Hormone Therapy SIDE EFFECTS (in prostate cancer) - w/ Joel Nowak, MA, MSW and Jan Manarite
Hormone Therapy SIDE EFFECTS (in prostate cancer) - w/ Joel Nowak, MA, MSW and Jan Manarite Side Effects 1 Views • 1 month ago

Joel Nowak is the co-founder and CEO of Cancer ABCs. He is a prostate caner patient who has been on Intermittent Hormone Therapy for several years. Joel is a CANCER THRIVER having been diagnosed with five different cancers including metastatic prostate cancer, thyroid cancer, renal (kidney) cancer, appendiceal cancer and melanoma.

Hormone therapy for prostate cancer (also called Androgen Deprivation Therapy or ADT) can be a very effective treatment for men with prostate cancer. Like most treatments, ADT usually has some side effects. Many of these side effects can be moderated and it's essential that we help patients, especially if the treatment is working, but the side effects make hormone therapy miserable.

In the effort to help patients and caregivers engage in Share Decision-making, Joel discusses many of these side effects, with ideas and resources that may help control the side effects.

If you look at the list of the potential side effects that hormone therapy can cause it can become overwhelming. However, it is important to know that most men only experience a small number of the possible side effects. It is always impossible to know what side effects an individual man might experience and how intense the side effect might be. Everyone's side effects are different and there is no way to predict the nature of an individual's side effects.

* Note - It's important to remember that most serious side effects are rare. Always ask your doctor.

Pharmacology of Bicalutamide (Casodex) ; Pharmacokinetics, Mechanism of Action, Uses, Effects
Pharmacology of Bicalutamide (Casodex) ; Pharmacokinetics, Mechanism of Action, Uses, Effects Side Effects 1 Views • 1 month ago

Bicalutamide (brand name: Casodex) is a non-steroidal anti-androgen (NSAA) medication primarily used in the treatment of advanced prostate cancer. It is effective as part of androgen deprivation therapy (ADT) due to its ability to block the action of male sex hormones (androgens). Here's an overview of its pharmacology:

Mechanism of Action
Androgen Receptor Antagonist:

Bicalutamide binds competitively to androgen receptors, preventing androgens like testosterone and dihydrotestosterone (DHT) from activating the receptors.

This inhibition blocks androgen-mediated growth signals in androgen-dependent prostate cancer cells, leading to their apoptosis and suppression of tumor growth.

It is often combined with LHRH agonists or antagonists or surgical castration to achieve maximum androgen blockade.

Pharmacokinetics
Administration:

Administered orally as a single daily dose, usually 50 mg.

Absorption:

Well-absorbed after oral administration, with peak plasma concentrations reached within 1–2 hours.

Distribution:

Highly protein-bound and widely distributed in body tissues.

Metabolism:

Extensively metabolized in the liver by cytochrome P450 enzymes, primarily CYP3A4.

Excretion:

Excreted primarily in feces, with a small fraction eliminated in the urine.

Half-Life:

The elimination half-life of bicalutamide is approximately 6 days, allowing for once-daily dosing.

Indications
Prostate Cancer:

Used in combination with LHRH agonists (e.g., leuprolide) or after surgical castration to treat advanced or metastatic prostate cancer.

Off-Label Uses (less common):

Management of hirsutism or androgen-related disorders in females.

Adverse Effects
Common Side Effects:

Hot flashes.

Gynecomastia (breast enlargement) and breast tenderness.

Decreased libido and erectile dysfunction.

Nausea and diarrhea.

Hepatotoxicity:

Rare but can cause elevated liver enzymes or hepatocellular damage.

Liver function monitoring is recommended during therapy.

Other Effects:

Fatigue and weakness.

Anemia.

Rash or pruritus.

Contraindications and Precautions
Contraindications:

Females, particularly during pregnancy or lactation, as it can cause harm to the fetus.

Severe hepatic impairment.

Precautions:

Regular monitoring of liver function and complete blood counts is advised.

Use with caution in patients with pre-existing cardiovascular disease due to potential risks of fluid retention or anemia.

Drug Interactions
CYP3A4 Inhibitors:

Drugs like ketoconazole may increase bicalutamide plasma levels, increasing the risk of toxicity.

Anticoagulants (e.g., Warfarin):

Bicalutamide may enhance the effects of warfarin, increasing the risk of bleeding.

Other Hepatotoxic Drugs:

Combining with drugs that have hepatotoxic potential may exacerbate liver toxicity.

Clinical Considerations
Bicalutamide is primarily used for maximum androgen blockade (MAB) in prostate cancer treatment, aiming to enhance the efficacy of LHRH analogs.

Its favorable side effect profile compared to older anti-androgens (e.g., flutamide) makes it a preferred choice in many cases.

Which is Better - Surgery vs. Radiation for Prostate Cancer?
Which is Better - Surgery vs. Radiation for Prostate Cancer? Side Effects 1 Views • 1 month ago

PCRI’s Executive Director, Mark Scholz, MD, explains why there has been a surgery vs. radiation controversy, describes the pertinent risks and considerations when deciding between the two treatments, and explains why he thinks radiation is usually going to be the preferable option, all things considered.

