Video abstract of original research paper "Safety and effectiveness of rapid-acting intramuscular olanzapine for agitation associated with schizophrenia – Japan postmarketing surveillance study" published in the open access journal Neuropsychiatric Disease and Treatment by Katagiri H, Taketsuna M, Kondo S, et al.

Objective: The objective of this study was to evaluate the safety and effectiveness of rapid-acting intramuscular (IM) olanzapine in the treatment of acute agitation associated with schizophrenia in real-world clinical settings in Japan.

Methods: In this multicenter, postmarketing surveillance (PMS) study, patients with acute agitation associated with schizophrenia were treated with IM olanzapine daily in a daily clinical setting. The observational period ranged from 1 to 7 days, including the day of initial administration. Safety was assessed by reporting treatment-emergent adverse events (TEAEs) and adverse drug reactions (ADRs). The Positive and Negative Syndrome Scale – Excited Component (PANSS-EC) score was used to evaluate effectiveness at baseline and at 2 hours (after each administration), 2 days, and 3 days (end of the observational period) from the last administration of the IM olanzapine injection.

Results: The safety analysis set included 999 patients, and the initial dose of 10 mg was administered to 955 patients. TEAEs were reported in 28 patients (36 events), the most common of which were dyslalia (5 patients), akathisia and somnolence (4 patients each), hepatic function abnormal (3 patients), and constipation and dehydration (2 patients each). One serious adverse event of akathisia occurred during the observation period. The PANSS-EC score (mean ± standard deviation) was 23.3±6.4 (n=625) at baseline, 16.9±7.0 (n=522) at 2 hours after initial injection, and 14.9±6.5 (n=650) at the last observation carried forward.

Conclusion: The results of this Japanese PMS study demonstrated that IM olanzapine is safe and has a favorable effectiveness profile in the treatment of schizophrenia patients with acute agitation.

Antipsychotics, previously known as neuroleptics and major tranquilizers, are a class of psychotropic medication primarily used to manage psychosis, principally in schizophrenia but also in a range of other psychotic disorders

Common Side Effects: Drowsiness, weight gain, dry mouth, stomach upset, QTc interval prolongation, restlessness, dystonia (uncontrollable muscle contractions), tardive dyskinesia (repetitive, involuntary movements), parkinsonism (slowness, stiffness, and tremor), neuroleptic malignant syndrome (rare, but fatal), and seizures.

Huntington disease, Parkinson disease, Lesch-Nyhan syndrome, pervasive developmental disorder are some other conditions where antipsychotics are an option though it is not the primary drug of choice.

First-Generation Antipsychotics:

All dopamine receptor antagonists are available and can be administered in oral form. Except for thioridazine, pimozide, and molindone, all other first-generation antipsychotics can also be given parenterally. Haloperidol and fluphenazine can be delivered in long-acting depot parenteral form.

Second-Generation Antipsychotics:

These can be administered in oral or parenteral forms. Risperidone, olanzapine, aripiprazole, and paliperidone are available as extended-release or long-acting injectable forms. Clozapine, asenapine, and olanzapine are available in the sublingual formulation.

Allergies reign supreme. Successfully combating them entails an enormous array of pills, potions and sprays none of which provides a knockout in all cases. Non-sedating anti-histamines such as Zyrtec may offer some relief for the runny nose, watery eyes and sneezing that so often form the core of seasonal or perennial hay fever or allergic rhinitis.

Anti-histamines may also prove beneficial in treating hives or urticaria. This allergic skin reaction often involves liberation of histamine and an incessantly itchy eruption. Dramatic improvement but often not complete success follows shortly after beginning a course of anti-histamines.

Usually these drugs fail to provide relief for the nasal symptoms accompanying the common cold since histamine is not a major culprit. Similarly anti-histamines regularly lack effect for simple itchy skin.

The older first generation anti-histamines routinely cross into the brain and may contribute to sleepiness which makes them less desirable for daytime use. To a large degree second generation anti-histamines including Zyrtec, Allegra and Claritin avoid this unwanted effect and additionally may be taken only once daily. However of all the newer anti-histamines Zyrtec occupies more of the brain’s anti-histamine receptors and may produce a degree of tiredness.

When Zyrtec gained FDA marketing approval it contained two mirror image molecules known as levo-cetirizine and dextro-cetirizine. But the body typically only recognizes 1 of these so-called enantiomers. With patent expiration and in an attempt to extend their income stream the pharmaceutical company removed the inactive dextro-cetirizine and rebranded the old Zyrtec as the newer patent protected Xyzal.

Tradition focuses our attention on anti-histamine therapy for allergy symptoms. Most experts suggest an even better option for many if not most individuals would be intranasal cortisone sprays such as Flonase and Nasacort. These provide more universal relief while focusing their complete attention on the local symptoms without providing significant systemic effects.

While the name brand products sell at a major premium, equally effective generics offer the same relief without the unnecessary financial toll.

Tryptophan vs. melatonin for sleep, which is better? I believe tryptophan is better for sleep than melatonin. Let’s talk about why. 

