Xarelto is a prescription medication used to prevent blood clots from forming as the result of an irregular heartbeat/atrial fibrillation. Also called Rivaroxaban, the drug is commonly given after hip or knee replacement surgery. Patients with other types of blood clots including deep vein thrombosis (DVT) or pulmonary embolus (PE) may be given Xarelto along with those who need to prevent past blood clots from forming again. The drug is considered an anticoagulant and works by blocking specific clotting proteins in the patient’s blood.

Your Medication Rivaroxaban

Rivaroxaban is also known by the brand name: Xarelto.

Rivaroxaban comes in 10, 15 and 20 mg tablets.

Rivaroxaban is most commonly used to prevent blood clots. No matter what your doctor has you taking it for, rivaroxaban is usually taken by mouth most commonly once a day with food and a full glass of water. If you miss a dose, take the missed dose as soon as you think about it. If it is close to time for your next dose, skip the missed dose and resume your normal dosing schedule. Do not take 2 doses at the same time or extra doses.

Some common side effects with rivaroxaban include bleeding, or hemorrhage.
Some less common side effects include back pain, nausea, itching, and tiredness.

Do not take rivaroxaban if you are taking another medication to prevent blood clots.

While taking rivaroxaban, remember it is important to tell your doctor or pharmacist if you have:
• Signs of an allergic reaction (rash, itching, swelling, tightness of the chest, etc.)
• An upcoming surgery
• Any signs of nerve problems like back pain, numbness or tingling, muscle weakness, paralysis, or loss of bladder or bowel control.
• Any signs of bleeding problems, like bruising; black, tarry, or bloody stools; bleeding gums; blood in the urine; coughing up blood; feeling dizzy; nosebleeds; throwing up blood

All forms of rivaroxaban need to be stored at room temperature in a dry place, protected from light. Keep rivaroxaban in a safe place out of reach of children and pets. Before throwing out any unused rivaroxaban, check with your pharmacist about proper disposal.

Ivy Altomare, MD; Keith R. McCrae, MD; and Terry Gernsheimer, MD, review the potential for treating adult idiopathic thrombocytopenia purpura with rituximab.

Torge Rempe, MD, PhD, University of Florida College of Medicine, Gainesville, FL, discusses a single-center study that aimed to explore treatment responses in patients with neuromyelitis optica spectrum disorder (NMOSD), specifically those testing positive for AQP4 antibodies. A distinction was made between Caucasian (22 patients) and non-Caucasian (26 patients, predominantly African-American) patients. Results unveiled that non-Caucasian patients exhibited a younger age of onset (median age 29 vs. 48 in Caucasians) and had a higher treatment failure rate (42.9% vs. 13% in Caucasians) on rituximab therapy. Prof. Rempe emphasizes the unrepresentative nature of current clinical trial data, which predominantly features Caucasian participants. The study highlights the necessity for broader and diverse clinical trials to better understand treatment efficacy across different populations. This interview took place at the European Committee for Treatment and Research in Multiple Sclerosis (ECTRIMS) Congress 2023 in Milan, Italy.

Ritalin or methylphenidate was initially approved about 65 years ago yet remains among the top 50 prescribed medicines in American. Ritalin ranks just behind asthma medicines as the second most common long term prescription agent for children. The main indication for Ritalin is ADHD – attention deficit hyperactivity disorder.

Several different forms of Ritalin exist – an immediate release form, a sustained release version and a long acting capsule. The likelihood of abuse and dependence generally appears at a dose of Ritalin higher than necessary to control symptoms of AHDH. Difficulty sleeping, mood disorders, restlessness, and interference with both linear growth and weight gain are well established side effects.

Approximately half of children and adults diagnosed with ADHD will begin drug therapy. Diagnosis of ADHD remains about 20 times more common in America compared to many European countries. In some European countries Ritalin is not legally available.

