Should psychiatrists stick with FDA-approved SSRIs like sertraline and paroxetine for PTSD, or consider off-label options like prazosin? This episode dives into the evidence, exploring prazosin’s benefits for PTSD nightmares and hyperarousal, alongside tips for titration and alternative treatments like fluoxetine, trazodone, and topiramate.

When it comes to PTSD treatment, the debate between sticking to FDA-approved medications or exploring off-label options is ongoing. SSRIs like sertraline and paroxetine are FDA-approved but have limitations, including inconsistent efficacy in military trauma and significant side effects like sexual dysfunction, withdrawal symptoms, and weight gain. This episode of the Carlat Psychiatry Podcast delves into why some experts, including Dr. Chris Aiken and Kellie Newsome, recommend fluoxetine over FDA-approved SSRIs and why prazosin might be a first-line choice for PTSD patients with nightmares and nocturnal hyperarousal.
Prazosin, an alpha-1 antagonist, has shown significant benefits in reducing PTSD nightmares and hyperarousal, with additional benefits for daytime symptoms like hypervigilance, difficulty concentrating, and anhedonia. Despite a large 2018 trial casting doubt on its efficacy, meta-analyses continue to support prazosin for sleep and daytime PTSD symptoms. This episode provides practical guidance on starting and titrating prazosin, including dosage recommendations (starting at 1 mg QHS and titrating up to 12–16 mg/day) and monitoring for risks like orthostatic hypotension and falls. Learn why prazosin’s safety profile and tolerability make it a compelling choice, particularly for patients struggling with SSRI-induced apathy, sleep disruption, or sexual dysfunction.
For patients with alcohol use disorder, the combination of prazosin and cyproheptadine has shown promise in reducing alcohol consumption. The episode also explores other off-label options like clonidine, trazodone, mirtazapine, and topiramate, each with unique benefits and considerations for PTSD comorbidities. While SSRIs remain a standard recommendation, this podcast emphasizes patient-centered care, highlighting the importance of tailoring treatment to individual needs and symptoms.
Additionally, we tackle common misconceptions about medications like mirtazapine’s role in sleep apnea and clarify recent research errors, ensuring listeners stay informed with accurate and practical insights. Whether you’re considering prazosin, fluoxetine, or other pharmacological strategies for PTSD, this episode equips you with evidence-based recommendations and thoughtful commentary on managing this complex condition. Stay tuned for next week’s episode, where we’ll explore topiramate and additional off-label options for PTSD.

Prazocin 2.5 mg tablet | Prazosin 5 mg tablet | Prazosin hydrochloride tablet
Prazosin is used to treat hypertension (high blood pressure) and lower any future risk of heart attack and stroke. In addition to this, it is also used to treat mild prostate gland enlargement in men (known as Benign Prostate Hyperplasia) and heart failure. Hypertension (high blood pressure) is a lifelong or chronic condition in which the blood pressure against the artery walls becomes high. The higher this blood pressure, the harder the heart has to pump. It also used for scorpion sting. Prazosin inhibits the postsynaptic alpha-1 adrenoceptors. This inhibition blocks the vasoconstricting (narrowing) effect of catecholamines (epinephrine and norepinephrine) on the vessels, leading to peripheral blood vessel dilation.
Prazosin contains Prazosin, an alpha-blocker primarily used to treat hypertension (high blood pressure). It acts by relaxing the blood vessels, reducing the heart's workload, and making the heart more efficient at pumping blood throughout the body. Thus, it helps to lower high blood pressure, reducing the chances of any future heart attack or stroke.
Common side effects:- The most common side effects of Prazosin are drowsiness, headache, weakness, dizziness, priapism (prolonged erections), nausea, and feeling exhausted.

