Losartán es un fármaco perteneciente a un grupo de medicamentos denominados antagonistas de los receptores de la angiotensina II. La angiotensina II es una sustancia producida en el cuerpo que se une a unos receptores presentes en los vasos sanguíneos, provocando que éstos se estrechen y dando lugar a un aumento de la presión arterial.

Losartán impide la unión de la angiotensina II a estos receptores, lo que origina que los vasos sanguíneos se relajen y, por tanto, que disminuya la presión arterial. Losartán además disminuye el empeoramiento de la función renal en pacientes con presión arterial alta y diabetes tipo 2.

Así pues, losartán está indicado en el tratamiento de la presión arterial elevada, en el tratamiento de pacientes con insuficiencia cardiaca crónica y para proteger el riñón en pacientes hipertensos con diabetes tipo 2 que presenten análisis clínicos alterados.

En el mercado existen presentaciones de losartán en forma de comprimidos de 12,5; 25; 50 y 100 mg.
Para el tratamiento de la hipertensión, la dosis se debe ajustar de forma individualizada según los valores de presión arterial de cada paciente. Normalmente la dosis inicial en adultos es de 50 mg al día y, en niños y adolescentes mayores de 6 años que pesen entre 20 y 50 kg, es de 25 mg al día.

Después, en algunos pacientes adultos, la dosis se puede aumentar hasta 100 mg al día y en los niños y adolescentes comentados anteriormente, se puede aumentar la dosis hasta un máximo de 50 mg, una vez al día.

Sin embargo, para el tratamiento de la insuficiencia cardiaca, la dosis inicial es de 12,5 mg al día. Normalmente, la dosis se debe aumentar de forma gradual semanalmente. Es decir:
o 12,5 mg al día durante la primera semana
o 25 mg al día durante la segunda semana
o 50 mg al día durante la tercera semana
o 100 mg al día durante la cuarta semana
o y 150 mg al día durante la quinta semana), hasta llegar a una dosis de mantenimiento determinada por el médico.

Eso sí, es importante saber que sólo se puede usar una dosis máxima de losartán de 150 mg una vez al día.
Este medicamento puede tomarse con o sin alimentos y es importante tomar el comprimido a la misma hora cada día.

Los efectos máximos de losartán, al igual que los de cualquier antihipertensivo, pueden tardar en alcanzarse hasta 3-6 semanas. Por lo tanto, si al principio del tratamiento ves que tu presión arterial no está bien controlada, consulta a tu médico, pero no modifiques la dosis sin su consentimiento.

Las reacciones adversas más habituales suelen estar relacionadas con la disminución de la presión arterial que este medicamento produce, de tal manera que son muy frecuentes los mareos, el dolor de cabeza o el enrojecimiento de la cara, al igual que con cualquier otro fármaco antihipertensivo.

Estas reacciones suelen ser más frecuentes al inicio del tratamiento y se reducen paulatinamente según lo continuamos. Por ello, es importante que al iniciar el tratamiento tengas cuidado al conducir o realizar actividades que pudieran resultar peligrosas y que requieran de concentración.
Cabe destacar que la reacción adversa más importante es la aparición de reacciones alérgicas graves. Si notas hinchazón en los labios, la cara o la lengua, acude lo más rápidamente posible a tu médico.

Entre las principales recomendaciones, es fundamental no utilizar losartán sin receta médica y no aumentar la dosis indicada por el médico. Además, es importante cuidar la alimentación y realizar ejercicio físico.

Por último, desecha el tratamiento que te haya sobrado en el punto SIGRE de tu farmacia.

Famotidine, sold under the brand name Pepcid among others, is a medication that decreases stomach acid production.[2] It is used to treat peptic ulcer disease, gastroesophageal reflux disease, and Zollinger-Ellison syndrome.[2] It is taken by mouth or by injection into a vein.[2] It begins working within an hour.[2]
Common side effects include headache, intestinal upset, and dizziness.[2] Serious side effects may include pneumonia and seizures.[2][3] Use in pregnancy appears safe but has not been well studied while use during breastfeeding is not recommended.[4] It is a histamine H2 receptor antagonist.[2]

How Famotidine works
Famotidine is a histamine H2 receptor antagonist. It works by reducing the acid produced in the stomach. This helps to relieve acid-related indigestion and heartburn.

Drug class: SGLT-2 inhibitors.

What is Farxiga?.

Farxiga is an oral diabetes medicine that helps control blood sugar levels. Dapagliflozin works by helping the kidneys get rid of glucose from your bloodstream.

What is Farxiga used for?.

Farxiga is used together with diet and exercise to improve blood sugar control in adults with type 2 diabetes mellitus. This medicine is not for treating type 1 diabetes.

