Pharmacology Of Fluoxetine (SSRI) - The Antidepressant Medication
Pharmacology Of Fluoxetine (SSRI) - The Antidepressant Medication Side Effects 4 Views • 5 months ago

Video transcript :-
Fluoxetine belongs to the drug class of selective serotonin reuptake inhibitors and is primarily used as an antidepressant among its other uses.
Major indications for Fluoxetine include the following.
Major depressive disorder, commonly known as depression.
Generalized anxiety disorder.
Obsessive-compulsive disorder.
Panic disorder.
Bulimia nervosa.
Premenstrual dysphoric disorder.
And in the treatment of post-traumatic stress disorder, or PTSD.
Fluoxetine works by selectively inhibiting the serotonin transporter, or SERT, which prevents the reuptake of the neurotransmitter serotonin into presynaptic neurons.
This action increases the levels of serotonin in the synaptic cleft, which enhances serotonergic neurotransmission, leading to improved mood and emotional regulation.
Unlike older antidepressants such as tricyclic antidepressants, fluoxetine has minimal effects on norepinephrine and dopamine reuptake.
Fluoxetine is administered orally, and is available as tablets, capsules, and liquid formulations.
Delayed release capsules are also available and used for weekly dosing in some individuals.
The drug is well absorbed into the body with an oral bioavailability of more than 70%.
Once in the bloodstream, it is highly plasma protein bound.
The drug is primarily metabolized in the liver by converting into active metabolite norfluoxetine.
Excretion is primarily through urine.
Some common adverse effects of fluoxetine include the following.
Neurological effects such as Insomnia, agitation, headache, anxiety, confusion, autonomic instability, and restlessness.
Sexual dysfunction, including decreased libido, and delayed ejaculation.
Gastrointestinal effects such as nausea, diarrhea, and dry mouth.
Cardiovascular effects such as QT prolongation.
Reduced appetite, which can lead to weight loss.
Serotonin syndrome, typically in overdose.
Hyperthermia.
And muscle rigidity.
Absolute contraindications for the use of fluoxetine include the following.
Concurrent use with MAO inhibitors such as phenelzine and selegiline as it increases the risk of serotonin syndrome.
And hypersensitivity to fluoxetine.
Relative contraindications include bipolar disorder, as fluoxetine can trigger manic episodes.
Severe hepatic impairment.
And pregnancy.

Cymbalta and Chronic Pain
Cymbalta and Chronic Pain Side Effects 4 Views • 5 months ago

Dr. Pagnani of Rittenhouse Psychiatric Associates discusses Cymbalta (duloxetine) and its use in treating certain chronic pain conditions, regardless of whether a patient has a history of an anxiety or mood disorder. This medication (like all antidepressant medications) has the potential for various side effects (some serious) and if someone is considering a medication trial, it is vital that they make an appointment with a mental health professional.

Dr. Pagnani is the founder and medical director of Rittenhouse Psychiatric Associates, an Instructor at The Johns Hopkins Hospital for the Department of Psychiatry, he supervises psychiatric residents at Jefferson Hospital, he serves on the Young Professional Board of Uplift, the Center for Grieving Children and he maintains a private practice of Psychiatry in Philadelphia where he sees patients for one-time psychiatric evaluations and second opinions. He is a licensed physician and surgeon in NJ, PA, NY, MD, DE, FL and CA and can provide consultations to individuals via HIPAA compliant Telehealth visits living in these locations.

Rittenhouse Psychiatric Associates is a private practice of psychiatry consisting of about 25 psychiatrists, psychiatric nurse practitioners and psychotherapists with physical locations in Philadelphia and Paoli, PA and serving all of PA, NJ, NY and FL virtually via HIPAA compliant tele-psychiatry. Individuals seeking not only evaluations but long-term psychiatric care, can contact Rittenhouse Psychiatric Associates directly.

These videos aim to increase awareness and discussion of mental health topics and to encourage struggling individuals to schedule an appointment directly with a mental health provider. Mental health needs to be normalized, and simply speaking about mental health topics openly, is an important first step.

Dr. Pagnani and Rittenhouse Psychiatric Associates are NOT advocating for or against the use of any mentioned psychiatric medications for any specific individual. Additionally, any side effects mentioned are not a complete list of potential side effects or contraindications to taking any mentioned medications, and all risks and benefits of a medication should be discussed with an individual’s provider at an appointment, before starting, stopping or making changes to a medication regimen.

