sotalol
sotalol Side Effects 2 Views • 5 months ago

A drug recommended to use to treat and prevent only life-threatening ventricular arrhythmias due to potentially serious side effects (generally not recommended for less severe arrhythmias). It is taken by mouth or intravenously.

It is a non-selective beta-adrenergic receptor blocker with class II (beta-blocker) and III (potassium channel blocker) antiarrhythmic properties. It is a "pure" potassium channel blockers which produces little or no other cardiac ion channel electrophysiologic effect, as well as dofetilide and possibly ibutilide.

It can cause life-threatening ventricular tachycardia associated with QT interval prolongation. To minimize the risk of drug-induced arrhythmia, give it by oral administration in a facility that can provide cardiac resuscitation and continuous electrocardiographic monitoring.
Do not use it if baseline corrected QT is 450 msec or greater. If the QT interval prolongs to 500 msec or greater, reduce the dose, lengthen the dosing interval, or discontinue the drug.

It is nearly completely absorbed after oral administration and undergoes essentially no first-pass hepatic metabolism, resulting in its absolute bioavailability at 90-100%.
It is primarily eliminated by renal excretion, with approximately 80-90% of a dose being excreted unchanged in the urine, and a small amount is excreted unchanged in the feces.
Calculate creatinine clearance to determine appropriate dosing.

(Common side effects)
• bradycardia
• hypotension
• chest pain
• shortness of breath
• dizziness
• weakness
• blurred vision
• nausea, vomiting
• edema

(Serious side effects)
• QT prolongation
• heart failure
• bronchospasm

Why you should NOT use aromatase inhibitors while on TRT
Why you should NOT use aromatase inhibitors while on TRT Side Effects 2 Views • 5 months ago

Hi, I am Dan Reznicek from Pacific Northwest Urology, today we are going to be talking about aromatase inhibitors drugs sometimes referred to as estrogen blockers. We are going to discuss their use in men who are on Testosterone replacement therapy. There is a lot of mis-information online and in other youtube videos online out there claiming the benefits of these drugs. Often the muscle building “bros” talk about “balancing your estrogen”or optimizing the level. But it isn’t just misinformation online. I have also been getting a lot of men coming to my practice that I see from testosterone clinics who are on the drug. Doctors are prescribing them these medications that we are going to find out later, cause men erectile dysfunction and decreased libido. I ask them why they are on the medication. They say they want to normalize or balance their estrogen levels. They want me to check their estrogen levels if the are o testosterone replacement therapy even if they are doing well overall to make sure they aren’t getting too much estrogen. So unlike other videos that just give opinions on these medications, I am going to review the science and clinical trials that actually tested these drugs. I am going to show studies on how we are learning more every year about the physiology of estrogen in men and find that the hormone is essential to many tissues in the body. We are then going to review studies on the use of aromatase inhibitors. These will demonstrate clearly that taking these medications while on testosterone therapy has been shown to cause a host of medical problems from decreased bone mineral density to low libido and erectile dysfunction. That is right, it sounds crazy but the hormone estrogen is essential for male libido and erectile function. Even just a slightly lower level of estrogen can cause a problem.

Naltrexone for weight loss: Does it work?
Naltrexone for weight loss: Does it work? Side Effects 3 Views • 5 months ago

How to use Naltrexone for weight loss, how naltrexone helps curb appetite, and how to use it to burn fat.

Did you know that naltrexone, also referred to as low dose naltrexone, can help with weight loss?

It's actually an effective weight loss medication provided that it is used in the right way.

LDN or naltrexone or low dose naltrexone (all names for the same medicine) is typically used to treat alcohol dependence. But it was found that it can also be used to treat autoimmune conditions. Then later it was found that LDN also seems to have some weight loss properties.

LDN has very few side effects and it works through a different mechanism compared to other medications which makes it a great add-on for many people.

AS ALWAYS, make sure that you use LDN with OTHER weight loss therapies such as diet, exercise, and so on. Using it in this way is obviously the best.

We are going to discuss 6 ways that LDN (or low dose naltrexone) can help with weight loss:

#1. Naltrexone targets insulin resistance.

Insulin resistance is one of the primary drivers of weight gain and obesity in MANY people. Up to 50% of people in the US suffer from insulin resistance in some form.

LDN helps to drive down insulin levels which can help with weight loss.

#2. Naltrexone increases growth hormone.

Some studies show that naltrexone can also increase or normalize growth hormone levels. And because growth hormone helps maintain muscle mass and fat mass in balance, it will help manage weight.

#3. Naltrexone modulates your appetite.

Naltrexone also helps to MODULATE your appetite. Notice I said modulate and not suppress. It actually helps to reduce cravings for foods you shouldn't be eating and it helps you to eat the normal amount of calories you should be each day (instead of overeating).

#4. Naltrexone reduces inflammation.

Naltrexone ALSO helps to lower inflammation. Inflammation is a driver of fat cell dysfunction and weight gain by itself. So if you reduce inflammation you will help with weight loss indirectly.

This reduction in inflammation is also why LDN is often used to treat autoimmune conditions.

#5. Naltrexone improves your sleep.

Naltrexone has also been shown to regulate sleep cycles and to help improve sleeping patterns. Because a lack of sleep is associated with obesity, anything which improves your sleep may help with weight loss.

#6. Naltrexone improves and increases thyroid hormone.