0:43 The main reason why there is a controversy about whether surgery or radiation is better for treating prostate cancer is that as recently as 10-15 years ago the technology for radiation was significantly less effective and more toxic than it is today. There were also fewer options for salvage therapy after radiation in the event of a relapse. Since then, radiation technology has advanced significantly, but the dialogue has been comparatively mired. Today, radiation can be precisely delivered, and radiation is achieving even better cure rates than surgery.

1:59 Surgeons are constrained by the fact that the bladder and the rectum are within millimeters of the prostate. They cannot cut a margin around the gland. Radiation oncologists are not limited in this way.

2:21 External beam radiation has a cure rate about as good as surgery, and radioactive seed implants have even better cure rates than surgery.

2:33 Most men will live for a very long time after being treated for prostate cancer, and so it is important to consider the side effects of each treatment, especially now that they all have comparable cure rates.

2:54 There is a risk of sexual dysfunction with both surgery and radiation, however, the rate of impotence after surgery is significantly higher than after radiation.

3:15 There is a risk of incontinence with surgery that is almost non-existent with radiation. An operation performed by a top surgeon on an ideal candidate has about a 5-10% risk of causing incontinence, and that is with the best possible conditions. Stress incontinence occurs in around 50% of men, and ejaculation of urine occurs in about 20% of men.

4:00 Rectum burns used to be a concerning side effect of radiation, but that risk has been eliminated with the advent of SpaceOAR, a gel that separates the prostate from the rectum and protects it from radiation.

Mavacamten: A Revolutionary Treatment for Obstructive Hypertrophic Cardiomyopathy cas: 1642288-47-8
Mavacamten: A Revolutionary Treatment for Obstructive Hypertrophic Cardiomyopathy cas: 1642288-47-8 Side Effects 1 Views • 1 month ago

Mavacamten is a medication that was developed for the treatment of symptomatic obstructive hypertrophic cardiomyopathy (oHCM), a condition in which the heart muscle becomes abnormally thick, making it harder for the heart to pump blood.

Here's a brief overview:

Mechanism of Action: Mavacamten is a myosin inhibitor. It works by targeting the heart muscle contractility and reducing the excessive contractile force in oHCM, which in turn can help reduce the obstruction and improve symptoms.

Indication: It's primarily indicated for patients with symptomatic oHCM, a subset of hypertrophic cardiomyopathy where there's an obstruction to the outflow of blood from the heart due to the thickened heart muscle.

Benefits: Clinical trials have shown that Mavacamten can improve exercise capacity, alleviate symptoms (like chest pain and shortness of breath), and reduce the thickness of the heart muscle in patients with oHCM.

Administration: It's an oral medication, meaning it's taken by mouth.

Approval: As of my last update in January 2022, Mavacamten had received approval from the U.S. Food and Drug Administration (FDA) for the treatment of oHCM.

Safety and Side Effects: Like all medications, Mavacamten can have side effects. It's essential for patients to discuss potential risks and benefits with their healthcare provider.

It's worth noting that medical knowledge and drug approvals can evolve, so it's always a good idea to consult with a healthcare professional or check the latest resources for the most up-to-date information.

Barbara, who was told as a child she had a heart murmur, was used to living with symptoms such as occasionally becoming out of breath or feeling faint on a very hot day. This was her “normal,” so it didn’t occur to her to see a doctor about it. But one day when she was being prescreened for unrelated back surgery, she was diagnosed with symptomatic obstructive HCM. Barbara was put on a beta blocker at first, but her symptoms were still not improving. She was told that she would be a candidate for surgery, but by advocating for herself, she learned about a clinical trial. She was eligible to enroll, and after completion she learned she was receiving CAMZYOS.

Calquence Medication: Understanding its Benefits and Side Effects
Calquence Medication: Understanding its Benefits and Side Effects Side Effects 1 Views • 1 month ago

What is Calquence medication?.
Calquence, also known by its generic name acalabrutinib, is a medication that belongs to a class of drugs called Bruton's tyrosine kinase (BTK) inhibitors. It was approved by the U.S. Food and Drug Administration (FDA) in 2017 for the treatment of certain types of lymphomas, including mantle cell lymphoma (MCL). Calquence is usually prescribed for patients who have not responded well to other treatments or who have experienced a relapse.
One of the key advantages of Calquence over other BTK inhibitors is its selectivity. Calquence specifically targets BTK, a protein involved in the growth and survival of cancer cells. By inhibiting BTK, Calquence disrupts the signaling pathways that contribute to cancer cell proliferation, leading to tumor regression and improved patient outcomes.
Benefits of Calquence medication.
Calquence offers several benefits for cancer patients, particularly those with mantle cell lymphoma. One of the primary advantages is its ability to inhibit cancer cell growth. By selectively targeting BTK, Calquence disrupts the signaling pathways that promote the survival and proliferation of cancer cells. This can lead to a reduction in tumor size and potentially even complete remission in some cases.

