Videoconsejo sanitario sobre Clonazepam, para qué sirve este medicamento, cuándo y cómo debemos tomarlo. Videoconsejo impartido por la farmacéutica Elena Vega, elaborado por el Consejo General de Colegios Farmacéuticos y producido por Medicina TV.

Al igual que otros fármacos ya tratados en este canal (como diazepam o lorazepam), clonazepam pertenece al grupo de las benzodiazepinas. Este fármaco actúa sobre los receptores benzodiacepínicos, potenciando la acción de un neurotransmisor inhibidor llamado GABA, por lo que ejerce todos los efectos farmacológicos de las benzodiazepinas: anticonvulsivante, sedante, relajante muscular y ansiolítico.

A diferencia de la mayoría de benzodiazepinas, clonazepam se usa, debido a sus efectos anticonvulsivantes, como agente antiepiléptico. Es capaz de reducir rápidamente muchos tipos de actividad neuronal anormal, por lo que está autorizado para el tratamiento de niños y adultos con diversos tipos de epilepsias: ausencias, crisis tónico-clónicas, crisis parciales o estatus epiléptico. De hecho, se ha mostrado eficaz en algunos casos resistentes a la terapia convencional.

En la farmacia están disponibles distintas presentaciones de clonazepam en forma de comprimidos por vía oral de 0,5, 1 y 2 mg. Pero también existen presentaciones en forma de solución oral en gotas y en forma de ampollas con solución para inyección intravenosa, especialmente indicadas para su uso en lactantes y niños pequeños.

Si se usan gotas en niños, es importante saber que hay que mezclarlas con agua, té o zumos y administrarlas con cuchara, nunca directamente desde el envase a la boca.

La dosis será fijada por el médico de forma individualizada según la edad y el peso, y también según la respuesta clínica y la tolerancia al fármaco, teniendo en cuenta que el empleo concomitante de otros antiepilépticos puede aumentar los efectos secundarios.

El tratamiento se iniciará con la menor dosis diaria posible (en adultos, por ejemplo, menor de 1,5 mg/día en 3 tomas) y posteriormente se irá ajustando. La dosis máxima diaria que nunca debe superarse es de 20 mg diarios en adultos y de 0,2 mg/kg/día en niños de hasta 10 años. Es importante destacar que los comprimidos de clonazepam se pueden partir para dividir la dosis.

Las benzodiazepinas, en general, y el clonazepam en particular, suelen ser bien toleradas, aunque existe cierto riesgo de aparición de fenómenos de dependencia tras un uso continuado o en dosis superiores a las recomendadas. Por tanto, evita tratamientos más largos de lo prescrito, y consulta a tu médico sobre cómo suspender esta medicación cuando ya no la necesites ya que la retirada del fármaco debe ser gradual y no debe hacerse de forma brusca.

Las reacciones adversas, que pueden llegar a ser importantes, suelen desaparecer sin necesidad de interrumpir el tratamiento. Sobre todo destacan las relacionadas con el sistema nervioso, como una menor concentración, desorientación, sedación, cansancio y somnolencia, por lo que habría que tener especial cuidado si se va a conducir o utilizar maquinaria peligrosa, ya que nuestra capacidad de reacción podría estar reducida.

Se recomienda, además, evitar el consumo de bebidas alcohólicas durante el tratamiento, debido a que el alcohol también tiene efectos depresores sobre el sistema nervioso central y sus reacciones adversas podrían verse potenciadas.

Entre otras recomendaciones importantes podemos destacar no utilizar clonazepam sin receta médica, no aumentar la dosis indicada por el médico y prestar atención ante una posible aparición de efecto rebote al terminar el tratamiento (con agitación o ansiedad).

Por último, desecha el tratamiento que te haya sobrado en el punto SIGRE de tu farmacia.

Taking any medication long term can be scary, and psychiatric medications are no exception. I’ve been on Lamictal (aka Lamotrigine) for ten years, and this is my story. I talk in depth about long term side effects and whether it is still effective after all this time… which is typically what I get asked about when it comes to my treatment since it’s been a decade now.

But Lamictal is amazing and it saved my life, and it’s saved a lot of people’s lives. And while long term side effects can cause one to worry, it’s important to remember that it’s sometimes that specific medication that gave you the ability to get a long term side effect in the first place.