How to Discontinue Cymbalta Safely
Cymbalta (duloxetine) is a medication that is usually given to treat issues of depression. Regardless of the reason for being prescribed Cymbalta, it’s important that when an individual decides to discontinue any medication, they are also incorporating an alternative treatment to address any underlying issues.

Cymbalta brain zaps, dizziness, nausea, brain zaps, and fatigue are other commonly reported Cymbalta withdrawal effects. Cymbalta withdrawal may be experienced either when attempting to discontinue Cymbalta or, in some cases, in between doses — known as interdose withdrawal. Interdose-withdrawal most commonly occurs where drugs have a relatively short half-life, and withdrawal symptoms can emerge some hours after a dose is taken.

So many people have been negatively affected by quitting Cymbalta that the term Cymbalta Discontinuation Syndrome is used commonly to classify the symptoms associated with withdrawal. If the symptoms become too much to handle at home, many attend healing centers for Cymbalta withdrawal.

Cymbalta is an SNRI (serotonin-norepinephrine reuptake inhibitor). For those who are reliant on Cymbalta long-term, the possibility of a serotonin deficiency exists. In order to effectively treat any long-term effects of Cymbalta, focusing on potential underlying causes is important. Reviewing potential underlying causes with a specialist can also help to ensure the treatment plan is accurate to address individual needs and characteristics.

An investigation to determine any underlying causes that may be contributing to an individual’s symptoms should include questions such as:

Have hormones and nutritional elements been tested for deficiencies?
Has the diet been upgraded sufficiently?
Have alcohol and recreational drugs been ruled out or discontinued?
Have food allergies been tested for that could be causing psychiatric or physical symptoms?
Has situational stress contributed to symptoms? Has counseling such as CBT been utilized?
Is there an accumulation of neurotoxins, such as heavy metals, which are contributing to symptoms such as anxiety, psychosis, or depression?
By reviewing factors such as these and others, with a professional, the individual is able to pinpoint and resolve or eliminate many obstacles to wellness.

Statin Associated Muscle Symptoms and Pain
Statin Associated Muscle Symptoms and Pain Side Effects 4 Views • 5 months ago

In this video, I challenge whether statins cause muscle pain. The class of cholesterol lowering medications called statins are incredibly effective at preventing cardiovascular disease such as heart attacks and strokes. Yet many people are afraid of taking these medications for fear of side effects. One of the most common side effects is muscle pain. But how common are statin associated muscle symptoms? And do statins really cause muscle pain? Let’s look at what some of the newest clinical trials say about this controversial topic.

2-Minute Neuroscience: Methylphenidate
2-Minute Neuroscience: Methylphenidate Side Effects 4 Views • 5 months ago

Methylphenidate, better known as Ritalin or Concerta, is a stimulant medication primarily used to treat ADHD. In this video, I discuss how methylphenidate affects the brain and how this is thought to lead to therapeutic effects in ADHD.

TRANSCRIPT:

Methylphenidate is a stimulant medication primarily used to treat ADHD, although it is sometimes also used to treat daytime sleepiness associated with narcolepsy and has several other off-label uses. Methylphenidate is better known as Ritalin or Concerta, which are two of the numerous brand names the drug is sold under.

Methylphenidate is a stimulant drug. Its primary mechanism of action is thought to be the inhibition of reuptake of the neurotransmitters dopamine and norepinephrine. Reuptake is a process used by neurons to remove excess neurotransmitters from the synaptic cleft. In reuptake, proteins called transport proteins move neurotransmitters out of the synaptic cleft and typically back into the neuron that released them. Thus, by inhibiting reuptake, methylphenidate increases levels of dopamine and norepinephrine in the synaptic cleft. These elevated levels of dopamine and norepinephrine lead to increased activity at the receptors for these neurotransmitters, and cause various alterations in brain activity.

For example, methylphenidate has been shown to increase activity in the prefrontal cortex, an area of the brain that’s considered important to attention and other executive functions. The exact way the pharmacological actions of methylphenidate help improve the symptoms of ADHD is not fully understood, but in general it’s thought that dopamine and norepinephrine signaling are important to attention and other executive functions, and thus increasing levels of these neurotransmitters leads to therapeutic effects in ADHD.

Although inhibiting dopamine and norepinephrine reuptake appears to be the primary mechanism of action for methylphenidate, the drug does have a number of other actions in the nervous system. For example, it also stimulates serotonergic and adrenergic receptors. It’s unclear exactly what contribution these other actions may make to methylphenidate’s therapeutic effects, but studies have found that the effect on adrenergic receptors specifically may be important to the drug’s ability to enhance cognitive function.

