Hyperbaric oxygen therapy (HBOT) is inherently safe for most people.

With a little training on how to equalize the pressure in their ears, most people can go right into a chamber, no problem.

But there are relative and absolute contraindications to hyperbaric oxygen therapy, and there are a handful of people that really shouldn't go in or need to be cleared by a doctor before deciding to go into a hyperbaric chamber.

In this video we’ll discuss 5 RELATIVE contraindications for HBOT.

By relative, we mean that it depends on the severity of the issue. People with these conditions may actually benefit from HBOT, but it depends on the severity, and the patient should see their related specialist to be cleared before getting into a chamber.

CINCINNATI (WKRC) - Side effects of cancer treatment can occur years later but a newer way to treat it from the inside out is showing great success.

The team at Mercy Health Fairfield Hospital says it can be life changing.

"I had to go to radiation for 50 days and five of those treatments were internal radiation," said Katheryn Sivrais.

Sivrais had radiation after a diagnosis of both breast and cervical cancer. "They tell you ahead of time, it is killing the cancer cells, but it is also going to damage good tissue."

Her surgeon Dr. James Fitzpatrick said, "She was cured of the cervical cancer, but the collateral damage was to the bladder which was right next to the cervix."

Katheryn turned to hyperbaric oxygen therapy. "It's 100 percent oxygen at a higher atmospheric pressure."

The treatment has been around for a number of years and used in general wound healing, usually for those with diabetes. But in this case, Sivrais been able to heal wounds inside her body. Hyperbaric oxygen therapy appears to reverse what's called vascular compromise that can result from radiation treatment. It is delivered two hours a day, five days a week.

"Patients will typically see improvement after 12 to 15 treatments, but we find that there's a higher success rate when they go to 40 treatments or more," said Dr. Fitzpatrick.

During treatment time patients can watch TV or movies, or take a snooze. When therapy is finished, studies show at least six out of ten have results similar to Sivrais, no more pain and complete tissue and organ healing.

There's not really a downside to this therapy but no one therapy works for everyone.

In most cases, it is covered by medical insurance plans.

There are several hospitals and health systems now in our area which offer this kind of therapy.

If you have any travel plans in the middle of your hyperbaric protocol, there are absolutely some things you need to know about when you can or cannot fly in an airplane in and around those sessions in hyperbaric medicine.

This whole conversation about flying after HBOT is all about nitrogen, specifically about the possibility of getting decompression illness (the bends).

SSRIs are the most widely-used treatment for depression, and have been since their introduction to the market in the late 1980s. They were formulated based on the hypothesis that depression is due to low levels of the neurotransmitter serotonin. In this video, I discuss how SSRIs work, along with some questions that have been raised about the serotonin hypothesis since the introduction of SSRIs.

TRANSCRIPT:

Welcome to 2 minute neuroscience, where I explain neuroscience topics in 2 minutes or less. In this installment I will discuss selective serotonin reuptake inhibitors, or SSRIs.

SSRIs were developed in the 1970s with the goal of treating depression by increasing serotonin levels. This goal was formulated based on the serotonin hypothesis of depression, which suggests that depression is caused by low levels of the neurotransmitter serotonin.

SSRIs work by inhibiting a mechanism called reuptake. In reuptake, a protein called a transporter transports excess neurotransmitter molecules out of the synaptic cleft, usually back into the neuron that released them. SSRIs inhibit the reuptake of serotonin.

By inhibiting the removal of serotonin from the synaptic cleft, this causes levels of serotonin in the synaptic cleft to rise. These increases in serotonin levels have been hypothesized to be the mechanism by which SSRIs can treat the symptoms of depression.

There are reasons now, however, to believe that there must be more to the mechanism of SSRIs than just changing serotonin levels. For example, when someone begins taking an SSRI, they generally have to wait about 4 weeks before their symptoms improve. Evidence suggests, however, that their serotonin levels rise as quickly as within an hour after taking the medication. Thus, it seems likely that SSRIs may affect serotonin levels, which then leads to other effects that influence the symptoms of depression.

Therefore it’s probable that more than serotonin levels must be changed for SSRIs to work, which suggests that depression isn’t caused only by low serotonin levels. Additionally, evidence has now emerged that questions the effectiveness of SSRIs. Some research has found they do not work much better than placebos, and may only benefit those who are most severely depressed, which is a minority of patients who actually take the drugs.

Dr. Pagnani of Rittenhouse Psychiatric Associates discusses the tapering or "deprescribing" of Psychiatric Medications including SSRIs (lexapro), SNRIs (effexor), Benzodiazepines (klonopin, xanax) and even buprenorphine. Patients who have taken medication for years, and failed at tapering or stopping these medications due to withdrawal ("brain zaps," muscle twitches, headaches, dizziness, anxiety, insomnia) may benefit from treatment with a provider specifically trained in tapering these medications. Additionally, Rittenhouse Psychiatric Associates providers can work with local compounding pharmacies to create doses in-between manufacturer made doses of these medications, to make the process of tapering or deprescribing as easy as possible. Sometimes when a patient undergoes this process, they find that the medication actually IS very helpful for their mood, anxiety or other symptoms, but they may not need the full dose that they have taken for years. This is NOT a failure, and can result in a decrease in side effects (and improved quality of life). Patients who have felt over-medicated, dull, cognitively slowed, or excessive sleepiness, may find that at a lower dose, they have an improved quality of life, and still have symptom control. Psychiatric medications can be incredibly important for an individual's care, and should never be decreased or stopped without consulting directly with a mental health professional. However, just because someone was started on a medication, or just because they have withdrawal when they try to decrease it, doesn't mean that they need to stay on it "forever."

Reaction! 🎥 This viral video is from a little while back but I wanted to share my insight while it’s #pmddawarenessmonth!⁠

Famotidine (Pepcid AC) is one example of a histamine H2-receptor antagonist, also known as an H2 blocker.⁠

H2 blockers work by reducing the release of stomach acid, but they also block histamine receptors.⁠

So let’s say Nina is suffering from PMDD symptoms like extreme anxiety and depression in her luteal phase.⁠

She tries Pepcid AC and experiences dramatic relief in the lead-up to her period (and posts about it on socials 😋).⁠

This likely means Nina is dealing with elevated histamine levels, and the H2 blocker is acting like a Band-Aid—blocking histamine receptors in the gut (and beyond) to ease her symptoms.⁠

Yay for symptom relief - I’m all about it, especially when PMDD is affecting your relationships and life plans!!⁠
