Corticosteroids are a class of drugs based on hormones formed in the adrenal gland or derived synthetically. They are created from cholesterol and divided into two major groups: glucocorticoids and mineralocorticoids. Common glucocorticoids are cortisol (hydrocortisone), prednisolone, dexamethasone, and corticosterone. An important mineralocorticoid in humans is aldosterone. Corticosteroids influence a wide range of physiological processes, including stress response, immune response, regulation of inflammation, carbohydrate metabolism, protein catabolism, blood electrolyte levels, and behaviour. The secretion of these hormones increases during stress related to anxiety and severe injury. Corticosteroids are often prescribed for severe allergies, skin problems, asthma, and arthritis. They are also used to help prevent organ rejection in transplant recipients. Chronic overproduction of these substances is associated with various disorders, such as Cushing′s syndrome

Your Medication Memantine

Memantine is also known by the brand name: Namenda.
Memantine comes in 5 and 10 mg tablets, as well as 7, 14, 21, and 28mg extended-release capsules.
Memantine is most commonly used for treating dementia and Alzheimer’s disease. It can be taken by mouth with or without food. If you are taking a long-acting product it needs to be swallowed whole. Do not take any capsules that do not look normal or are damaged.
Standard dosing of memantine ranges from 5 mg up to 28mg.
Some common side effects with memantine include dizziness, headache, or hard stools.
Some less common side effects with memantine include confusion, anxiety, and vomiting.
If you miss a dose of memantine:
• Skip the missed dose and go back to your normal time.
• Do not take 2 doses at the same time or extra doses.
• If you miss taking this drug for a few days in a row, call your doctor before you start taking it again.

When taking memantine, remember it is important to tell your doctor or pharmacist if you have:
• Signs of an allergic reaction, like rash; hives; itching; blistered, or peeling skin with or without fever
• Mood changes
• Low mood (depression)
• Change in the way you act
• Very bad dizziness or passing out
• Seizures
The best way to store memantine:
• Store at room temperature.
• Store in a dry place. Do not store in a bathroom or refrigerator.
• Keep all drugs in a safe place. Keep all drugs out of the reach of children and pets.
• Check with your pharmacist about how to throw out unused drugs.

- Memantine is N-Methyl-D-Aspartate receptor antagonist. Continuous activation of the N-methyl-D-aspartate (NMDA) receptors in the central nervous system caused by glutamate is thought to cause some of the Alzheimer's disease symptoms.

This overactivation is thought to contribute to neurotoxicity due to the excitatory properties of glutamate.

Memantine is a NMDA antagonist that goes by the brand name Namenda. There are also titration packs available in the name brand. Memantine treats but does not cure confusion and dementia associated to Alzheimer’s disease. There are multiple dosage forms being a capsule, solution, and tablet. Special considerations are for patients with renal and hepatic impairment. Memantine does appear to have higher exposure in women than men. Memantine is purposed to work on the glutamate receptor blocking the receptor much like magnesium does under “normal” conditions. This drug has a long elimination half-life between 60-80 hours. The main side effects are weight gain, abdominal pain, constipation, diarrhea and vomiting. A serious side effect is Stevens Johnson Syndrome. Two main types of drugs that interact with Namenda are alkalinizing agents and carbonic anhydrase inhibitors. The drug can be taken with or without food and if a dose is missed it should not be doubled on the next dose. The missed dose should be skipped and resumed regularly.

In this video Doctor O'Donovan explains how to use DONEPEZIL (aka Aricept) a medication used in the treatment of DEMENTIA (e.g. Alzheimer's disease, dementia with Lewy bodies, vascular dementia etc)

WHAT IS DONEPEZIL?

Donepezil is a medicine that helps with some types of dementia.

It does not cure dementia. However, it treats some of the symptoms of Alzheimer's disease, Parkinson's disease or dementia with Lewy bodies.

It can also help with "mixed dementia". This is when you have more than one type of dementia.

Donepezil is available on prescription only.

It comes as tablets, including tablets that melt in your mouth, and as a liquid that you drink.

1. Donepezil can help with symptoms like being forgetful or confused.
2. The most common side effects of donepezil are diarrhoea, headache and feeling sick (nausea).
3. You can take it with or without food.
4. Drinking alcohol stops donepezil from working as well as it should. It also increases the risk of side effects.

Well it was bound to happen eventually. After recording & editing episode 58 it was brought to my attention that memantine was already covered back in episode 19! I guess here is the new version? Sorry everybody.

EPISODE #19 Summary:  Memantine is a NMDA antagonist that goes by the brand name Namenda. There are also titration packs available in the name brand. Memantine treats but does not cure confusion and dementia associated with Alzheimer's disease. There are multiple dosage forms being a capsule, solution, and tablet. Special considerations are for patients with renal and hepatic impairment. Memantine does appear to have higher exposure in women than men. Memantine is proposed to work on the glutamate receptor blocking the receptor much like magnesium does under “normal” conditions. This drug has a long elimination half-life between 60-80 hours. The main side effects are weight gain, abdominal pain, constipation, diarrhea and vomiting. A serious side effect is Stevens Johnson Syndrome. Two main types of drugs that interact with Namenda are alkalinizing agents and carbonic anhydrase inhibitors. The drug can be taken with or without food and if a dose is missed it should not be doubled on the next dose. The missed dose should be skipped and resumed regularly.