Clozapine Withdrawal, Tapering, and Clozapine Alternatives. Alternative to Meds Clozapine Help Video
Clozapine Withdrawal, Tapering, and Clozapine Alternatives. Alternative to Meds Clozapine Help Video Side Effects 4 Views • 5 months ago

Our treatment specialist and founder of ATMC, Lyle Murphy, answers your biggest concerns about the second generation antipsychotic drug Clozapine.

Clozaril is the brand name for the active ingredient, clozapine. Other brand names include Clopine, Clozapine Synthon, and FazaClo.

Q&A From you!

00:00 - Intro
00:20 - Why do so many doctors feel that Clozapine is the most effective medication for schizophrenia?
00:58 - Is clozapine a mood stabilizer?
01:21 - What are the worst side effects of clozapine?
03:13 - Why is clozapine called an atypical antipsychotic and not a benzodiazepine?
04:19 - Can clozapine cause tremors?
05:11 - What's next for the schizophrenic when clozapine fails? Doctors seem to always have another one ready?
06:04 - Could clozapine affect one's cognitive abilities/IQ?
08:31 - Is Clozapine worth the risk for those suffering from severe schizophrenia, considering how dangerous some of the side effects are?
10:07 - What antipsychotic is a good alternative to clozapine if you have to have something?
12:24 - Why would someone be prescribed a "therapeutic" dose of clozapine?
12:53 - Is olanzapine a better antipsychotic than clozapine if you worry about your physical health most?
15:20 - Which is less likely to cause tardive dyskinesia, olanzapine 2.5mg or clozapine 25mg?
15:55 - How long do antipsychotics withdrawal symptoms last from 2 months from 100 mg of Clozapine?
17:40 - What are the chances of a low dose (25mg) of clozapine causing tardive dyskinesia?
18:09 - Does a significant weight loss change the efficacy of clozapine?
19:04 - Can hypomotility caused by clozapine affect the absorption and efficacy of it?
20:32 - Will fasting for 24 hours affect the absorption of clozapine and cause a problem?
24:25 - ATMC

Careful Clozaril withdrawal can minimize the potential for rebound psychosis and cholinergic crises. Clozaril belongs to a class of drugs called SGA’s or second-generation antipsychotics.
Used in the treatment of certain mental conditions such as schizophrenia and psychosis, Clozaril is often prescribed only AFTER other antipsychotics did not work or caused severe side effects.
Alternative to Meds has been the expert on antipsychotic withdrawal and Clozaril alternatives for over 15 years. We have published evidence regarding our success. Underlying issues can in many cases be addressed in much less toxic ways. It is possible that some medical conditions could have been overlooked. Commonly some or all of the original contributing factors have improved or changed, and the diagnosis one was given may no longer lead to recovery.

What Is Clozaril (clozapine) Used For?

Clozaril is used in the treatment of certain mental disorders, where other (usually at least two) other antipsychotic medications have not worked, or where the side effects were too harsh and could not be tolerated by the patient. A candidate for a prescription of Clozaril could be someone diagnosed with conditions such as:

Treatment-resistant schizophrenia.
Treatment-resistant schizoaffective disorder with a known risk for suicide.
Please note that elderly patients with dementia-related psychosis should NOT be prescribed Clozaril (or other antipsychotic medications) due to the increased risk of death.

Discontinuing/Quitting Clozaril (clozapine)
There are circumstances where immediate withdrawal of Clozaril is medically needed as a life-saving intervention. These include potentially life-threatening adverse reactions to the drug which are delineated in detail under the adverse effects section, for instance, cardiovascular compromise, risk of stroke, signs of tardive dyskinesia, and other severe reactions.

In the majority of cases, gradual Clozaril tapering methods are recommended for coming off the drug.

Always seek medical guidance and direction from your doctor before making changes to medications.

Dermovate cream is a topical corticosteroid that is used to reduce inflammation and itching associated with various skin conditions, such as eczema, psoriasis, dermatitis, and other inflammatory skin disorders. It should be used only as directed by a healthcare professional.

The most common side effects of Dermovate cream include skin irritation, burning or stinging sensation, redness, and thinning of the skin. Prolonged use of this cream can also lead to skin discoloration, stretch marks, and increased hair growth in the treated area. If any of these side effects become severe or persistent, it is important to consult a doctor.

The daily dose of Dermovate cream will vary depending on the specific skin condition being treated and the severity of the symptoms. It is important to follow the instructions provided by a healthcare professional and not exceed the recommended dose.

Dermovate cream is contraindicated in individuals who are allergic to corticosteroids or any of the other ingredients in the cream. It should also not be used on broken or infected skin, as it can worsen the condition.