Learn about one of the top causes of hypertension and how one important mineral can help.
0:00 Introduction: Essential hypertension
0:32 What is high blood pressure?
1:11 What causes hypertension?
2:29 Potassium and heart health
4:04 Best sources of potassium
5:08 How to lower blood pressure naturally
In this video, we’re going to take a look at hypertension. Ninety percent of all hypertension is considered essential hypertension. This means the cause is unknown. Anti-hypertensive medication is one of the most prescribed medicines in the world.
Normal blood pressure is 120/80. Research suggests a strong relationship between potassium deficiency and high blood pressure. Many people are deficient in potassium because the requirements are so high—4700 mg daily.
You need about half as much sodium as potassium, but many people consume three times as much sodium as potassium!
Potassium directly controls the tone of the vascular system. This directly affects blood pressure. Potassium also supports the endothelial layer of the arteries and helps improve insulin resistance.
Potassium supplements often only contain 99 mg of potassium, which is incredibly low considering how much potassium you actually need.
You can get potassium from salad, but you must consume around 7 to 10 cups daily. This also provides magnesium. The combination of potassium and magnesium can help reduce high blood pressure.
Diets high in carbs and sugar cause you to lose potassium. Adrenaline and cortisol deplete potassium as well.
To support healthy blood pressure levels, try increasing your salad intake. You can also look for a high-quality electrolyte powder that's high in potassium.

Thanks for watching! I hope this explains why potassium is the best mineral for lowering blood pressure naturally. I’ll see you in the next video.

Is my potassium low? What are signs of low potassium?
Potassium helps with fluid balance, muscle contraction, and nerve impulse transmission. If potassium is too low these functions will suffer, and so will your health & happiness.
Your body controls your serum potassium level very tightly. It will steal potassium from muscles, bones and even your brain to keep the serum potassium within normal limits. Therefore, you can have a normal serum potassium lab result, and still be very deficient in potassium.

### **The Effects of High Potassium on Your Body (Hyperkalemia)**
**Potassium** is an essential mineral that helps regulate nerve signals, muscle contractions, and heart function. However, when potassium levels become too high (**hyperkalemia**), it can have serious effects on your health.
### **🔴 Symptoms of High Potassium Levels**
Mild cases may not show symptoms, but as potassium levels rise, you may experience:
✔️ Muscle weakness or cramps
✔️ Irregular heartbeats (arrhythmia)
✔️ Nausea or vomiting
✔️ Numbness or tingling in the hands and feet
✔️ Shortness of breath
✔️ Fatigue or dizziness
### **⚠️ Causes of High Potassium**
🔹 **Kidney Disease** – The kidneys regulate potassium levels, and if they don’t function properly, potassium builds up in the blood.
🔹 **Excessive Potassium Intake** – Eating too many potassium-rich foods (bananas, avocados, potatoes, spinach) or supplements.
🔹 **Certain Medications** – ACE inhibitors, ARBs, and potassium-sparing diuretics can cause potassium retention.
🔹 **Uncontrolled Diabetes** – High blood sugar can lead to potassium imbalances.
🔹 **Severe Dehydration** – Can cause the body to hold onto potassium.
### **🩺 Health Risks of High Potassium**
⚠️ **Heart Problems** – Hyperkalemia can cause dangerous heart rhythm issues, leading to cardiac arrest in severe cases.
⚠️ **Muscle Paralysis** – Extremely high potassium levels can weaken or paralyze muscles.
⚠️ **Nerve Dysfunction** – Can affect nerve signaling, causing numbness or tingling sensations.
### **📉 How to Lower High Potassium Levels**
✔️ **Limit High-Potassium Foods** – Reduce intake of bananas, oranges, potatoes, and beans.
✔️ **Stay Hydrated** – Drinking enough water helps flush excess potassium from the body.
✔️ **Take Medications if Needed** – Doctors may prescribe diuretics or potassium binders.
✔️ **Dialysis for Severe Cases** – In patients with kidney failure, dialysis may be required to remove excess potassium.
### **🩺 When to See a Doctor?**
Seek **immediate medical attention** if you experience:
🔴 Severe irregular heartbeats
🔴 Muscle paralysis or extreme weakness
🔴 Difficulty breathing
**High potassium can be life-threatening, but with the right management, it can be controlled!** 💙💊 Let me know if you need more details! 🚀

