Mirtazapine Update + Now What?
Mirtazapine Update + Now What? Side Effects 2 Views • 5 months ago

My experience with the tetracyclic antidepressant mirtazapine, a discussion of its side effects, and what my psychiatrist wants me to try next that might finally help me get answers to my medication sensitivity problem.

Previous video:

Additional notes: Thanks to mirtazapine, I missed out on going to a gig Sunday night! I wasn't well enough in time and it wouldn't have been safe to go out not knowing if I'd doze off in a bad place. And sure, it's not a big deal, there'll be other gigs, but it was one of my goals to get me out of the house more, and I wasn't able to even try to achieve it! I should have waited until today to try this medication, and in future should be smarter about the timing of these things. And that's what's really frustrating about bad side effects, they essentially put my life on pause, including all the practical things I've been trying to do to get my life back on track. Even the extra sleep had no value, because I still woke up with no energy, and actually feeling a lot worse. I'd really rather get no sleep than get _that_ sorta sleep. Also one point where I _miiiight_ just care about weight gain... I have so many nice goth clothes in my current size!! Hahaha I mean I haven't been wearing them anyway, but it _would_ be rather inconvenient if I couldn't fit any of them all of a sudden...! 😅

Video transcript :-
Omeprazole belongs to the drug class of proton pump inhibitors and is widely used in gastric acid related disorders to reduce acid secretion.
Major indications of omeprazole include the following.
In the treatment of peptic ulcer and gastroesophageal reflux diseases.
Zollinger-Ellison syndrome.
As a part of the Helicobacter pylori eradication therapy.
For prevention of gastritis in patients who are taking long term non-steroidal anti-inflammatory drugs.
And for stress ulcer prophylaxis in critically ill patients.
Omeprazole is a prodrug that becomes activated in the acidic environment of parietal cells in the stomach.
Once activated, it binds irreversibly to the H+ K+ ATPase pump, also known as the proton pump, on the gastric parietal cell membrane.
This inhibits the final step in gastric acid production, reducing both basal and stimulated acid secretion.
The effects persist for 24-48 hours due to the irreversible nature of the binding, even after the drug is metabolized.
Oral route is the most common method of administration of omeprazole.
Capsules, tablets, or suspensions are available for this purpose.
Intravenous route is reserved for patients unable to tolerate oral therapy or in emergency conditions like upper gastrointestinal bleeding.
Omeprazole is rapidly absorbed in the small intestine with an oral bioavailability of 30-40%.
It should be taken before meals to prevent delays in the absorption of the drug.
Omeprazole is extensively metabolized in the liver by cytochrome P450 enzymes, primarily CYP2C19 and CYP3A4.
And the metabolites are excreted primarily via the urine, and to a much lesser extent, in feces.
Omeprazole is generally well-tolerated by most individuals.
Potential adverse effects include the following.
Gastrointestinal effects such as nausea, vomiting, diarrhea, constipation, and abdominal pain.
Central Nervous System effects such as headache, dizziness, and rarely, confusion or somnolence.
Long-term effects include hypomagnesemia, which can lead to muscle spasms, seizures, or arrhythmias.
Vitamin B12 deficiency due to reduced gastric acid production.
Increased risk of fractures due to impaired calcium absorption.
Increased risk of Clostridioides difficile infections and other enteric infections.
And very rarely, chronic kidney disease.
Contraindications for omeprazole therapy include the following.
Hypersensitivity to omeprazole or other proton pump inhibitors.
Concurrent use with certain drugs such as clopidogrel and ketoconazole.
And severe liver disease.

Proton-pump inhibitors(PPIs) are powerful antacids and one of the most widely sold drugs in the world. Find out about the side effects of these popular drugs.

4:37 Thanks for watching!

In this video, we’re going to take a look at proton-pump inhibitors, also known as PPIs. PPIs are the most potent inhibitors of stomach acid production. They work by blocking the pump in your stomach that releases hydrochloric acid.

Proton-pump inhibitors are one of the most popular drugs in the world. They are used to treat conditions like acid reflux, GERD, gastritis, and stomach ulcers.

Proton-pump inhibitor use does not come without side effects. Here are some of the unwanted side effects that come along with PPIs:
•Headache
•Nausea
•Diarrhea
•Abdominal Pain
•Gas
•Anxiety
•Dizziness
•Fatigue
•Bone Fractures
•Muscle Damage
•Increased risk of the following health problems: dementia, kidney disease, C-Diff, SIBO, fungus, candida, polyps, colitis, and hypochlorhydria(low stomach acid).

Additional symptoms caused by hypochlorhydria(low stomach acid) include the following:
• Anemia
• Decreased vitamin B12
• Decreased absorption of vitamin C, B, and K
• Increased risk of infection
• Overgrowth of bacteria
• Increased risk of carcinoma in stomach

To increase your stomach acid, try drinking 2 tablespoons of apple cider vinegar mixed in a glass of water, with each meal. If you don’t like the taste, try adding stevia.

