Narrator: When you go to sleep, if I pinched you, you'd be up. If I shook you, you'd be up, right? But under anesthesia, I'm gonna pinch you and do a full operation and you're not up. So it's really further on the spectrum of unconsciousness.
Narrator: When you wake up after being put under with general anesthesia you barely feel like any time has passed. You could have been out for an hour or a day and you wouldn't know the difference.
Fong: When you go to a natural sleep, people call your name, your alarm goes off, you wake up, right? This is not what is gonna happen during general anesthesia. You're gonna be unconscious.
Narrator: You're closer to being in a coma than being asleep.
Anesthesia was first used during surgery in 1846. The drug provided at that time was ether. Now anesthesiologists more commonly use a combination of drugs like propofol and fentanyl which interrupt neural pathways so you don't feel pain and you don't remember the surgery.
Fong: Three things that you need for general anesthesia are you need amnesia so that they don't remember, analgesia so they have pain relief and then operating conditions for the surgeon. Some surgeries you need the patient to be very relaxed so you would use a muscle relaxant. Other surgeries the patient just needs to be asleep and anesthetized but they don't need relaxation so how they do that varies upon the different medications that you're using. Some will depress excitatory neurons and some will enhance inhibitory neurons.
Narrator: Excitatory neurons, for example, get excited and send signals to other neurons to fire. Depressing them means less signals telling your brain you're in pain. Inhibitory neurons do the opposite. They make it harder for neurons to generate these electrical signals. In either case this means fewer active neurons overall which is important because when your body is being poked and prodded, neurons would typically fire to tell your brain you're in pain. If those neurons aren't firing, your brain doesn't know that your body is, well, being cut open.
Fong: Basically it interrupts the pathways and the communication between your neural networks. We're aiming for them to be not in pain by looking at their vital signs, their heart rate, their blood pressure. Then we want to make sure that they're unconscious.
Narrator: Without anesthesia, many important surgeries wouldn't be possible because they'd be way too traumatic.
Fong: Surgery didn't move forward, really, until anesthesia moved forward. You know, you watch those old movies. They give you a swig of alcohol, they put a tourniquet and they hack your leg off. People don't do well with that, right? If you had a bad heart, that would be the end of that.
Narrator: After the procedure is complete the doctors stop administering the meds and the most powerful effects of the drugs wear off but even though you're conscious again you might continue to experience some of the drugs side effects.

In this video we will cover everything you need to know about Pantoprazole, also known as Protonix. We will discuss how and when to use it, we will cover its side effects, dosage and much, much more.
Generic name: Pantoprazole.
Brand names: Protonix.
Available as a tablet and as an injection.
How to use it?
- Tablet: swallow it whole with half a glass of water. These tablets are covered with a protective film, therefore you should not break them or bite on them. As this will decrease their effectivity.
- Injection: your doctor or nurse will place the injection. Usually you do not need to do this yourself.
Here it is important to mention you can take pantoprazole at any time of the day, but it is best to do so at fixed times. This will make you less likely to forget a dose.
When to use it?
Pantoprazole is part of a group of drugs called, proton-pump inhibitors. These drugs selectively inhibit the production of stomach acid. Therefore Pantoprazole can be used to treat:
- Heartburn also called Gastroesophageal reflux disease.
- An inflammation of your gullet/esophagus.
- An ulcer in your stomach or intestines
- Infection of the H. Pylori bacteria.
What is the correct dosage?
The precise dose will depend on your specific indication of use, the side effect you might experience and the severity of your symptoms. Therefore always use the precise amount and frequency your doctor prescribed you.
- However, when treating GERD your doctor might prescribe a dose of 40mg 1x day for 4-8 weeks. If necessary this dose can be increase to 40mg 2x per day.
- For the treatment of an ulcer in your stomach or intestines your doctor might prescribe the same dose of 40mg 1x day for 4-8 weeks. Which again, can be increase to 40mg 2x per day if necessary.
- And when treating an H. Pylori infection your doctor might prescribe you 40mg 2x per day for 1-2 weeks, in combination with several antibiotics.
What are the side-effects?
- Common (1-10%): gastric polyps.
Uncommon (0.1-1%): sleep disturbances, headache, dizziness, diarrhea, nausea, vomiting, bloating, constipation, dry mouth, abdominal pain, skin rash, itching, fatigue and fractures.
Rare (0.01-0.1%): hypersensitivity, weight change, depression, visual impairment, taste disorder, elevated body temperature and edema.
Very rare (0.01%): changes in your blood values.
Safety:
First of all, pantoprazole can cause dizziness or blurry vision. If you experience any of these, then please do not drive as this could be dangerous.
Furthermore, combining pantoprazole with alcohol, can increase its side effects. So take proper care when doing so.
In addition, pantoprazole can have interactions with other drugs your are taking. always discuss this with your prescribing doctor.
Lastly, the available medical evidence shows that pantoprazole seems to be safe to use for pregnant women and women who are breastfeeding, however there are some exceptions. Therefore always discuss this with your doctor.

