Ranolazine is primarily used for chronic angina management. I discuss pharmacology, drug interactions, adverse effects, and more in this podcast episode.
Ranolazine is well known to have drug interactions. CYP3A4 is of major importance but there are other subtle drug interactions that are important.
QTc prolongation has been reported with ranolazine so it is important to recognize risk factors and other medications that may contribute to this concern.
Enzyme inducers like carbamazepine, phenytoin, and St. John’s wort are all associated with reducing the concentrations of ranolazine.

Assalamu Alaikum
Ranolazine is new FDA approved drug for the management of of chronic angina pectoris. Ranolazine inhibits persistent or late inward sodium current in heart muscle in a variety of voltage-gated sodium channels. Inhibiting that current leads to reductions in intracellular calcium levels. It is reduces the HbA1c and fasting blood glucose levels, so also used for diabetes mellitus. This in turn leads to reduced tension in the heart wall, leading to reduced oxygen requirements for the muscle. It may be used concomitantly with β blockers, nitrates, calcium channel blockers, antiplatelet therapy, lipid-lowering therapy, ACE inhibitors and angiotensin receptor blockers. It is also effective at preventing atrial fibrillation, and has been studied as monotherapy as well as in combination with other medications used to treat irregular heartbeat.

🩺ABOUT DR. BREWER
Dr. Brewer started as an Emergency Doctor. After seeing too many preventable heart attacks, he went to Johns Hopkins to learn Preventive Medicine. While there, he went on to run the post-graduate training program (residency) in Preventive Medicine. From there, he made a career of practicing and managing preventive medicine and primary care clinics. His later role in this area was Chief Medical Officer for Premise, which has close to 1,000 primary care/prevention clinics. He was also the Chief Medical Officer for MDLIVE, the second largest telemedicine company. More recently, he founded PrevMed, a heart attack, and stroke prevention clinic.
At PrevMed, we focus on heart attack, stroke, and cognitive decline. We serve patients who have already experienced an event as well as those who have not developed a diagnosis or event. Dr. Brewer provides services via telemedicine. We find a lot of undiagnosed prediabetes or insulin resistance. Treating unrecognized risk factors like prediabetes allows reduction of risk and prevention of disease.

Repatha is a prescribed medication called a PCSK9 inhibitor which is very effective at lowering bad LDL cholesterol. Repatha works differently than statins. While effective, some people report side effects such as really bad muscle pains after taking Repatha. In a previous video, I interviewed Robert, a physician and marathoner, about what happened to him after he took this medication. That video was so popular that I wanted to bring Robert back to see how he's doing now that he's no longer taking it. There is life after Repatha.

This comprehensive overview is suitable for the layperson and the student of medicine, alike. I review the indications, contraindications, mechanisms of action, major side effects, risks and benefits of this medication, and even delve into a few rare side effects and offer a few esoteric pearls.
This video covers the difference between a receptor agonist, antagonist and partial agonist, and explains how, depending on the context, a partial agonist may be relatively antagonistic vs. enhancing of a particular neurotransmitter action. It also defines examples of psychotic negative symptoms (as opposed to positive symptoms, like delusions and hallucinations), and reviews the implications of this medication's extraordinarily long half-life. I also briefly cover tardive dyskinesia and neuroleptic malignant syndrome.
This information is sophisticated, but presented in a straightforward manner, and I do believe that the non-professional consumer of mental healthcare information will find it useful and informative.
