In most commodity markets, increasing competition drives the price of resources down to the benefit of consumers, but in the pharmaceutical market for MS drugs, something insidious is happening. In 1993, the first MS disease modifying therapy came onto the market (Betaseron©), but now there are twenty-two FDA-approved drugs. Despite this plethora of options from several manufacturers, the prices are climbing rapidly toward an absurd $100,000 per year. This video argues that doctors are largely to blame for this phenomenon.

In the previous tutorial we introduced antirheumatic drugs called DMARDs. Now let's discuss in greater detail a particular small molecule DMARD called methotrexate. This was originally designed to be an anti-cancer drug, but it was fortuitously found to be effective against rheumatoid arthritis, among other inflammatory diseases. How does this drug work? Leflunomide is an immunosuppressant used to treat RA and psoriatic arthritis and Multiple sclerosis. Leflunomide is a prodrug that is rapidly and almost completely metabolized following oral administration to its pharmacologically active metabolite, A77 1726 or teriflunomide. The mechanism of action of leflunomide has not been fully determined, but appears to primarily Inhibit the mitochondrial enzyme dihydroorotate dehydrogenase.

Lansoprazole is a medication from the Proton Pump Inhibitor (PPI) class.
It reduces acid production in the stomach, which helps relieve pain, gastritis, acid reflux, ulcers, and similar conditions.
🩺 Common Uses of Lansoprazole:
Lansoprazole is mainly used for treating Gastroesophageal Reflux Disease (GERD).GERD is a long-term digestive disorder where stomach acid or food frequently flows back into the esophagus.
⚠️ Common Symptoms of GERD:
Burning sensation in the chest (heartburn), especially after eating or while lying down
Acid reflux – sour or acidic taste in the mouth
Sore throat or dry cough
Feeling like something is stuck in the throat
Indigestion or frequent belching
Breathing difficulty or sleep disturbances
Hoarseness or weak voice
Nausea or occasional vomiting
Lansoprazole is also used to treat Erosive Esophagitis.
⚠️ Common Symptoms of Erosive Esophagitis:
Chest burning — especially after meals or while lying down
Acid reflux — bitter or sour taste, rising acid in throat
Sore or irritated throat
Difficulty swallowing
Pain or tightness in the chest

*Additional Proton Pump information: The proton pump that we talked about in this video is more specifically a Hydrogen and Potassium Proton Pump. It works by using ATP as energy, to essentially push Hydrogen into the stomach which binds to chloride to form hydrochloric acid. And during the process, it pulls Potassium out of the stomach. This is why it is often called the Hydrogen Potassium ATPase proton pump. Even this is a simplified explanation of how the H+/K+ ATPase pump works!

Like any other medication it comes with side effects. But in the case of Lamictal it's side effects range from more common (and less serious) to very rare (but potentially life threatening).

Bipolar disorder is a serious mental illness that involves recurring episodes of depression and either hypomania or mania. It normally requires medication to treat the acute episodes as well as keep them from coming back once they resolve. But there are people who go periods of time without medication.
However, if you go too long without meds while you are having an episode, the period of time you spend in those episodes without treatment makes future episodes harder to treat. It’s called the kindling effect.
A therapy that can help bipolar disorder is interpersonal and social rhythm therapy. This therapy has been shown to reduce the time of episodes and delaying the onset of future episodes. This can be useful if you are trying to stay off medication in your interepisode period. Interpersonal therapy focuses on problems in your relationships and social rhythm therapy helps you establish regular daily routines.

(1) Get out of bed, (2) First contact with another person, (3) Start work, housework or volunteer activities, (4) Have dinner, and (5) Go to bed

While this medication can be very effective at preventing bipolar depressive episodes, it can take several months for a patient to achieve an effective dose (due to its risk of Stephen's Johnson Syndrome, a very serious rash) and individuals in an acute depression may benefit from stabilization with another medication, while this is being titrated safely.