Betaine hydrochloride is a natural acidifier that can also be used, especially if you need something a bit stronger. The only time you wouldn't want to use these acidifiers is if you have an ulcer.

Instead of using a PPI to deal with GERD and acid reflux, the best thing to do is address your diet.

OTC: Omeprazole
OTC: Omeprazole Side Effects 2 Views • 5 months ago

Having stomach issues like heartburn or acid reflux? This video is for you! I'll be going over omeprazole, an over-the-counter medication used to treat heartburn or acid reflux.

Omeprazole, which is sold under the brand name Prilosec, is used to treat frequent and uncomplicated cases of heartburn in adults. Frequent means that heartburn occurs at least twice a week. This medication works by suppressing acid secretion.

Omeprazole is available as a capsule or tablet. It is best to take this medication 30 to 60 minutes before meals. Be sure to take with a full glass of water and swallow the tablet or capsule whole, making sure not to crush or chew. You may take this medication with antacids.

Typical Dosing: Oral: 20 mg once daily for 14 days; treatment may be repeated after 4 months if needed.

This medication should not be taken if you have trouble or pain when swallowing. Do not take omeprazole if you are vomiting up blood, or have bloody or black stools. Additionally, do not use if you experience heartburn with lightheadedness, dizziness or sweating, or any chest or shoulder pain with shortness of breath or radiating pain, as these are serious conditions that you should report to your doctor or pharmacist. If your heartburn continues beyond 14 days of treatment or if you need more than one course of treatment every 4 months, be sure to inform your healthcare provider.

Omeprazole can cause common adverse effects like headache, nausea, diarrhea, or gas .

Omeprazole can interact with several medications and it should not be taken if you are taking clopidogrel, or Plavix. Be sure to inform your pharmacist that you are taking omeprazole, so they can check for any other potential drug interactions. If you have had heartburn for longer than 3 months, have unexplained weight loss, or nausea and vomiting, tell your doctor or pharmacist before beginning this medication.

In summary, omeprazole is an over-the-counter medication used to treat heartburn and acid reflux. This should be taken once daily, for fourteen days, with a repeat treatment after four months if necessary. It is important to talk to your doctor or pharmacist if you are experiencing weight loss or mood changes, if you have trouble swallowing, or have bloody or black stool. If you have questions about omeprazole, talk to your doctor or pharmacist today.

Doctor explains CO-CODAMOL (Paracetamol/Codeine) | Doses, side effects, interactions and more!
Doctor explains CO-CODAMOL (Paracetamol/Codeine) | Doses, side effects, interactions and more! Side Effects 2 Views • 5 months ago

In this video Doctor O'Donovan explains key facts about Co-codamol - a combination painkiller made up of paracetamol and codeine. It's used to treat various types of pain including headaches, muscle pain, migraines, and toothache and it is especially helpful when common painkillers like ibuprofen or aspirin aren't effective.

What is Co-Codamol?

Co-codamol is a combination of two medicines - paracetamol and codeine phosphate. It is known by its brand names Kapake, Solpadol and Tylex.

What are the strengths and doses of co-codamol?

Co-codamol tablets and capsules come in 3 different strengths, containing 8mg, 15mg or 30mg of codeine. All 3 strengths contain 500mg of paracetamol – the same as in a standard paracetamol tablet or capsule.

The strength of co-codamol appears as 2 numbers on the packet. For example, 8/500 means each tablet or capsule contains 8mg of codeine and 500mg of paracetamol.

You can buy the lowest strength of co-codamol (8/500) without a prescription but only from a pharmacy. The higher strengths (15/500 and 30/500) are only available on prescription from a doctor.

What are the doses of co-codamol?

Adults can take 1 or 2 tablets (of any strength) up to 4 times in 24 hours. It's important to leave a gap of at least 4 to 6 hours between doses of co-codamol. The maximum dose is 8 co-codamol tablets in 24 hours.

Taking too much co-codamol can be very dangerous. That's because the paracetamol in it can cause liver damage. Do not increase the dose of co-codamol or take more than 2 tablets at once, even if your pain is very bad.

Can I drink alcohol if I take co-codamol?

According to NHS UK "It's best to stop drinking alcohol during the first few days of treatment until you see how co-codamol affects you. Drinking alcohol while you're taking co-codamol can make you feel more sleepy. It can also increase the risk of serious side effects."

Can I become addicted to co-codamol?

If you take co-codamol regularly for a long time, you can become addicted to the codeine in it. That is because codeine (one of the ingredients) is addictive. It is really important that you are aware of this. If you have bought co-codamol at a pharmacy, follow the instructions that come with the medicine and only take it for up to 3 days at a time.

You might also know this drug by it's trade names: Calpol, Tylenol and Panadol. It's good for headaches, fever and booboos. Analgesia and antipyretic.

As always we go through things in simple terms and then for the hardcore enthusiasts we're into the complex stuff.