Farxiga is also used to lower the risk of death from heart attack, stroke, or heart failure in adults with type 2 diabetes who also have heart disease.

Farxiga is also used to lower the risk of needing to be in the hospital for heart failure in adults with type 2 diabetes who also have heart disease.

Farxiga is also used to lower the risk of further worsening of kidney disease, end-stage kidney disease (ESKD), death due to cardiovascular disease, and hospitalization for heart failure in adults with chronic kidney disease.

Warnings:

You should not use Farxiga if you have diabetic ketoacidosis, severe kidney disease, or if you are on dialysis.

Tell your doctor if you are sick with vomiting or diarrhea, or if you eat or drink less than usual.

Taking Farxiga can make you dehydrated, which could cause you to feel weak or dizzy (especially when you stand up).

Farxiga can cause serious infections in the penis or vagina. Get medical help right away if you have burning, itching, odor, discharge, pain, tenderness, redness or swelling of the genital or rectal area, fever, or if you don't feel well.

Some people taking this medicine have had bladder cancer, but it is not clear if dapagliflozin was the actual cause.

Before taking this medicine:

You should not use Farxiga if you are allergic to dapagliflozin, or if you have:

severe kidney disease (or if you are on dialysis); or

diabetic ketoacidosis (call your doctor for treatment).

To make sure Farxiga is safe for you, tell your doctor if you have ever had:

polycystic kidney disease;

liver disease;

bladder infections or other urination problems;

problems with your pancreas, including surgery;

alcoholism, or if you currently drink large amounts of alcohol; or

if you are on a low salt diet.

Follow your doctor's instructions about using Farxiga if you are pregnant or you become pregnant. Controlling diabetes is very important during pregnancy.

You should not use dapagliflozin during the second or third trimester of pregnancy.

Do not breastfeed.

Farxiga is not approved for use by anyone younger than 18 years old.

How should I take Farxiga?.

Take Farxiga exactly as prescribed by your doctor. Follow all directions on your prescription label and read all medication guides or instruction sheets. Your doctor may occasionally change your dose.

You may take Farxiga with or without food.

Your blood sugar will need to be checked often, and you may also need to test the level of ketones in your urine. Dapagliflozin can cause life-threatening ketoacidosis (too much acid in the blood). Even if your blood sugar is normal, contact your doctor if a urine test shows that you have high ketones in the urine.

Blood sugar can be affected by stress, illness, surgery, exercise, alcohol use, or skipping meals.

Low blood sugar (hypoglycemia) can make you feel very hungry, dizzy, irritable, or shaky. To quickly treat hypoglycemia, eat or drink hard candy, crackers, raisins, fruit juice, or non-diet soda. Your doctor may prescribe glucagon injection in case of severe hypoglycemia.

You may get dehydrated during prolonged illness. Call your doctor if you are sick with vomiting or diarrhea, or if you eat or drink less than usual.

This medicine can affect the results of certain medical tests. Tell any doctor who treats you that you are using Farxiga.

Your treatment may also include diet, exercise, weight control, and special medical care.

Store at room temperature away from moisture and heat.

Farxiga dosing information:

Usual Adult Dose for Diabetes Type 2:

To improve glycemic control:
Initial dose: 5 mg orally once a day
-May increase to 10 mg orally once a day for additional glycemic control if lower dose has been tolerated
Maximum dose: 10 mg/day

To reduce the risk of hospitalization for heart failure: 10 mg orally once a day.

Farxiga side effects:

Get emergency medical help if you have signs of an allergic reaction to Farxiga: hives; difficult breathing; swelling of your face, lips, tongue, or throat.

Seek medical attention right away if you have signs of a genital infection (penis or vagina): burning, itching, odor, discharge, pain, tenderness, redness or swelling of the genital or rectal area, fever, not feeling well. These symptoms may get worse quickly.

Iron deficiency is one of the most common mineral deficiencies, but iron can also be dangerous if you’re getting too much. Iron toxicity is one of the most underdiagnosed and overlooked conditions.

When you take iron supplements, you don't absorb very much. The excess iron accumulates in the digestive tract, causing constipation, nausea, diarrhea, bloating, and irritation of the colon. The type of iron in iron supplements feeds pathogens.

Red meat is a better source of iron. Heme iron from red meat and fish is absorbed much more easily. It goes straight into the bloodstream. Red meat also has other nutrients like B vitamins, zinc, and selenium. High-quality protein also helps to balance iron.

Many processed foods contain added iron in the forms of ferrous fumarate, ferrous sulfate, and ferrous gluconate. The most common sources of iron are supplements, fortification, and cooking with cast iron cookware. Liver damage will also cause you to accumulate iron.