What Is Pantoprazole?
Pantoprazole, sold under the brand name Protonix, among others, is a proton pump inhibitor used for the treatment of stomach ulcers, short-term treatment of erosive esophagitis due to gastroesophageal reflux disease (GERD), maintenance of healing of erosive esophagitis, and pathological hypersecretory conditions including Zollinger–Ellison syndrome. It may also be used along with other medications to eliminate Helicobacter pylori. Effectiveness is similar to other proton pump inhibitors (PPIs). It is available by mouth and by injection into a vein.
Side Effects of Pantoprazole?
Common side effects include headaches, diarrhea, vomiting, abdominal pain, and joint pain. More serious side effects may include severe allergic reactions, a type of chronic inflammation known as atrophic gastritis, Clostridium difficile colitis, low magnesium, and vitamin B12 deficiency. Use in pregnancy appears to be safe. Pantoprazole is a proton pump inhibitor that decreases gastric acid secretion. It works by inactivating (H+/K+)-ATPase function in the stomach.
Study of pantoprazole began in 1985, and it came into medical use in Germany in 1994. It is available as a generic medication. In 2019, it was the sixteenth most commonly prescribed medication in the United States, with more than 28 million prescriptions.

Pantoprazole belongs to a class of drugs known as proton pump inhibitors, or PPIs. It works by reducing the production of acid in the stomach. Acid is produced by special cells called parietal cells in the stomach lining. Excess acid can cause various problems, including acid reflux, heartburn, and stomach ulcers.
It's important to note that while Pantoprazole can be highly effective in treating these conditions, it is essential to follow your healthcare provider's instructions and take the medication as prescribed. Like any medication, Pantoprazole may have potential side effects, although they are generally rare.

If you’ve taken an over-the-counter decongestant in the past 20 years, you’ve probably taken something with phenylephrine as the main active ingredient. A Food and Drug Administration panel recently recommended that it be taken off shelves because it doesn’t work any better than a placebo. What happened?

Decongestant nasal sprays are common over-the-counter medications to open up clogged nasal passages and alleviate nasal congestion. Examples of popular decongestant nasal sprays include afrin, zicam, sinex, neo-synephrine, etc. Decongestant nasal sprays accomplish symptom relief by chemically constricting the blood vessels thereby reducing the blood flow into the nose. With reduced blood flow, any mucosal swelling decreases alleviating congestion and improving nasal airflow. After a period of time, the medication wears off allowing the blood vessels to open back up and increasing blood flow into the nose again. Unfortunately, increased blood flow means the nasal mucosa will swell again causing recurrence of congestion and reduced nasal airflow. As such, the decongestant nasal spray will often be used again to alleviate nasal symptoms. Problem is that with prolonged chronic daily use, the need to continue and more frequently use this nasal spray will be required in order to achieve symptom relief. In a sense, the nose becomes addicted to the nasal spray.
This condition is known as rhinitis medicamentosa or rebound congestion.
Please note that saline, steroid, and anti-histamine nasal sprays are safe to use daily for prolonged periods of time (ie, flonase, nasacort, rhinocort, astepro, etc). It's ONLY the decongestant nasal sprays that are not safe to use for more than 3 consecutive days (can repeat after 3 weeks break).
