Bumetanide (sold under the registered trade name Bumex) is one of the most commonly prescribed medications (water pills or diuretics) used for treating edema (swelling) associated with heart failure, kidney disease, and liver disease. It is also used to manage high blood pressure. Here are some common side effects and important information to keep in mind:

Frequent Urination: Bumetanide works by causing your kidneys to remove excess fluid from your body, which leads to increased urination. While this can be inconvenient, it is often a desired effect to help get rid of excess fluid in conditions like heart failure or edema.

Dizziness: Dizziness can happen, especially if you stand up too quickly. This is because Bumetanide can lower your blood pressure, especially if the dose is too strong, or if you have other problems making you dehydrated, like diarrhea or vomiting. Dehydration can add to its effect and make the dizziness worse.

Electrolyte Imbalance: Bumetanide can cause your electrolyte levels (such as potassium and sodium) to drop. Your healthcare provider will likely monitor your electrolyte levels through blood work, usually a few weeks after starting or increasing the dose, to ensure they remain within safe ranges.

Dehydration: Because Bumetanide increases urination, it can lead to dehydration. Symptoms of dehydration include dry mouth, excessive thirst, dizziness, and reduced urine output.

Hearing Issues: Although rare, some people might experience ringing in the ears (tinnitus) or hearing loss. If you notice any hearing issues, notify your healthcare provider immediately.

Monitoring Blood Work: It is important to monitor your creatinine, potassium, and sodium levels through blood work, which can range from days to weeks after the initiation of Bumetanide or after an increased dose. The timing of these checks will depend on the dose and your personal sensitivity to the medication.

Always notify your healthcare provider if you develop any side effects or have any concerns or questions about your medication. They can provide the best advice and support for your specific situation.

Veronique Crutel holds a Doctor of Pharmacy degree from the Free University of Brussels, Belgium and an advanced studies degree in Radiopharmaceuticals from the French National Institute for Nuclear Science and Technology. She received a PhD in CNS PET-Scan Radiopharmaceuticals from Paris XI University in 1996. She transitioned then to the pharmaceutical industry and after a few years working for a contract research organization, she joined Servier in 2000 as a project leader in the cardiology department. In 2004, she moved to the neuropsychiatric department, working on the development of compounds in depression and schizophrenia.

Since 2016, she has been leading as a project director a multidisciplinary team on the development of Bumetanide in Autism Spectrum Disorder (ASD).

The FDA approved Buspar for anxiety in 1986, and it has been available as a generic busprione since 2001. Pharmacologists classify buspirone as a serotonin 1A partial agonist, as its primary action appears to be the activation of pre-synaptic serotonin receptors, which gradually decreases firing of serotonin neurons.

For years, buspirone has been considered a third or fourth tier option for treating ADHD. In addition to effectively treating anxiety associated with ADHD, it may also provide direct benefit for ADHD. 1-2-pyrimidinylpiperazine (1-PP), a major metabolite of buspirone, is an alpha-2 antagonist, which boosts levels of norepinephrine, dopamine and serotonin. Limited research indicates buspirone does reduce ADHD symptoms in humans.

A handful of studies, almost all in rodents, indicate that buspirone can successfully treat several side effects associated with Ritalin, including teeth grinding (bruxism), liver toxicity, and growth inhibition. Buspirone also appears to reduce the potential for substance abuse and cognitive dulling that are associated with long term Ritalin use in rodents.

In September 2023, the FDA approved gepirone, (Exxua) which is chemically similar to buspirone, for the treatment of depression. Gepirone is also metabolized to 1-PP, so you may well be hearing more about this class of drugs for depression, anxiety, and ADHD.

Ans: Citalopram is used to treat depression. Citalopram is in a class of antidepressants called selective serotonin reuptake inhibitors (SSRIs). It is thought to work by increasing the amount of serotonin, a natural substance in the brain that helps maintain mental balance.

Citalopram is also sometimes used to treat eating disorders, alcoholism, panic disorder (condition that causes sudden attacks of extreme fear with no apparent cause), premenstrual dysphoric disorder (a group of physical and emotional symptoms that occur before the menstrual period each month), and social phobia (excessive anxiety about interacting with others). Talk to your doctor about the possible risks of using this medication for your condition.

This medication may be prescribed for other uses; ask your doctor or pharmacist for more information.

Citalopram comes as a tablet and a solution (liquid) to take by mouth. It is usually taken once a day, in the morning or in the evening, with or without food. Take citalopram exactly as directed. Do not take more or less of it or take it more often than prescribed by your doctor.

Your doctor may start you on a low dose of citalopram and gradually increase your dose, not more often than once a week.

It may take 1 to 4 weeks before you notice the full benefit of citalopram. Continue to take citalopram even if you feel well. If you suddenly stop taking citalopram, you may experience withdrawal symptoms such as mood changes, irritability, agitation, dizziness, numbness, tingling or electric shock-like sensations in the hands or feet, anxiety, confusion, headache, tiredness, nausea, sweating, shaking, and difficulty falling asleep or staying asleep. Do not stop taking citalopram without talking to your doctor. Your doctor will probably decrease your dose gradually.