A lack of copper is a common cause of dysfunctional iron because copper helps control iron in the body. Many enzymes dependent on copper relate directly to the control of iron.

The first enzyme involved with copper converts iron into a less accumulative and less toxic form. Copper also allows you to transport iron through the body. Copper is involved in a powerful antioxidant that reduces oxidation caused by iron!

Glyphosate, antibiotics, high fructose corn syrup, and wheat products can cause copper deficiency. Iron deficiency anemia may really be iron dysfunctional anemia, which is often related to a copper deficiency. Oysters, shellfish, and organ meats are the best food sources of copper.

How Iron Supplements work for iron deficiency?

Iron is needed for the production of hemoglobin, which is an essential ingredient in red blood cells. It is a mineral that’s super important for your health and Iron deficiency is the most common nutritional deficiency in the US.

All of your cells contain some iron, but most of the iron in your body is actually in your red blood cells. It helps our muscles store and use oxygen, which is what essentially gives you life. Its also a part of a very important protein in our bodies called hemoglobin, which helps carry oxygen from our lungs to the rest of our bodies. And When we don’t have enough iron, these functions can slow down leaving us to feel weak and fatigued, making it harder to maintain our body temperatures, it can decrease immune function, and can cause iron deficiency anemia.

Iron deficiency anemia happens when our bodies have an increased need for iron:
-Pregnancy
-Poor absorption of the iron itself
-If you are a vegetarian and have to rely on only non-heme sources of iron from plants
-If you've had major blood loss.

In any case if you are taking iron supplements, available in capsules, tablets, chewables, and liquids, the most common one being ferrous sulfate at 325 mg, there are important tips you should know that can help you make the best use of that iron correctly and help relieve iron deficiency.

1.) Iron is best absorbed on an empty stomach. Food can heavily decrease its absorption. Milk, calcium, high fiber foods, caffeine and antacids should not be taken at the same time. A good rule of thumb is to take it 1 hour before you eat, or 2 hours after you eat. Research also shows, that if you take your iron supplement with a drink containing Vitamin C, like orange juice, it can actually help improve the absorption.

2.) Iron tablets may cause other drugs you are taking to not work as well, these include: tetracyclines, penicillins, and ciprofloxacin and drugs used for hypothyroidism, Parkinson disease, and seizures. So talk to your doctor or pharmacist letting them know you take iron supplements before you take any of these. Thirdly:

Lets say You start taking iron on an empty stomach but it has left you with stomach pain, constipation, diarrhea, and feeling sick: The good thing is that this will go away on its own over time when your body gets used to it. But if it doesnt: Your doctor can lower the dose and work its way up slowly to the preferred level or they can switch to a different iron supplement. If you still feel discomfort, speak to your doctor to see if its okay to take it with a small bit of food to avoid these side effects.

4.) Iron supplements can have side effects, so besides stomach discomfort, constipation, diarrhea, nausea or vomiting. It can leave your stool looking black, which is normal as it may be a sign that the tablets are working correctly. It can be alarming at first but dont be worried. With that being said, if your stool is tarry, has red streaks in it, let your doctor know then, to go from there.

5.) Liquid forms of iron can stain your teeth,so try mixing the iron with water or other liquids like orange juice and drinking the medicine with a straw.

Overall though, most people respond well to taking iron supplements, and your levels should return to normal after 2 months of iron therapy for most people. But You may need to continue taking supplements for another 6 to 12 months to build up the body's iron stores in the bone marrow. Never hesitate to ask your doctor or pharmacist how long you should be taking them or if you have any questions at all.

Because the human body has no mechanism to rid itself of excess iron, we evolved to regulate our absorption of iron. If we have low iron stores, our intestines boost their iron absorption. If our iron stores are too high, our intestines block the absorption.

But this only works with the primary source of iron in the human diet—the iron found in plant foods. However, our digestive system cannot regulate the iron in ingested blood—heme iron. The iron in animal foods can go right through our intestinal barrier—even if we already have too much in our system.

So how to I make sure I’m getting the right amount of iron? Simple, by eating lots of healthy iron-rich foods, like chickpeas and pumpkin seeds, while consuming vitamin C-rich foods in the same meal, such as citrus fruits, broccoli, bell peppers, which can improve plant iron absorption. I also would advise avoiding drinking tea and coffee with your meals because this can impair iron absorption.

Check your iron levels at least once a year or every 6 months for more accuracy. This can be done at your primary car physician and is most of the time covered by insurance.

So in conclusion I’m classifying specific iron supplements are harmful unless they are included with a multivitamin. I would never outright supplement with doses higher than the upper limit unless of course you’re under the supervision of a medical provider. I would highly recommend getting your iron from your diet and monitoring it through blood tests.