When you take melatonin to help you sleep, you’re actually taking a hormone. Over a period of time, you could end up needing more and more of this hormone to create the same effect. 

Instead, you could try taking the precursor to melatonin called tryptophan. Tryptophan is an amino acid that turns into 5-HTP, which turns into serotonin, which then turns into melatonin. Melatonin is a hormone that induces sleep, and it’s stimulated by darkness. 

Benefits of tryptophan:
• It has virtually no side effects 
• It can act as a natural antidepressant 
• It has anxiolytic properties (anti-anxiety)
• It can promote sleep 

However, if you’re on any psychiatric medications, talk to your doctor before taking tryptophan. You don’t want to end up in a situation where you have too much serotonin. You also don’t want to come off of medication too quickly or add something else to the mix that could create an imbalance. 

When you take tryptophan, you always want to take it on an empty stomach. Tryptophan competes with other amino acids. Anytime you take tryptophan with another protein, it won’t work. 

Turkey doesn’t make people tired because of tryptophan. People get tired on Thanksgiving because they consume a lot of carbs with their turkey. Any time you combine carbs with protein, you’re going to spike insulin, and that will make you tired.

Dr. Jawad talks about the benefits of taking 5 HTP. 5-HTP has most often been used by adults in doses of 150-300mg.

The most significant side effects and adverse reactions may occur with long-term use (many months or longer). Administration of 5-HTP alone depletes catecholamines (dopamine, norepinephrine, and epinephrine). When dopamine depletion is great enough, 5-HTP will no longer function.

Tryptophan → 5-HTP → serotonin

80-90% of your body’s serotonin is made in your intestines. But serotonin cannot cross the blood-brain barrier. So all the serotonin that your brain needs has to be made within the brain. This is where 5-HTP comes in…

5-HTP crosses the blood-brain barrier more readily than tryptophan. And gets synthesized into serotonin at a faster rate than from tryptophan.

As a neurotransmitter, serotonin influences directly and indirectly, the majority of brain cells. So if you want to boost serotonin in your brain, some find that supplementing with 5-HTP is better than taking tryptophan.

Tryptophan, which your body uses to make 5-HTP, can be found in turkey, chicken, milk, potatoes, pumpkin, sunflower seeds, turnip and collard greens, and seaweed.

When you take 5-HTP alone, you are also depleting dopamine, norepinephrine, and epinephrine. Synthesis of serotonin from 5-HTP, and dopamine from L-DOPA is catalyzed by the same enzyme, L-aromatic amino acid decarboxylase (AAAD).

Dopamine and serotonin precursor supplementation must be taken in proper balance. Because when you use only 5-HTP, it dominates dopamine at the AAAD enzyme synthesis level.

Blocking dopamine synthesis at the AAAD enzyme through competitive inhibition will lead to depletion of dopamine and the rest of the catecholamines.

Studies have found that when dopamine is depleted enough, 5-HTP will no longer function.

One huge word of WARNING: Do NOT take 5-HTP with any antidepressant medication. You put yourself in the very real danger of Serotonin Syndrome which can ultimately kill you.

Successfully supplementing with 5-HTP requires stacking it with a dopamine precursor like L-Tyrosine or Mucana along with B-Vitamins (for synthesis), and one of the sulfur-containing amino acids (methionine, cysteine, homocysteine, or taurine).

5-HTP helps:

Anxiety & Depression. 5-HTP is synthesized into serotonin in brain cells. Elevating levels of serotonin can help relieve the symptoms of anxiety and depression.

Neurotransmitters. 5-HTP is a necessary ingredient for serotonin synthesis which boosts mood and social behavior, improves memory, regulates appetite and digestion, sleep, sexual desire and function.

Brain Optimization. A critical balance of the primary neurotransmitters serotonin and dopamine are needed for the highly optimized brain. When these neurotransmitters are not in balance it affects mood, memory, cognition, decision-making, sexual desire, and a host of other critical life functions.

As a naturopathic doctor, I often get asked about tryptophan and 5-HTP. Which one is better and what are the differences? Here’s what you need to know:

👉 Tryptophan is an amino acid found in protein-rich foods like turkey, chicken, and eggs, and is converted into 5-HTP, which is then converted into serotonin. Serotonin is a neurotransmitter that helps regulate mood, sleep, and appetite. Tryptophan can also be converted down a different pathway called the kynurenin pathway, especially in those with higher levels of inflammation. Keep in mind that tryptophan also competes against other amino acids for transport across the blood brain barrier.

👉 5-HTP is a supplement that is a direct precursor to serotonin and can help with depression, anxiety, and insomnia. 5-HTP can readily cross the blood brain barrier without competition from other amino acids. 5-HTP turns into serotonin at a higher rate than tryptophan, so there is a risk of depleting dopamine. This is because the same enzyme is used to convert L-dopa into dopamine that is used to convert 5-HTP into serotonin.

Tryptophan and 5-HTP both have their place, but it depends on the unique needs of the individual and their medical history to determine which one, if any is best.
