The Na+/K+/2Cl- transporter protein, which is found in the walls of the ascending loop of Henle, is inhibited by these medications. These substances decrease salt or NaCl absorption, which dramatically boosts diuresis and increases urine output as a result.
But here is a catch, Blocking of the NKCC2 cotransporter makes potassium unable to be reabsorbed back into the lumen, which results in the loss of calcium and magnesium ions.
A secondary effect of loop diuretics is to increase the production of prostaglandins, which results in vasodilation and increased blood supply to the kidney.
The collective effects of decreased blood volume and vasodilation decrease blood pressure and ameliorate edema.
Several loop diuretics come in IV and oral forms.
Furosemide
Furosemide comes in oral tablet form in 20, 40, and 80 mg dosages. Injectable solutions come in 10 mg/mL doses. Oral solutions come in either 8 or 10 mg/mL doses.
The half-life for furosemide is 1.5 to 2 hours but can be up to 2.6 hours in those with renal/hepatic dysfunction or heart failure.
Torsemide comes in tablet form in 5, 10, 20, or 100 mg doses. The injectable solution is 10 mg/mL dosing. Torsemide is known to have the longest half-life at 3 to 4 hours and can be as long as 5 to 6 hours in patients with renal/hepatic dysfunction or heart failure.
Bumetanide comes in oral tablets of 0.5, 1, and 2 mg. The IV solution is 0.25 mg/mL. Bumetanide has a half-life of 1 hour and can be administered for 1.3 to 1.6 hours in those with renal/hepatic dysfunction or heart failure.
All three loops typically have a similar onset of action. Oral administration time between the three averages 30 to 60 minutes.

Adverse effects of loops Diuretics typically occur from electrolyte imbalances secondary to the diuresis effects, which include: hyponatremia, hypokalemia, hypochloremia, hypomagnesemia, metabolic alkalosis, prerenal azotemia, and dehydration.
The FDA has approved loop diuretics for treating hypertension alone or with the addition of other anti-hypertensives. However, loop diuretics alone are not used for first-line therapy.

Tranexamic acid is a synthetic derivative of lysine, classified as an antifibrinolytic agent.
It works by inhibiting the breakdown of fibrin clots, thereby promoting hemostasis.
It is commonly used to reduce bleeding in various clinical settings, including the following.
Trauma-related bleeding.
Surgical bleeding prevention in various types of surgeries, including Cardiothoracic surgeries, Orthopedic surgeries, Obstetric, and gynecologic surgeries.
In the treatment of heavy menstrual bleeding.
Hereditary angioedema.
Liver disease.
Hemophilia adjunct therapy during dental extractions.
And in the treatment of disseminated intravascular coagulation.
Tranexamic acid inhibits fibrinolysis by Competitive Inhibition of Plasminogen Activation.
It binds reversibly to lysine-binding sites on plasminogen, preventing its activation to plasmin.
Plasmin is the enzyme responsible for breaking down fibrin clots.
Tranexamic acid also weakly inhibits plasmin activity, further reducing fibrinolysis.
By stabilizing fibrin clots, it reduces bleeding without directly affecting platelet function or coagulation factors.
Tranexamic acid can be administered through various routes.
Intravenous route allows rapid onset of action and is commonly used in acute settings, such as trauma or perioperative bleeding.
Oral tranexamic acid is used for conditions like heavy menstrual bleeding or prophylaxis in hereditary angioedema.
It has a low bioavailability of approximately 30 to 50%.
Topical tranexamic acid preparations are also available and applied locally to surgical sites or wounds to control localized bleeding such as dental extractions and minor surgical procedures.
It is also available as solutions, sprays, or gels.
Intramuscular route is less commonly used.
Tranexamic acid is minimally metabolized and most of the drug is excreted unchanged via the kidneys.
The most common adverse effects of tranexamic acid include gastrointestinal disturbances such as nausea, vomiting, and diarrhea.
Dizziness, and headache.
Rare but Serious side effects include Thromboembolic events such as deep vein thrombosis, and pulmonary embolism.
Visual disturbances.
Allergic reactions such as rash, pruritus, or anaphylaxis.
And Hypotension, especially with rapid intravenous administration.
Absolute contraindications for the use of tranexamic acid include Active thromboembolic disease such as deep vein thrombosis, pulmonary embolism, or cerebral thrombosis.
Acquired defective color vision.
And history of hypersensitivity to tranexamic acid.
Relative contraindications include Renal impairment, History of thromboembolic events or clotting disorders, and Concurrent use of other antifibrinolytics or procoagulants.

