Warfarin aka Coumadin is the most used oral anticoagulant in the world and has been used for more than 50 years for therapy in prevention of thrombus formation and subsequent thromboembolic events. Anticoagulants are blood thinners which prevent formation of blood clots as well as prevent existent blood clots from increasing in size. Blood clots can hinder blood circulation and lead to serious medical conditions, like stroke, heart attack and pulmonary embolism. Thus warfarin is helpful in treating and preventing vein and artery blockage, stroke and heart attack.
Mechanism of Action
Warfarin slows down the formation of blood clots within 24 hours, but the complete effect takes place between 72 to 96 hours after taking the medication. Blood clot formation is a
complicated process which requires certain compounds known as coagulating factors to be present. These coagulating factors are produced by the liver. In the presence of sufficient vitamin K. Warfarin acts by inhibiting the presence of vitamin K and this inhibits the formation of coagulating factors. Substances that require vitamin K for their formation are: the anticoagulant proteins C and S and factors II, VII, IX, and X. Therefore, medications like warfarin block coagulating factors and disturb the process of clot formation. Thus slowing down blood clot formation. In the case of blood clots that are already formed, warfarin stops the clot from getting bigger. It also prevents breakage of the clot which can be otherwise dangerous if it travels in the bloodstream and blocks blood vessels. Warfarin cannot destroy or dissolve a blood clot that is already there. But the blood clot itself may slowly dissolve with time. Nor can Warfarin reverse ischemic tissue damage either, but, may help avoid secondary thromboembolic complications that can be dangerous and even fatal.
Indication
Warfarin (Coumadin) is indicated for/to:
• Prophylaxis, pulmonary embolism (PE), venous thrombosis and its extension, and to prevent blood clots from moving to other parts of the body.
• Reduce the risk of another stroke or heart attack in clients who already have had stroke or heart attack, and to reduce the risk of death.
• Prevent and treat blood clots associated with atrial fibrillation (irregular rapid heartbeat), heart-valve replacement and blood clots in lungs and legs.

Drug Cards Daily | Episode #21: warfarin (Coumadin) | Anticoagulant for Treatment of Myocardial Infarction and Thromboembolic Complications
Warfarin is an anticoagulant that is also known by the brand name Coumadin. It is used in the treatment of myocardial infarction and thromboembolic complications. There is a wide variety of tablet strengths from 1 mg to as high as 10 mg. When dosing warfarin it is very patient-specific. The therapeutic goal is based on various factors and treatment nomograms may vary from institution to institution. Prior to initiation it is wise to establish the patient’s genetic variant for CYP2C9 since this will play a large role in predicting their drug sensitivity. When attempting to reach a therapeutic target the healthcare professional uses INR readings to determine the level of anticoagulation. Generally a range between 2-3 is common with 2.5-3.5 when a high level of intensity is required. Other factors that may help determine patient response are age, race, and diet. The mechanism of action for warfarin lies in the antagonism of Vitamin K which leads toward the inhibition of coagulation factors 2, 7, 9, and 10. The rate-limiting factor can be seen as factor 2 due to it’s long half-life of 60-72 hours. Although INR elevation can be seen in 24-48 hours, therapeutic benefits are not achieved until around 5-7 days after initiation. There is a black box warning for bleeding risk so a patient's INR should be monitored regularly.

Dilantin is one of our oldest anti seizure medications that has been around. It’s indicated for patients who have seizures that start in one area of the brain and spread, so partial complex seizures. The problem with Dilantin is that, it works great for a rescue medication and it’s used a lot in cases of status epilepticus. It’s used post neurosurgery or epilepsy surgery but it has some bad long term side effects so it’s not used as much as it once was. It can cause gum hyperplasia and sort of thickening and coarsening of the facial features. There are people that are still on it who have been on it for years, because it’s the only thing that works but it’s not usually a first choice. It has a big issue with bone health, so patients who are on Dilantin, it could affect their bones, cause wasting of the bones, can cause thinning of the hair as well. Again, it’s an old school medication, not one of our first choices, but as Patty said, it’s very good in the operating room, emergency room, but you can’t give it too quickly in the IV, you have to give it slowly because it can cause cardiac problems. So, it’s one of the medications that they’re sort of phasing out of using in acute care, but clearly many patients are still on it, so it’s important to talk to your doctor, see if you should stay on that medication, are there other options, and to monitor the side effects closely.