So, put it this way, I’m a very grateful person.

Your medication Lamotrigine

Lamotrigine is also known by the brand name: Lamictal, Lamictal ODT, and Lamictal XR.

Lamotrigine comes in 25, 100, 150, and 200mg tablets, 5, and 25mg chewable tablets, 25, 50, 100, and 200mg dissolvable tablets, and 25, 50, 100, 200, 250, and 300mg extended release tablets.

Lamotrigine is most commonly used to treat seizures and bipolar problems. No matter what your doctor has you taking it for, lamotrigine is taken by mouth with or without food once or twice a day. Do not take this drug with dairy products, like milk or yogurt, or calcium rich juices. If upset stomach occurs take lamotrigine with food. If you miss a dose, take the missed dose as soon as you think about it. If it is close to time for your next dose, skip the missed dose and resume your normal dosing schedule. Do not take 2 doses at the same time or extra doses. For best results take each dose at the same time each day. Extended-release tablets must be swallowed whole, do not split or break tablets. Use dry hands to take the dissolvable tablet out of the foil and place on top of your tongue. Allow tablet to melt, then swallow.

Some common side effects with lamotrigine include nausea, vomiting, dizziness, drowsiness, tiredness, weakness, and constipation.
Some less common side effects include a very bad skin reaction, headache, abnormal thinking, swelling of the arms or legs, and weight gain or loss.
Do not take lamotrigine if: you have an allergic reaction like a rash, water retention, hives, extensive itching, or mouth ulcers to lamotrigine or any component of the formulation.
While taking lamotrigine, remember it is important to tell your doctor or pharmacist if you have:
• Signs of an allergic reaction (rash, itching, swelling, tightness of the chest, etc.)
• An allergy to lamotrigine.
• A very bad skin reaction, it can cause bad health problems and sometimes death. Get medical attention if you have signs like red, swollen, blistered, or peeling skin with or without fever, red or irritated eye, or sores in your mouth, throat, nose, or eyes. Most cases have happened within 2 to 8 weeks of starting this drug, but some could show up as much as 6 months after.
• This medication should not be abruptly discontinued because of the possibility of increasing seizure frequency.
All forms of lamotrigine need to be stored at room temperature in a dry place, protected from light.

Typically, people with bipolar disorder require a combination of medications that you take at the same time. Why? Because the medications treat different aspects of the disorder. The main four phases that we target in treating bipolar disorder are Acute depression, Acute mania, mixed states and the maintenance phase.

And acute means it’s the active state that you're in and it's relatively recent. maintenance phase is the in between episodes when things have stabilized.

Not all of the medications work for every phase. So you could be put on a medication because you're manic and then once that mania goes away you can have a depressive episode pop up because that original mania drug isn't doing a good enough job keeping your depression away.

Mechanism of Action of lamotrigine:
The precise mechanism(s) by which lamotrigine exerts its anticonvulsant action are unknown. In
animal models designed to detect anticonvulsant activity, lamotrigine was effective in preventing
seizure spread in the maximum electroshock (MES) and pentylenetetrazol (scMet) tests, and

models to human epilepsy, however, is not known.
One proposed mechanism of action of lamotrigine, the relevance of which remains to be
established in humans, involves an effect on sodium channels. In vitro pharmacological studies
suggest that lamotrigine inhibits voltage-sensitive sodium channels, thereby stabilizing neuronal
membranes and consequently modulating presynaptic transmitter release of excitatory amino
acids (e.g., glutamate and asp)

Generic name: Lamotrigine.
Brand names: Lamictal.
Available as chewable and soluble tablets.

How to use it?
The chewable tablets can be chewed. And the solvable tablets should be solved in a glass of water.
In addition, you can take these tablets at any time, however it is recommended to take them at fixed times. This will make it less likely that you forget a dose.

When to use it?
Lamotrigine blocks sodium channels in your brain, stabilizing the neuronal membranes and thus preventing the release of glutamate.
- Epilepsia
- Bipolar disorder

What is the correct dosage?
The precise dose will depend on your specific indication of use, the side effect you might experience and the severity of your symptoms.Therefore always use the precise amount and frequency your doctor prescribed you.