0:14 When to limit potassium consumption
1:13 The toxic effects of high potassium
2:05 A couple more things you should know about potassium
3:07 The right sodium and potassium ratio
4:14 Summary
The first thing you should know is that you will rarely if ever, have a problem with potassium if you get it from fresh food—even at 10 cups of vegetables per day. However, there are a few exceptions, including if you:
• Have stage 5 kidney disease
• Take an ace inhibitor
• Take a beta-blocker
• Use potassium-sparing diabetics
• Have Addison's disease
• Have hyperkalemia
Now let's talk about extreme doses of potassium. When you have between 8,000 and 10,000 mg of potassium, you may begin to feel nauseous. If you are already low on potassium and you take something like 14,000 mg of potassium, this could be very dangerous. Keep in mind that potassium doesn't store in the body. If you do prolonged fasting, you can deplete your potassium quickly. To prevent this, I recommend electrolyte fasting, which is including electrolytes in your fast.
If you already are deficient in potassium, vitamin B1 can exaggerate the symptoms. I also recommend that you keep your sodium and potassium levels balanced. If you consume a lot of potassium without any sodium, that's going to be a problem. The same problem happens if you have a lot of sodium and no potassium. You need four times as much potassium as sodium, so make your you have sodium and potassium in a 1 to 4 ratio.
In summary, the average healthy person would have to take a tremendous amount of potassium to develop a problem of high potassium levels in the blood. If you have 1,000-4,000 mg of potassium a day, you're not going to have a problem.

http://www.gregjoneslaw.com For many years, Plavix was the recommended heart medication for patients with a gastrointestinal intolerance for aspirin. The drug's manufacurers, claimed that Plavix was not only more effective than aspirin in preventing heart attacks and strokes, but it also caused fewer ulcers and other gastrointestinal complications. However, recent studies have found that Plavix can cause serious bleeding ulcers and gastrointestinal hemorrhaging in some patients when taken alone or in combination with aspirin or other similar heart drugs. Research also shows that patients with no history of heart disease who are taking Plavix and aspirin combined have a twofold risk of heart attack, stroke, and death. You should NOT take Plavix if you have a medical history of ulcers or gastrointestinal bleeding, PUD, or have recently suffered a trauma or are anticipating an upcoming surgery. Talk to your physician about alternative drug therapy.

Hydroxychloroquine is an anti malaria and autoimmune prescription drug that has shown effectiveness against viral infections. That versatility placed hydroxychloroquine at the forefront of testing for potential COVID-19 treatments. Studies done in a lab have shown the prescription drug can have a fighting chance against coronaviruses, but these results do not necessarily mean the drug is safe or effective on COVID-19 patients. Still, large observational studies began after the FDA allowed emergency use of hydroxychloroquine on COVID-19 patients. So what have they found?.

Chloroquine and Hydroxychloroquine (Plaquenil) are antimalarial drugs that help with symptoms of autoimmune disease. Unfortunately, LONG TERM use of these medications can cause retinal toxicity and can lead to permanent vision loss. In this video, I review the testing used to screen for plaquenil retinopathy during an eye exam.
NOTE: The risk of of Plaquenil Toxicity is higher when the cumulative dose is over 1000g not mg.

These drugs were originally designed to fight malaria but are not often prescribed to help with autoimmune diseases such as rheumatoid arthritis, lupus and sjogrens disease. Unfortunately, these medications can also have a toxic effect on the body. In this video, Joseph J. Allen OD FAAO from Doctor Eye Health reviews plaquenil side effects and the testing used during a plaquenil eye exam with either your optometrist or ophthalmologist.