Some argue that the explosive surge of ADHD in America may be related to overdiagnosis and lack of recognition of underlying social or family issues. Drug company influence in the medical community, advertising and pharmaceutical involvement in medical research might inflate the rate of diagnosis. At present 1 in every 5 boys carry the diagnosis of ADHD with the vast majority also being diagnosed with other psychiatric diseases as well.

Risperdal® (Risperidone) is an atypical antipsychotic drug which has been linked to serious and potentially permanent side effects such as gynecomastia, the development of female breasts in boys and young men. Increases in the hormone prolactin and lactation (breasts with milk) are potentially concurrent conditions.

Risperdal was originally approved by the FDA for adults suffering from schizophrenia or bipolar mania, but has been promoted and marketed so doctors increasingly prescribed it for children to treat ADHD, depression and behavioral problems-- health matters for which Risperdal® did not originally have FDA approval. These "off label" uses as prescribed by physicians are not illegal, however there are allegations the drug's manufacturer, Janssen, a division of Johnson & Johnson, withheld safety data and downplayed the gynecomastia risk.

Risperidone comes in 0.25, 0.5, 1, 2, 3, 4 mg tablets. Risperidone also comes in 0.25, 0.5, 1, 2, 3, 4 mg dispersible tablets.

Risperidone is most commonly used for: bipolar disorder, schizophrenia, or for irritability associated with autism. Risperidone may also be used for depression, post-traumatic stress disorder, Tourette’s syndrome, or for psychosis and agitation related to Alzheimer’s disease. Risperidone is usually taken by mouth once daily.

Standard dosing of risperidone ranges from: 0.5 mg up to 8 mg per day depending on treatment.

Some common side effects of taking risperidone include: sedation, drowsiness, headache, and weight gain.
Some less common side effects of taking risperidone include: slow heartbeat, low blood pressure, dry mouth, and nasal congestion.

Risperidone has shown increased death risk in elderly patients with dementia-related psychosis.

Risperidone is a second-generation anti-psychotic medication, primarily used in the treatment of various psychiatric disorders.
It works by modulating dopamine and serotonin receptors in the brain, aiding in restoring the balance of these neurotransmitters.
Major indications of risperidone include the following.
In the treatment of schizophrenia spectrum disorder in adults and children of 13 years or older.
And to reduce irritability associated with autism spectrum disorder in children and adolescents aged 5 to 16 years.
Off-label uses include post traumatic stress disorder, obsessive compulsive disorder, Tourette’s syndrome, and behavioral changes in dementia.
In this video, our primary goal is to explore the common Adverse Effects of Risperidone.
Risperidone is well-known to cause central nervous system side effects, which typically manifest as extrapyramidal symptoms, including the following.
Parkinsonism, which includes tremors, rigidity, and bradykinesia, or slowness of movement.
Restlessness.
Sustained muscle contractions.
And involuntary movements, especially with long-term use.
Metabolic side effects include weight gain, hypoglycemia and risk of new-onset diabetes, and dyslipidemia.
Endocrine effects include low prolactin levels, galactorrhea, gynecomastia, sexual dysfunction, and menstrual irregularities.
Sedation and Somnolence, especially in the beginning.
Cardiovascular Effects, including orthostatic hypotension, QT prolongation, and increased risk of stroke in elderly patients.

And finally, neuroleptic Malignant Syndrome, which is a rare, but potentially life-threatening reaction, characterized by hyperthermia, muscle rigidity, altered mental status, and autonomic instability.

A little more than one month ago I began taking Rinvoq for my ankylosing spondylitis. The effectiveness of my second biologic has shown signs of being ineffective; my chronic pain had become worse and my movements had become more limited. My doctor recommended that I consider Rinvoq, a medication that is still not approved for use here in British Columbia. This video summarizes my findings.

Please note that in spite of the years with ankylosing spondylitis, I had maintained a very healthy lifestyle: a diet that was primarily plant-based, stress reduction techniques, good sleep, and a disciplined regimen of exercise.