In addition to its anti-cancer properties, Calquence is generally well-tolerated by patients. It has a favorable safety profile compared to other BTK inhibitors, with fewer reports of severe adverse effects. This makes Calquence a viable treatment option for patients who may not tolerate or respond well to other medications.
How does Calquence medication work?.
Calquence works by specifically targeting and inhibiting Bruton's tyrosine kinase (BTK), a protein that plays a crucial role in the survival and proliferation of cancer cells. BTK is a key component of the B-cell receptor signaling pathway, which is essential for the growth and survival of B-cell lymphomas such as mantle cell lymphoma.
When BTK is activated, it triggers a cascade of signaling events that promote cancer cell growth and survival. Calquence binds to BTK, preventing its activation and inhibiting the downstream signaling pathways. This disrupts the growth and survival signals, leading to decreased cancer cell proliferation and increased cell death.
By specifically targeting BTK, Calquence minimizes the impact on healthy cells and reduces the risk of off-target effects. This selectivity is one of the key advantages of Calquence over other BTK inhibitors, as it allows for more effective and targeted cancer treatment.
Common uses of Calquence medication.
Calquence is primarily used for the treatment of mantle cell lymphoma (MCL), a rare and aggressive type of non-Hodgkin lymphoma. It is usually prescribed for patients who have received at least one prior therapy and have not responded well to other treatment options.
In addition to MCL, Calquence is being investigated for its potential use in other types of lymphomas and leukemias. Clinical trials are currently underway to evaluate its efficacy and safety in patients with chronic lymphocytic leukemia (CLL), diffuse large B-cell lymphoma (DLBCL), and other B-cell malignancies.

Cabometyx: A Comprehensive Guide to Understanding and Managing Potential Side Effects.
Cabometyx: A Comprehensive Guide to Understanding and Managing Potential Side Effects. Side Effects 1 Views • 1 month ago

Cabometyx is a targeted therapy that works by inhibiting the growth and spread of cancer cells. While it can be a life-saving treatment option, it is important to be aware of the potential side effects that may arise.
One of the most common side effects of Cabometyx is fatigue. Many patients experience a sense of tiredness and lack of energy, which can impact daily activities and overall quality of life. It is essential to communicate any fatigue to your healthcare team so that they can provide appropriate support and guidance.
Diarrhea is another common side effect of Cabometyx. The medication can disrupt the normal functioning of the digestive system, leading to loose or watery stools. It is important to stay hydrated and follow any dietary recommendations provided by your healthcare team to minimize the impact of diarrhea.
High blood pressure is a potential side effect that requires careful monitoring. Cabometyx can increase blood pressure levels, which can be especially problematic for individuals with pre-existing hypertension. Regular blood pressure checks and medication adjustments may be necessary to manage this side effect effectively.
Managing gastrointestinal side effects.
Coping with gastrointestinal side effects, such as diarrhea and nausea, can be challenging. However, there are strategies that can help minimize discomfort and improve overall well-being.
When it comes to diarrhea, maintaining good hydration is crucial. Drinking plenty of fluids, such as water and clear broths, can help replace lost fluids and electrolytes. Avoiding spicy and greasy foods, as well as caffeine and alcohol, can also be beneficial in reducing diarrhea episodes.
Nausea can be managed by eating small, frequent meals throughout the day rather than large meals. Avoiding strong smells and eating bland, easily digestible foods can also help alleviate symptoms. If nausea persists, anti-nausea medications prescribed by your healthcare team may be necessary.
Coping with fatigue and weakness.
Fatigue and weakness are common side effects of Cabometyx that can significantly impact daily functioning. However, there are strategies that can help manage these symptoms and improve overall energy levels.
Prioritizing rest and getting enough sleep is essential for combating fatigue. Listen to your body and allow yourself to rest when needed. Engaging in gentle exercises, such as walking or yoga, can also help boost energy levels and reduce fatigue.
Maintaining a well-balanced diet rich in nutrients is crucial for managing weakness. Incorporating lean proteins, fruits, vegetables, and whole grains into your meals can provide the necessary energy to combat weakness. Additionally, avoiding excessive caffeine and sugar consumption can prevent energy crashes.
Addressing hand-foot síndrome.
Hand-foot syndrome, also known as palmar-plantar erythrodysesthesia, is a potential side effect of Cabometyx that affects the palms of the hands and soles of the feet. It can cause redness, swelling, and pain, making it difficult to perform daily activities.

Dealing with hypertension and cardiovascular side effects.
High blood pressure is a potential side effect of Cabometyx that requires careful management. Regular blood pressure monitoring and medication adjustments may be necessary to keep blood pressure levels in check.
In addition to medication, lifestyle modifications can also play a crucial role in managing hypertension. Maintaining a healthy weight, engaging in regular physical activity, following a low-sodium diet, and reducing stress levels can all contribute to better blood pressure control.
It is important to communicate any symptoms or concerns related to cardiovascular side effects to your healthcare team promptly. They can provide guidance and make necessary adjustments to ensure your safety and well-being.
Managing dermatologic side effects.
Dermatologic side effects, such as skin rash and dryness, can occur with Cabometyx treatment. These side effects can be uncomfortable and impact self-esteem. However, there are strategies to help manage them effectively.
