On January 22, 2021, the Food and Drug Administration approved the combination of nivolumab (Opdivo, Bristol-Myers Squibb Co.) and cabozantinib (Cabometyx, Exelixis) as first-line treatment for patients with advanced renal cell carcinoma (RCC).

Efficacy was evaluated in CHECKMATE-9ER (NCT03141177), a randomized, open-label trial in patients with previously untreated advanced RCC. Patients were randomized to receive either nivolumab 240 mg over 30 minutes every 2 weeks in combination with cabozantinib 40 mg orally once daily (n=323) or sunitinib 50 mg orally daily for the first 4 weeks of a 6-week cycle (4 weeks on treatment followed by 2 weeks off) (n=328).

The trial demonstrated a statistically significant improvement in progression-free survival (PFS), overall survival (OS) and confirmed overall response rate (ORR) for patients treated with nivolumab plus cabozantinib compared with those who received sunitinib. Median PFS per blinded independent central review (BICR) was 16.6 months versus 8.3 months; HR 0.51 (95% CI: 0.41, 0.64). Median OS was not reached in either arm; HR 0.60 (95% CI: 0.40, 0.89). Confirmed ORR per BICR was 55.7% and 27.1% in the nivolumab plus cabozantinib and sunitinib arms, respectively.

The most common adverse reactions (= 20%) in patients receiving the combination of nivolumab and cabozantinib were diarrhea, fatigue, hepatotoxicity, palmar-plantar erythrodysaesthesia syndrome, stomatitis, rash, hypertension, hypothyroidism, musculoskeletal pain, decreased appetite, nausea, dysgeusia, abdominal pain, cough, and upper respiratory tract infection.

The recommended dose is nivolumab 240 mg every 2 weeks (30-minute intravenous infusion) or 480 mg every 4 weeks (30-minute intravenous infusion) in combination with cabozantinib 40 mg orally once daily without food until disease progression or unacceptable toxicity.

The nivolumab review used the Real-Time Oncology Review (RTOR) pilot program, which streamlined data submission prior to the filing of the entire clinical application. Both the nivolumab and the cabozantinib applications used the Assessment Aid, a voluntary submission from the applicant to facilitate the FDA’s assessment. The FDA approved this application approximately one month ahead of the FDA goal date.

The nivolumab application was granted fast track review, and both the nivolumab and cabozantinib applications were granted priority review. A description of FDA expedited programs is in the Guidance for Industry: Expedited Programs for Serious Conditions-Drugs and Biologics.

Indications and Select Safety Information
CALQUENCE is indicated in combination with bendamustine and rituximab (BR) for the treatment of adult patients with previously untreated mantle cell lymphoma (MCL) who are ineligible for autologous hematopoietic stem cell transplantation (HSCT), as monotherapy for the treatment of adult patients with MCL who have received at least one prior therapy, and for the treatment of adult patients with chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL).
Serious adverse events, including fatal events, have occurred with CALQUENCE, including serious and opportunistic infections, hemorrhage, cytopenias, second primary malignancies, cardiac arrhythmias, and hepatotoxicity, including drug-induced liver injury. The most common adverse reactions (≥30%) are diarrhea, upper respiratory tract infection, headache, musculoskeletal pain, lower respiratory tract infection, and fatigue. The most common Grade 3 or 4 laboratory abnormalities (≥10%) are absolute neutrophil count decreased, absolute lymphocyte count decreased, platelets decreased, and hemoglobin decreased.

Caplyta occupies the dubious honor of being the 13th second generation antipsychotic granted FDA approval. Other members of the family include Zyprexa, Abilify, Risperdal and Rexulti. Unlike some of the others, Caplyta is only marketed to treat symptoms of schizophrenia. Caplyta is an oral drug administered once daily with meals. The dose is 42 mg.

Schizophrenia includes positive and negative symptoms. The positive symptoms of delusions and hallucinations may improve with Caplyta but the drug appears less likely to help the negative symptoms of apathy, social withdrawal and blunted affect.

Short term trials involving relatively few patients demonstrate relatively unremarkable improvement. Caplyta does not appear to offer any distinct or unique advantages compared to the other 2nd generation antipsychotics. For the cash price of nearly $1600 a month one would expect special benefits. The cash price of other popular generic antipsychotics may be as low as $10 a month.

Caplyta may interact with a variety of other medicines. A special black box warning cautions against use in elderly patients with dementia. Sedation and somnolence occur in up to 1 in 4 patients. Difficulty swallowing due to esophageal problems may lead to aspiration. Dystonia presents as spasm of the neck muscles, tightness of the throat and difficulty breathing.

Unusual movements of the mouth, lips, tongue and eyelids known as tardive dyskinesia may or may not resolve with discontinuation of therapy. Metabolic changes include increase in blood sugar, weight gain and elevation of serum lipids. The white blood count may decrease and potentially serious declines in blood pressure on standing also known as orthostatic hypotension expose the patient to potential falls and fractures. Seizures may occur.

PCRI’s Executive Director, Mark Scholz, MD, explains why there has been a surgery vs. radiation controversy, describes the pertinent risks and considerations when deciding between the two treatments, and explains why he thinks radiation is usually going to be the preferable option, all things considered.

0:43 The main reason why there is a controversy about whether surgery or radiation is better for treating prostate cancer is that as recently as 10-15 years ago the technology for radiation was significantly less effective and more toxic than it is today. There were also fewer options for salvage therapy after radiation in the event of a relapse. Since then, radiation technology has advanced significantly, but the dialogue has been comparatively mired. Today, radiation can be precisely delivered, and radiation is achieving even better cure rates than surgery.

1:59 Surgeons are constrained by the fact that the bladder and the rectum are within millimeters of the prostate. They cannot cut a margin around the gland. Radiation oncologists are not limited in this way.

2:21 External beam radiation has a cure rate about as good as surgery, and radioactive seed implants have even better cure rates than surgery.

2:33 Most men will live for a very long time after being treated for prostate cancer, and so it is important to consider the side effects of each treatment, especially now that they all have comparable cure rates.
