Antidepressant mechanism - One hypothesis for the pathophysiology of depression is that it is due to low levels of monoamine neurotransmitters (mainly serotonin, norepinephrine and dopamine). That is why antidepressants aim to increase the levels of these neurotransmitters in the synaptic cleft. They do this by slowing the reuptake of the neurotransmitters so that they stay in the cleft longer and interact with post synaptic receptors more often. The first drugs in this group were non-specific and increased all of the monoamines, which lead to lots of side effects and safety issues related to toxicity. Newer antidepressants are more selective and mostly only effect 1 or 2 monoamines.
General principles:
Unfortunately, antidepressants take at least a month to start working. Good patient education about the delayed onset of effect and close monitoring of the patient during this initial period is extremely important. Patients can become hopeless if they expect the drug to start working right away. This may be one reason why antidepressants are associated with suicide, especially in patients 25 years old and younger. Another proposed mechanism is that a depressed person may have the energy to carry out their suicide once the medications start to work. There is now a black box warning for suicide on antidepressants. Some psychiatrists argue that they don’t actually see this association with suicide in clinical practice, and that the thing that really increases the risk for suicide is not treating a depressed person with the proper medications. However, it is still standard practice to have a close follow up with patients you are starting on antidepressants. Usually this will involve a follow up visit about 2 weeks after the medication is started. At this visit the drug will not have started working yet so you can’t evaluate efficacy, but you can monitor for side effects like suicidality. Another serious side effect you have to be on the lookout for soon after initiating treatment is mania. If a bipolar individual is incorrectly diagnosed as having depression, an antidepressant may induce a manic episode.
Another very serious side effect that has to be considered for antidepressants is Serotonin Syndrome. This usually occurs when you combined multiple antidepressants at the same time or combine an antidepressant with another medication that increases serotonin such as dextromethorphan or an opioid. It presents with tremor, diaphoresis, tachycardia, flushing and hypertension. If not corrected it can progress to delirium, AMS and death. Treatment includes medication cessation and the use of Cyproheptadine (a serotonin antagonist). In order to prevent this from happening you should have about a month “Wash Out” period when you are switching between antidepressants. So you taper the 1st medication down and then stop it, give the patient at least a month with no antidepressant and then start adding the new medication slowly.
Most side effects begin immediately after starting the medication, but diminish over the course of a month. This is another reason why patient compliance is poor with these meds. It makes them sick and the drug doesn’t work during the first few weeks. However, if they can stick with it the medications will likely start working and the side effects will diminish over time. A principle that applies to all of the antidepressants is “start low and go slow.” This means that you start with a lower dose and slowly increase it in order to decrease side effects and increase patient compliance. The dose you start the patient on may not even be at a therapeutic level, but every month or so you can increase the dose a bit.

The European Medicines Agency (EMA) recently approved dulaglutide (Trulicity®) at higher doses (3 mg and 4 mg solutions). Dulaglutide is indicated for the treatment of adults with insufficiently controlled type 2 diabetes mellitus as an adjunct to diet and exercise. Dulaglutide delays gastric emptying and has the potential to impact the rate of absorption of concomitantly administered oral medicinal products. It is delivered subcutaneously via pre-filled syringe.
Key points:
• Throughout the development cycle of dulaglutide, model‐informed drug development (MIDD) was applied to facilitate and accelerate decision‐making.
• A novel MIDD approach was adopted to evaluate drug-drug interactions (DDI), due to changes in gastric emptying.
• Acetaminophen PK from previous gastric emptying studies covering a wide range of dulaglutide doses was used to characterize the extent of gastric-emptying delay (GED) related to 4.5 mg of dulaglutide.
• The extent of GED was then applied to physiologically based pharmacokinetic (PBPK) models of individual 10 commonly co-administered oral agents to simulate effect of delayed gastric emptying on their PK profiles and inform drug labeling.

- What is Trazodone ( Desyrel )?
- Trazodone is an Antidepressant. It is a weak inhibitor of serotonin re uptake. So therapeutic benefit appears to be related to its ability to block postsynaptic serotonin type 2 receptors. It is also sedating, probably because of its potent histamine H1-blocking activity.
- It is available as 50, 100 and 150 mg tablets.
- Trazodone is indicated for the treatment of major depressive disorder. It has been used off-label for adjunct therapy in alcohol dependence, and off-label to treat anxiety, panic disorder, and insomnia.
- The usual dosage of Trazodone
- Initial dose: 150 mg orally per day in divided doses; this may be increased by 50 mg orally per day every 3 to 4 days.
- Maximum dose:
- Inpatients: 600 mg/day
- Outpatients: 400 mg/day

Firstly it's vital to know that you are not HIV positive. For this reason you will need to have testing for hiv before starting PrEP to make sure you are not already HIV positive. Truvada is also associated with some changes in kidney function so renal tests need to be ordered before staring this medication. There is some evidence that there can be reduced bone density. We recommend lettin gyour doctor know if you have any bone issues, as well as getting plenty of calcium rich foods, vitamin D as well as weight bareing exercise. Some people have diarrhoea, bloating and gastric side effects when starting Truvada, this settles quite quickly in most cases.

Belsomra (suvorexant) entered the US sleep-aid market almost a decade ago, and was recently joined by Dayvigo (lemborexant) and Quviviq (daridorexant).
All of these agents work to block orexin receptors in the brain, a distinctively different mechanism than other sleeping pills.
Despite several distinct advantages over other classes of sleeping agents, sales have not been spectacular. But recent discoveries, particularly a suggestion that suvorexant may reduce the amyloid Beta plaques and Tau fibrillary tangles that are hallmarks of Alzheimer's Dementia has driven up attention.
Experts think that these agents may also have a role in treating depression, anxiety and ADHD.

Dayvigo Lemborexant New Sleep Medication | My Nightmare experience NEVER AGAIN
Oh My GOD. That is all I could say, after my experience with this pill.
My experience started off great, a nice euphoric feeling, I fell asleep so peacefully only to have my sleep completely hold me prisoner of a private HELL. My dreams are very vivid to begin with & I have a lot of Nightmares & Strange dreams, habitually.
The Sleep Medication is supposed to cut off the chemical that is produced called Orexin (/ɒˈrɛksɪn/), also known as hypocretin, is a neuropeptide that regulates arousal, wakefulness, and appetite. ... They project widely throughout the central nervous system, regulating wakefulness, feeding, and other behaviors. It is designed to KEEP you asleep.
I had no prior knowledge that the medication has been known to cause Nightmares, that was not in the pamphlet that came with the Sleep medication.
I have been diagnosed with severe anxiety & I have panic attacks as well. My will is also incredibly strong. I am so glad I was able to shake myself awake but I was still a prisoner of the drug & I could not stay away so I delved right back into my pocket dimension of Hell in my bed.
I haven't been the same since the days I took DAYVIGO Sleep medication. It is 3 am eastern right now & I don't want to sleep because even though I stopped the pills, I still have insane NIGHTMARES.
IF YOU ARE SUSCEPTIBLE TO NIGHTMARES DO NOT TAKE THIS SLEEP MEDICATION AT ANY COST! If you suffer depression do not take this medication, it will mess you up.