- When treating epilepsy in Adults the initial oral dose is 25 mg/day. This can be slowly increased up to a maintenance dose of 100-200 mg/day. The maximum dose would be 500mg/day.
- When treating bipolair disorder in Adults the initial oral dose is 25mg 1x/day, which you doctor will slowly increase up to a maintenance dose of 100-400 mg/day.

What are common the side-effects?
- Very common (10%): rash and headache.
- Common (1-10%): irritation, drowsiness, insomnia, dizziness, tremor, nausea, vomiting, diarrhoea, dry mouth, pain and fatigue.
- Uncommon (0.1-1%): double vision, blurred vision and hair loss.
- Rare (0.01-0.1%): Stevens-Johnson syndrome, meningitis and conjunctivitis.

Furosemida es un fármaco diurético que actúa sobre los riñones reduciendo la reabsorción de electrolitos y aumentado la eliminación de agua en la orina. Con ello, es capaz de reducir la presión arterial.

Está autorizado para el tratamiento del edema, que es una acumulación de líquidos en distintas partes del cuerpo y se asocia a diversas enfermedades, como insuficiencia cardiaca, cirrosis hepática y enfermedad renal, o incluso a quemaduras. También se emplea en el tratamiento de hipertensión arterial leve y moderada.

La furosemida está disponible en forma de comprimidos de administración por vía oral, siendo recomendable tomarlos con el estómago vacío, tragarlo sin masticar y con suficiente cantidad de líquido. La dosis utilizada debe ser la más baja para producir el efecto deseado y siempre según las indicaciones del médico prescriptor. La dosis media suele ser de 20 mg cada 24 horas (20-80 mg en adultos), pero se deberá ajustar la dosis de mantenimiento de forma individualizada en función de la respuesta y la tolerabilidad. En caso de olvido de una dosis, tómate el siguiente comprimido a la hora habitual, pero no dupliques la siguiente dosis.

Además, la furosemida está disponible en forma de viales con solución para inyección intravenosa, especialmente dirigidos a pacientes con insuficiencia renal.

Los pacientes alérgicos a sulfonamidas (como las sulfoniureas antidiabéticas o ciertos antibióticos) y las mujeres en periodo de lactancia no deben tomar furosemida. La furosemida puede presentar interacciones farmacológicas con un amplio número de principios activos, por lo que se recomienda consultar al médico al respecto si estás tomando otros medicamentos.

Entre las reacciones adversas más frecuentes destaca la deshidratación, el desequilibrio de electrolitos y descenso excesivo del volumen total de sangre. Por ello, es importante prestar atención en pacientes de edad avanzada o con demencia. Además, puede producir mareos o somnolencia, especialmente al inicio del tratamiento o cuando se aumenta la dosis habitual. No conduzca ni maneje herramientas o máquinas si se siente mareado.

Igual que con otros diuréticos, los tratamientos prolongados pueden provocar alteraciones que se manifiestan con sed intensa, dolor de cabeza, calambres musculares o síntomas gastrointestinales.

Latanoprost or Xalatan treats both elevated pressure inside the eye and glaucoma. This very widely prescribed medicine favorably alters the course of intraocular hypertension and slows or halts progression of vision changes much more dramatically than its predecessor timolol.

Unfortunately in its early stages glaucoma remains totally symptom free. By the time an individual notices vision change, considerable irreversible damage exists in nerve cells at the back of the eye. Glaucoma tends to strike those over age 50. This is one of the reasons periodic eye examinations become increasingly important with aging.

Elevated pressure within the eye results from resistance to the outflow of aqueous humor manufactured by the ciliary body. This results in intraocular pressure in excess of 21 mm which jeopardizes the health of tissues of the optic nerve. Used only once each evening Latanoprost generally suffices to restore normal pressure. When necessary a second or third medicine may be added.

Opened containers of Latanoprost should be stored at room temperature. After 6 weeks any remaining material should be discarded. Latanoprost tends to degrade relatively quickly especially if exposed to excessive heat or sunlight.

Contact lenses should be removed prior to applying Latanoprost and not replaced for at least 15 minutes afterward. Side effects related to Latanoprost are limited to local irritation and redness. Some darkening of the iris may occur along with pigmentation of the eyelid skin and notable eyelash growth.
