Adamantanes such as amantadine and memantine are known to have antiviral properties. The tricyclo-bridged hydrocarbon structure interferes with viroporin protein channel responsible for release of RNA-viruses. In a human model cell line, expression of the CTSL gene coding for the cathepsin L, a lysosomal protease involved in SARS-CoV-2 entry to cells, is significantly downregulated by amantadine. Antiviral effects shown for amantadine and memantine in models for bovine coronavirus, mouse hepatitis virus, human coronavirus OC43, and SARS‐CoV led to the suggestion that adamantanes could be used for COVID-19

Albendazole is a broad-spectrum anthelmintic belonging to the benzimidazole class.
It is effective against a variety of parasitic worm infections, including nematodes, cestodes, and some protozoa.
Albendazole is indicated for the treatment of various infections, including the following.
Nematode infections such as ascariasis, hookworm, trichuriasis, and strongyloidiasis.
Cestode infections such as neurocysticercosis and hydatid disease.
Mixed helminth infections, particularly in endemic regions.
Protozoal infections like giardiasis.
And cutaneous larva migrans.
Albendazole works by binding to beta-tubulin, inhibiting microtubule polymerization in parasitic cells.
This disrupts vital cellular processes such as glucose uptake, leading to depletion of energy stores and eventual death of the parasite.
It exhibits selective toxicity as it has a higher affinity for parasitic tubulin compared to mammalian tubulin.
Albendazole is administered orally as tablets or suspension.
It has poor oral bioavailability. However, the absorption can be enhanced if taken with a fatty meal.
Albendazole is rapidly converted in the liver to its active metabolite, albendazole sulfoxide, which is responsible for most of its therapeutic effects.
The drug and its metabolites are excreted mainly in urine and, to a lesser extent, in bile.
Half-life is 8-12 hours for albendazole sulfoxide.
Major Adverse Effects of albendazole include the following.
Gastrointestinal effects such as abdominal pain, nausea, vomiting, and diarrhea.
Hepatic effects such as elevation of liver enzymes and, rarely, hepatotoxicity.
Hematologic effects such as bone marrow suppression, leading to neutropenia or pancytopenia.
Neurologic effects such as headache, dizziness, or seizures, especially in neurocysticercosis due to inflammatory reactions caused by dying parasites.
And allergic reactions like skin rash, urticaria, or rarely, anaphylaxis.
Contraindications for the use of albendazole include the following.
Known allergy to albendazole or other benzimidazoles.
Pregnancy.
Contraindicated during the first trimester due to potential teratogenic effects.
Women of childbearing age should use contraception during treatment and for one month afterward.
Liver impairment.
And bone marrow suppression.

These are both over-the-counter analgesic medications that are very similar especially when just seeing them on the shelf in the drug store, but there are some key differences to know about when you are taking either of them. In this video, I start off by giving a general overview of NSAIDs (non-steroidal anti-inflammatory drugs), then I break down the key differences between ibuprofen and naproxen. As a nurse practitioner, I am commonly asked about these two drugs and so I hope this video helps you understand the differences between ibuprofen and naproxen as well.

Vitamin D belongs to the family of fat-soluble vitamins, which include vitamins A, D, E and K. These vitamins are absorbed well with fat and are stored in the liver and fatty tissues.

There are two main forms of vitamin D in the diet:

Vitamin D2 (ergocalciferol): Found in plant foods like mushrooms.
Vitamin D3 (cholecalciferol): The flesh of fatty fish (such as salmon, tuna, and mackerel) and fish liver oils are among the best sources. Small amounts of vitamin D are found in beef liver, cheese, and egg yolks.
Natural sources of vitamin D are limited, especially if you're vegetarian or don't like fish.
Foods such as cow's milk, soy milk, orange juice, cereals, and oatmeal are sometimes fortified with vitamin D.

However, sunlight is the best natural source of vitamin D3. The UV rays from sunlight convert cholesterol in your skin into vitamin D3

Limit your calcium intake, especially when taking a high dose of vitamin D. You can also increase fluid intake to 2.5 liters per day to prevent kidney stones.

In this video, we’re going to talk about vitamin D toxicity. Some people are afraid of taking too much vitamin D, when it may not be as scary as you might think. 