Phenytoin belongs to a class of medications called anticonvulsants or anti-epileptic drugs.
Popular brands of phenytoin are Dilantin, Epsolin, Epanutin, and Eptoin.
It works by reducing the abnormal electrical activity or decreasing the impulses in the brain which is responsible for causing seizures.
Phenytoin is prescribed for treating or preventing episodes of generalised tonic-clonic seizures or grand mal epilepsy, complex partial seizures, and seizures occurring during or after brain surgery.
Avoid alcohol consumption during phenytoin treatment.
Maintain good oral hygiene to minimize the risk of gum problems.
Take additional contraceptive measures to avoid pregnancy.

Drug Cards Daily | Episode #33: valsartan (Diovan) | Treating NSTEMI, STEMI, Heart Failure and Hypertension
Valsartan, also known as Diovan, is an angiotensin II receptor blocker (ARB). It is used for treating NYHA class II to IV heart failure along with both treating and managing hypertension. Another common indication is for both STEMI and NSTEMI patients. Typical initiation dosing for heart failure, STEMI and NSTEMI is 20 mg twice daily with titrations up to 160 mg twice daily. When initiating treatment for hypertension the range begins at 80-160 mg po every day up to a max dose of 320 mg per day. Valsartan works by blocking AT2 from the AT1 receptor and is considered to be more efficient than ACEs along with having less side effects such a cough. Some common side effects are orthostatic hypotension, dizziness, and lightheadedness. Common monitoring parameters for valsartan are blood pressure, blood pressure urea, pregnancy, and electrolytes. Patients should avoid salt substitutes containing potassium. There is a black box warning for fetal toxicity because drugs that affect the renin-angiotensin system can cause injury/death to the fetus.

INDICATION AND SAFETY SUMMARY
EBGLYSS™ (EHB-glihs) is an injectable medicine used to treat adults and children 12 years of age and older who weigh at least 88 pounds (40 kg) with moderate-to-severe eczema (atopic dermatitis) that is not well controlled with prescription therapies used on the skin (topical), or who cannot use topical therapies. EBGLYSS can be used with or without topical corticosteroids.
It is not known if EBGLYSS is safe and effective in children less than 12 years of age or in children 12 years to less than 18 years of age who weigh less than 88 pounds (40 kg).
WARNINGS – DO NOT USE EBGLYSS if you are allergic to lebrikizumab-lbkz or to any of the ingredients in EBGLYSS. See the Patient Information leaflet that comes with EBGLYSS for a complete list of ingredients.
BEFORE USING
BEFORE USING EBGLYSS, TELL YOUR HEALTHCARE PROVIDER ABOUT ALL YOUR MEDICAL CONDITIONS, INCLUDING IF YOU:
• Have a parasitic (helminth) infection.
• Are scheduled to receive any vaccinations. You should not receive a “live vaccine” if you are treated with EBGLYSS.
• Are pregnant or plan to become pregnant. It is not known if EBGLYSS will harm your unborn baby. If you become pregnant during treatment with EBGLYSS, you or your healthcare provider can call Eli Lilly and Company at 1-800-LillyRx (1-800-545-5979) to report the pregnancy.
• Are breastfeeding or plan to breastfeed. It is not known if EBGLYSS passes into your breast milk.
TELL YOUR HEALTHCARE PROVIDER ABOUT ALL THE MEDICINES YOU TAKE, including prescription and over-the-counter medicines, vitamins, and herbal supplements.
POSSIBLE SIDE EFFECTS
EBGLYSS CAN CAUSE SERIOUS SIDE EFFECTS, INCLUDING:
• ALLERGIC REACTIONS. EBGLYSS CAN CAUSE ALLERGIC REACTIONS THAT MAY SOMETIMES BE SEVERE. Stop using EBGLYSS and tell your healthcare provider or get emergency help right away if you get any of the following signs or symptoms:
o breathing problems or wheezing
o swelling of the face, lips, mouth, tongue or throat
o hives
o itching

o skin rash
o cramps in your stomach area (abdomen)

• eye and eyelid inflammation, including redness, swelling, and itching

HOW TO TAKE
• SEE THE DETAILED “INSTRUCTIONS FOR USE” THAT COMES WITH EBGLYSS FOR INFORMATION ABOUT HOW TO PREPARE AND INJECT EBGLYSS AND HOW TO PROPERLY STORE AND THROW AWAY (DISPOSE OF) USED EBGLYSS PREFILLED PENS AND PREFILLED SYRINGES.
• Use EBGLYSS exactly as prescribed by your healthcare provider.
• EBGLYSS is given as an injection under the skin (subcutaneous injection).
• If your healthcare provider decides that you or a caregiver can give the injections of EBGLYSS, you or a caregiver should receive training on the right way to prepare and inject EBGLYSS. Do not try to inject EBGLYSS until you have been shown the right way by your healthcare provider. In children 12 years of age and older, EBGLYSS should be given by a caregiver.
