Study this Barbiturates mnemonic and other NCLEX mnemonics with Pixorize.
Barbiturates are a class of drugs, easily recognizable with their “-barbital” ending, with the most common drug being phenobarbital. Barbiturates are CNS depressants, meaning they slow down brain and neural activity. This makes barbiturates useful in treating seizures and treating insomnia. Side effects of barbiturates include sedation, dependence, and respiratory depression. Barbiturates are not commonly prescribed anymore because of their high abuse potential and the risk of fatal overdose.

Dr. Steve Wolf: Phenobarbital is one of our oldest anti-seizure medications that have been around. It's indicated for both seizure types. The major problem with it is sedation, sleepiness. That's why the newer medications have a lot less sleepiness and side effects and waking issues.
Patty McGoldrick: Now it still is used a lot of times in infants, children who present with neonatal seizures, and that's one of the first medications that's given to them. It's also used for people who present in status, which is prolonged seizures, and who may be in the emergency room. Not necessarily as the first medication that's used for that, but somewhere down the line.
Dr. Wolf: The problem is in the emergency room, when you use it, it can really knock you out, and put you to sleep. And of course, if you give too much of it, it actually can affect your respiration.
Patty: Now, the other big issue with phenobarbital is that we've been using it for years to control seizures in newborns and in infants. And actually, the latest research has shown it can cause much more cognitive damage than we initially thought. So we're trying not to use that as rapidly as we did in the past. We use some of the newer medications. We try to get children off phenobarbital pretty quickly if they were using it.
Dr. Wolf: There were studies in the 80's showing that it can affect hyperactivity in young children, as well as reading scores and attention scores. So again, this drug can have a lot of potential cerebral side effects. That's why it's nice about some of the newer medications that are out there that might have less cognitive effects. So it's important to discuss with the people taking care of you whether this is the right medication for you, and what are the other choices.

Add this Phenytoin NCLEX mnemonic and other mnemonics with Pixorize to your NCLEX study plan.
Phenytoin is an anticonvulsant drug used to treat seizures. Phenytoin has a narrow therapeutic range of 10-20 mcg/mL. High doses of phenytoin above this cutoff can lead to symptoms of acute toxicity. These symptoms include ataxia, slurred speech, and nystagmus. The provider should be notified immediately if these symptoms are seen. Another expected side effect of phenytoin is gingival hyperplasia, so encourage your patients to practice good oral hygiene and visit the dentist regularly.

While phenobarbital has been useful in treating conditions like anxiety; it has become a common drug for abuse and addiction. It’s a fast-acting drug and the effects do not usually last as long as other medications.
This makes people bingeing for more doses to keep its effects or use it with other substances including alcohol. When abused, it can cause several side effects and treatment is only possible in a recovery center.

Before the GLP-1's like Semaglutide, Retatrutide and tirzepatide, phentermine was the go-to weight loss medication. Phentermine is an old school weight loss medication with some new and interesting research surrounding it. It's been around for over 60 years and has helped people lose weight predominately though appetite suppression. Even though there is a ton of research surrounding phentermine, there is some new research as well. Namely, long term phentermine use. We look at 2 different studies that take a look at off-label phentermine use including some patients who took this medication for 24 months non-stop. The dosing of phentermine is all over the board but one source showed people taking 15mg per day and 30mg per day.
Phentermine, just like all medications, can have side effects. Phentermine side effects include physical and psychological. Phentermine has a number of contraindications so make sure you're working with a qualified medical professional if this is a medication you're interested in.
We also briefly take a look at a combination weight loss therapy - phentermine and topiramate brand name Qsymia.

Study this Ephedrine / Phenylephrine NCLEX mnemonic and other mnemonics with Pixorize.
Ephedrine, pseudoephedrine, and phenylephrine are medications that have different effects depending on their route of administration. When given nasally or orally, they can be used to treat congestion. When used as a decongestant, a potential side effect is rebound nasal congestion, so patients should not take these medications for more than 3-5 days and should taper off the drug slowly. When given through an IV, these drugs act systemically as vasopressors, where they constrict the blood vessels to increase blood pressure. Regardless of the route of administration, side effects to look out for include CNS stimulation, which can present as insomnia, restlessness, agitation, or nervousness. These drugs can also cause uncontrolled hypertension, and should not be given concurrently with other drugs that cause hypertension, or to patients who already have high blood pressure and other cardiovascular conditions.

Phenylephrine is a selective alpha-1 adrenergic agonist used as a decongestant, vasopressor, and mydriatic agent. It works by causing vasoconstriction, which reduces nasal congestion and raises blood pressure, but should be used cautiously due to risks like hypertension and rebound congestion.

- Phenytoin is an anticonvulsant agent. It blocks voltage-gated sodium channels by selectively binding to the channel in the inactive state and slowing its rate of recovery. Thereby stabilizing threshold against hyperexcitability.
- It is available as 30 mg, 50 mg and 100 mg tablets. And also available as oral suspension and as injection.
- Phenytoin is used in the treatment of epilepsy, except absence seizure (petit mal).
- it is used in the control of grand mal and psychomotor seizures, prevention and treatment of seizures occurring during or after neurosurgery, control of grand mal type of status epilepticus (using parenteral administration).

Duration
- According to clinical response
Contra-indications
- Do not administer in case of hypersensitivity to phenytoin.
Adverse effects
- May cause:
- gastro-intestinal disturbances: gingival hypertrophy, nausea, vomiting.
- neurological disorders: dizziness, visual disturbances, mental confusion.
- blood disorders: monitor blood counts if possible and administer folic acid in case of prolonged use, as it may cause folic acid deficiency.
- allergic reactions: cutaneous eruption, fever, adenopathy.
Precautions
- Do not stop treatment abruptly, decrease daily doses gradually.
- It is not recommended to combine phenytoin with oral contraceptives, sulphonamides, or chloramphenicol. Combination with other drugs must be closely monitored (diazepam, phenobarbital, digoxin, corticosteroids, etc.).