According to an article by the Mayo Clinic, vitamin D may not actually be as toxic as was once believed. 

In the 1940s, vitamin D was used for rheumatoid arthritis. But, the amounts being used were between 200,000 and 300,000 IUs of vitamin D. The reason this could have helped is that vitamin D helps support the immune system and helps lower inflammation. 

In the 1950’s they were fortifying products like dairy products with vitamin D. But, there were some complaints from parents with children who had side effects. The fortification of vitamin D in dairy was banned in Europe, but not in some other countries, including America. 

Years later, it was discovered that these children might have experienced these side effects due to a rare genetic disease called Williams syndrome. With Williams syndrome, you don’t actually want to take vitamin D at all. 

Now, when you look at vitamin D, it always says something along the line of “don’ take too much, you could experience a toxic effect.” 

But, according to this article by the Mayo Clinic, it would take getting 50,000 to 100,000 IUs of vitamin D for months to years before vitamin D would become toxic. This is actually a lot more than what most people consider toxic. 

When talking about vitamin toxicity, we’re referring to hypercalcemia, which is too much calcium in the blood. 

When people are afraid of vitamin D toxicity, they end up taking smaller amounts like 1,000 to 2,000 IUs per day. But, with this amount, it could actually take four months or more to bring a vitamin D deficiency up to a normal level. 

If you want to decrease the potential risk of kidney stones, you could drink 2.5 liters of fluid every day, avoid taking calcium and avoid dairy. 

When using high doses of vitamin D, always check with your doctor. High doses of vitamin D can increase blood/urine calcium levels resulting in calcium deposits in the kidney. Also drink 2.5 liters of fluids per day, take cofactors with vitamin D (magnesium, K2 and zinc) and have your doctor monitor your PTH, vitamin D blood levels and creatinine and avoid excess calcium supplements of high calcium foods when taking vitamin D levels over 10,000 IUs.

Generic name: Quetiapine.
Brand names: Seroquel.
Available as a tablet and modified-release tablet.

How to use it?
Swallow the tablet completely with half a glass of water. Do not break them and don’t chew on them, as this might lower the effectivity of the tablet.
In addition Quetiapine should not be taken with food, as this can also decrease its effectivity. Therefore take it at least 1 hour before a meal.

Duration of treatment:
The duration of treatment depends on the indication of use and the severity of your symptoms. If your symptoms improve with the use of Quetiapine you usually have to keep using it for 12-24 more months. And sometimes multiple years.

When to use it?
Quetiapine is part of a group of drugs called antipsychotics. It can be used to treat several mental health conditions:
- Schizophrenia.
- Manic or depressive episodes in bipolar disorder.
- Severe depressive episode

What is the correct dosage?
Unfortunately I can’t provide you with guidelines for the correct dosages, as these differ a lot depending on the specific indication, the side effects you experience and the severity of your complaints.
Usually your doctor starts with a low dose and quickly builds it up over a few days. Afterwards your maintenance dose will be adapted throughout your treatment based on the blood levels of quetiapine you will be having.
Therefore always use the prescribed dose and frequency your doctor gave you.


What are the side-effects?
- Very common (more than 10%): drowsiness, dry mouth, dizziness, headache, muscle cramps and weight gain.
- Common (1-10%): increased appetite, nightmares, suicidal thoughts, irritation, blurred vision, shortness of breath, fast heart rate, palpitations, vomiting and changes in your blood values.
- Uncommon (0.1-1%): hypersensitivity, diabetes, restless legs and sexual dysfunction.

Safety:
- You should not eat or drink grapefruit juice, as this might increase the side effects you are experience.
- Furthermore be careful when you combine Quetiapine with alcohol. As this might increase the dazing effect of quetiapine.
- In addition it is usually safe to drive while using this drug, however it can cause you to become drowsy, sleepy and/or dizzy. If you experiencing any of these side effects then please do not drive.
- Lastly, Quetiapine can have interactions with several drugs. Therefore always discuss this with your treating physician.

Pregnancy and Lactation:
Quetiapine can be dangerous in both cases, especially for pregnant women and their unborn babies. However there are some exceptions.Therefore always discuss this with your prescribing doctor.