To use Dermovate cream, clean and dry the affected area before applying a thin layer of the cream. Gently rub it into the skin until it is fully absorbed. It is important to wash hands after applying the cream to avoid spreading it to other areas of the skin or mucous membranes.

It is recommended to use Dermovate cream for the shortest duration possible to minimize the risk of side effects. If the symptoms do not improve or worsen after using the cream, it is important to consult a healthcare professional for further evaluation and treatment.

Antihistamines are medications that counteract the action of histamine. Histamine is most notoriously known as a mediator of allergic reactions, but it’s also involved in important physiological processes such as immune response, gastric acid secretion, sleep and wake cycle, cognitive ability and food intake.
Histamine is synthesized from the amino acid histidine. It is present in all tissues but most abundant in the skin, lungs and gastrointestinal tract. Most of histamine in tissues is stored as granules inside mast cells. In the brain, histamine also functions as a neurotransmitter. It is found in histaminergic neurons of the hypothalamus, whose axons project throughout the brain.
Histamine exerts its action by binding to histamine receptors, H-receptors, all of which are G-protein- coupled. There are four H-receptors, with different tissue expression patterns and functions. Both H1 and H4 are involved in allergic inflammation, but only H1-antihistamines are currently available for allergy treatment. The major function of H2-receptor is to stimulate gastric acid secretion, so H2-antihistamines are used to treat gastric acid disorders such as gastric reflux and peptic ulcers.
The term “antihistamine” generally refers to allergy-treating H1-antihistamines.
Most allergies occur upon a repeated exposure to an allergen. Mast cells that were previously sensitized to the allergen are activated, releasing histamine and other inflammatory chemicals. Histamine causes dilation and increased permeability of blood vessels, stimulation of sensory nerves, contraction of smooth muscle; and is responsible for most allergic symptoms, ranging from watery eyes, runny nose, sneezing, itching; to swelling, hives, and difficulty breathing due to bronchospasm. When released systemically, histamine can cause extensive vasodilation and bronchoconstriction which may lead to life-threatening anaphylaxis.
Most H1-antihistamines are not similar to histamine in structure and do not compete with it for binding to H1-receptor. Instead, they bind to a different site on the receptor and stabilize it in its inactive state. The first-generation H1-antihistamines derive from the same chemical class as muscarinic, adrenergic and serotonin antagonists, so they also have anti-cholinergic, anti-adrenergic, and anti-serotonin effects. More importantly, they can cross the blood-brain barrier and interfere with histamine functions in the brain, causing drowsiness, cognitive impairment and increased appetite. Some of these drugs are actually used for their sedative side effect, as sleeping aid medications.
Second-generation antihistamines are less able to cross the blood-brain barrier, and are therefore minimally or non-sedating. They are also highly selective for H1-receptor and have no anti-cholinergic effects.

ANTIHISTAMINES AND THE BRAIN
ANTIHISTAMINES AND THE BRAIN Side Effects 4 Views • 5 months ago

http://www.socalbraincenter.com

Dr. Julie Brown, Carrick trained concussion doctor, board-certified chiropractic neurologist and fellow in traumatic brain injury and rehabilitation. It is almost spring here. In fact, we spring our hours forward this weekend. But it's almost spring, and pollen, histamines start reacting in people. I just want to talk about antihistamines. I can't prescribe these things, and nor do I want to. A histamine ... it's a complex reaction, and one of the things that it does is, it wakes us up. It actually makes it hard to fall asleep. If pollen's everywhere and you're reacting to it, it may be harder for you to sleep at night just because we've now raised cortisol and we're awake. The goal would really be to break down those histamines to a more functional level to where they're not making your brain awake. My reason for this video is regarding antihistamines. Many of them, and I would just do a Google search and/or talk to your doctor about it, or your pharmacist ... many of them are anticholinergic.
That means that their mechanism is that they go up and they dock at the end of a neuron that uses choline as a receptor, and many of our learning and memory neurons require choline. If you're taking antihistamines that are anticholinergic, you're actually blocking your ability to remember things. Imagine if you're a student trying to study for a test or you have to memorize something for your job and you're taking these things. It is making your job that much harder, plus it is not actually breaking down histamines and correcting the problem. There are other approaches to it, and you can have different genes, different exposures that create these cycles for histamine, and how they need to be broken down can be different for everybody. Make sure you talk to your doctor about your concerns and, honestly, hop on Google for that. Not for all of it, but just to get an idea to be able to have the conversation with your doctor. 💊 Citalopram e Escitalopram,