Important Safety Information

Contraindications
• Tresiba® is contraindicated during episodes of hypoglycemia and in patients with hypersensitivity to insulin degludec or any of the excipients in Tresiba®

Warnings and Precautions
• Never Share a Tresiba® FlexTouch® Pen, Needle, or Syringe Between Patients, even if the needle is changed. Patients using Tresiba® vials should never share needles or syringes with another person. Sharing poses a risk for transmission of blood-borne pathogens.
• Hyperglycemia or Hypoglycemia with Changes in Insulin Regimen: Changes in an insulin regimen (e.g., insulin strength, manufacturer, type, or injection site or method of administration) may affect glycemic control and predispose to hypoglycemia or hyperglycemia. Repeated insulin injections into areas of lipodystrophy or localized cutaneous amyloidosis have been reported to result in hyperglycemia; and a sudden change in the injection site (to an unaffected area) has been reported to result in hypoglycemia. Make any changes to a patient’s insulin regimen under close medical supervision with increased frequency of blood glucose monitoring. Advise patients who have repeatedly injected into areas of lipodystrophy or localized cutaneous amyloidosis to change the injection site to unaffected areas and closely monitor for hypoglycemia. Adjustments in concomitant anti-diabetic treatment may be needed.
• Hypoglycemia: Hypoglycemia is the most common adverse reaction of insulin, including Tresiba®. Severe hypoglycemia can cause seizures, may be life-threatening or cause death. Hypoglycemia can impair concentration ability and reaction time; this may place the patient and others at risk in situations where these abilities are important (e.g., driving or operating other machinery). Hypoglycemia can happen suddenly and symptoms may differ in each patient and change over time in the same patient. Symptomatic awareness of hypoglycemia may be less pronounced in patients with longstanding diabetes, in patients with diabetic neuropathy, using drugs that block the sympathetic nervous system (e.g., beta-blockers) or who experience recurrent hypoglycemia. The long-acting effect of Tresiba® may delay recovery from hypoglycemia compared to shorter-acting insulins.
Risk Factors for Hypoglycemia: The risk of hypoglycemia generally increases with intensity of glycemic control. The risk of hypoglycemia after an injection is related to the duration of action of the insulin and, in general, is highest when the glucose lowering effect of the insulin is maximal. As with all insulins, the glucose lowering effect time course of Tresiba® may vary among different patients or at different times in the same patients and depends on many conditions, including the area of injection as well as the injection site blood supply and temperature. Other factors which may increase the risk of hypoglycemia include changes in meal pattern, changes in level of physical activity, or changes to concomitant drugs. Patients with renal or hepatic impairment may be at higher risk of hypoglycemia. Patients and caregivers must be educated to recognize and manage hypoglycemia. In patients at higher risk for hypoglycemia and patients who have reduced symptomatic awareness of hypoglycemia, increased frequency of blood glucose monitoring is recommended.
• Hypoglycemia Due to Medication Errors: Accidental mix-ups between insulin products have been reported. To avoid medication errors between Tresiba® and other insulins, always instruct patients to always check the insulin label before each injection. To avoid dosing errors and potential overdose, never use a syringe to remove Tresiba® from the Tresiba® FlexTouch® disposable insulin prefilled pen.
• Hypersensitivity Reactions: Severe, life-threatening, generalized allergy, including anaphylaxis, can occur with insulins, including Tresiba®. If hypersensitivity reactions occur, discontinue Tresiba®; treat per standard of care and monitor until symptoms and signs resolve.

Hey guys and welcome back to Mad About Skin where in todays video we are talking about Tretinoin and the reason why so may people online seem to be stopping tretinoin. From Kelly Driscoll through to Susan Yara and more, influencers are turning their backs on tretinoin but it doesnt mean you need to also.

Tretinoin is a high strength prescription only retinoid which has some amazing anti-ageing benefits for the skin. It has however some big side effects which you have to work your way through before you get to the tretinoin benefits at the other side. If your skin can tolerate tretinoin then using it consistently can increase the rate of cell turnover for smoother skin, fade hyperpigmentation and boost collagen to minimise fine lines and wrinkles. There are alternatives to tretinoin and so lets take a look at tretinoin in more detail.

How to use tretinoin for the best anti aging results and who maybe should avoid tretinoin in their skin care routine.

After using tretinoin (retin-a) for some time I decided to switch to retinal instead and now I use tretinoin 2 times per week for the best results and little sensitivity.

Triamcinolone, Triamcinolone Acetonide .

Why is this medication prescribed?

Triamcinolone, a corticosteroid, is similar to a natural hormone produced by your adrenal glands. It often is used to replace this chemical when your body does not make enough of it. It relieves inflammation (swelling, heat, redness, and pain) and is used to treat certain forms of arthritis; skin, blood, kidney, eye, thyroid, and intestinal disorders (e. g. , colitis); severe allergies; and asthma. Triamcinolone is also used to treat certain types of cancer.

