabetes affects more than four million people in the UK, and is a major cause of illness and death. It also accounts for 10 per cent of NHS expenditure . The problems associated with diabetes can be greatly reduced if the blood sugar levels are lowered. A number of different drugs work to reduce blood sugar, and while overall they are similarly effective, individual patients will vary in terms of how much the drug lowers their blood sugar and the side effects they have. Choosing the right drug for a patient is difficult.
Diabetes consultant Professor Andrew Hattersley CBE, of the University of Exeter, who oversaw the research, said: “This is the first study in which the same patient has tried three different types of glucose-lowering drug, enabling them to directly compare them and then choose which one is best for them. We’ve shown that going with the patients’ choice results in a better glucose control and less side effects than any other approach. When it’s not clear which drug is best to use, then patients should try before they choose. Surprisingly, that approach has never been tried before.”