This medication is sometimes prescribed for other uses; ask your doctor or pharmacist for more information.


How should this medicine be used?

Triamcinolone comes as a tablet and syrup to be taken by mouth. Your doctor will prescribe a dosing schedule that is best for you. Follow the directions on your prescription label carefully, and ask your doctor or pharmacist to explain any part you do not understand.

Do not stop taking triamcinolone without talking to your doctor.
Stopping the drug abruptly can cause loss of appetite, upset stomach, vomiting, drowsiness, confusion, headache, fever, joint and muscle pain, peeling skin, and weight loss. If you take large doses for a long time, your doctor probably will decrease your dose gradually to allow your body to adjust before stopping the drug completely.
Watch for these side effects if you are gradually decreasing your dose and after you stop taking the tablets or oral liquid, even if you switch to an inhalation. If these problems occur, call your doctor immediately.
You may need to increase your dose of tablets or liquid temporarily or start taking them again.

Take triamcinolone exactly as directed. Do not take more or less of it or take it more often than prescribed by your doctor.


What special precautions should I follow?

Before taking triamcinolone,

tell your doctor and pharmacist if you are allergic to triamcinolone, aspirin, tartrazine (a yellow dye in some processed foods and drugs), or any other drugs.

tell your doctor and pharmacist what prescription and nonprescription medications you are taking, especially anticoagulants ('blood thinners') such as warfarin (Coumadin), arthritis medications, aspirin, cyclosporine (Neoral, Sandimmune), digoxin (Lanoxin), diuretics ('water pills'), estrogen (Premarin), ketoconazole (Nizoral), oral contraceptives, phenobarbital, phenytoin (Dilantin), rifampin (Rifadin), theophylline (Theo-Dur), and vitamins.

if you have a fungal infection (other than on your skin), do not take triamcinolone without talking to your doctor.
tell your doctor if you have or have ever had liver, kidney, intestinal, or heart disease; diabetes; an underactive thyroid gland; high blood pressure; mental illness; myasthenia gravis; osteoporosis; herpes eye infection; seizures; tuberculosis (TB); or ulcers.

tell your doctor if you are pregnant, plan to become pregnant, or are breast-feeding. If you become pregnant while taking triamcinolone, call your doctor.
if you are having surgery, including dental surgery, tell the doctor or dentist that you are taking triamcinolone.

if you have a history of ulcers or take large doses of aspirin or other arthritis medication, limit your consumption of alcoholic beverages while taking this drug. Triamcinolone makes your stomach and intestines more susceptible to the irritating effects of alcohol, aspirin, and certain arthritis medications. This effect increases your risk of ulcers.


What special dietary instructions should I follow?

Your doctor may instruct you to follow a low-sodium, low-salt, potassium-rich, or high-protein diet. Follow these directions.

Triamcinolone may cause an upset stomach. Take triamcinolone with food or milk.


What should I do if I forget a dose?

When you start to take triamcinolone, ask your doctor what to do if you forget a dose. Write down these instructions so that you can refer to them later.

If you take triamcinolone once a day, take the missed dose as soon as you remember it. However, if it is almost time for the next dose, skip the missed dose and continue your regular dosing schedule. Do not take a double dose to make up for a missed one.


What side effects can this medication cause?

Triamcinolone may cause side effects. Tell your doctor if any of these symptoms are severe or do not go away:

upset stomach.
stomach irritation.
vomiting.
headache.
dizziness.
insomnia.
restlessness.
depression.
anxiety.
acne.
increased hair growth.
easy bruising.
irregular or absent menstrual periods.

If you experience any of the following symptoms, call your doctor immediately:

skin rash.
swollen face, lower legs, or ankles.
vision problems.
cold or infection that lasts a long time.
muscle weakness.
black or tarry stool.

How long does it take for #triamcinolone cream to work?

A course of treatment for seven days is usually sufficient.

If your symptoms have not improved after this time (or if they get worse), speak again with your doctor for further advice.

Topical corticosteroids like triamcinolone should not be used for long periods of time or on large areas of the body.

If you are using a moisturiser along with this preparation, apply the moisturiser first.

Then wait 10-15 minutes before applying Aureocort® ointment.

This allows time for the moisturiser to be absorbed before the topical corticosteroid is applied.

Your skin should be moist but not slippery when you apply Aureocort®.

Do not use Aureocort® on your face unless a skin specialist doctor has said you should.

If you have been told to use it on your face, be careful not to get any preparation near your eyes and do not use it for longer periods of time than you have been advised.

Unless advised to do so by your doctor, do not apply a bandage or dressing to the area being treated, as this will increase absorption of the preparation and increase the risk of side-effects.

Continue to use Aureocort® until the flare-up has gone and then stop it.
