Videoconsejo sanitario sobre el nitrofurantoína, para qué sirve este medicamento, cómo y cuándo debemos administrarlo. Videoconsejo impartido por la farmacéutica Paula Mateos, elaborado por el Consejo General de Colegios Farmacéuticos y producido por Medicina TV.

La nitrofurantoína es un antibiótico derivado del nitrofurano. Es activa sobre un número importante de bacterias. No obstante, y al igual que cualquier otro antibiótico, no tiene ningún efecto frente a infecciones causadas por virus, como gripe, resfriado, COVID-19 o la mayoría de las infecciones de garganta.

Está indicado para el tratamiento de la cistitis aguda en niñas mayores de 3 meses, adolescentes y mujeres adultas.

La nitrofurantoína está disponible tanto en forma de comprimidos como en suspensión oral. La dosis recomendada para el tratamiento de la cistitis aguda no complicada en mujeres adultas es de 50-100 mg cada 8 horas durante 5-7 días.

Para niñas mayores de 3 meses y adolescentes, la dosis recomendada es de 5-7 mg/kg de peso por día, sin superar la dosis de adulto, repartidas en cuatro tomas durante 5-7 días. En el caso de niñas menores de 6 años, se debe usar la suspensión oral.

Se recomienda tomar nitrofurantoína durante la comida o con un vaso de leche.

Es importante usar nitrofurantoína solo para el tratamiento curativo de cistitis agudas y no como profilaxis, con una duración del tratamiento limitada a un máximo de 7 días puesto que un tratamiento prolongado puede tener riesgos pulmonares, hepáticos, alérgicos y neurológicos.

Las reacciones adversas más frecuentes en tratamientos con nitrofurantoína es la aparición de reacciones cutáneas alérgicas como prurito y urticaria; mareos; y problemas gastrointestinales como náuseas, vómitos, dolor abdominal y diarrea.

Pero el mayor riesgo de nitrofurantoína se debe al mal uso y al abuso que hacemos de los antibióticos, y es la aparición de resistencias, que pueden hacer que este antibiótico deje de ser eficaz. Para evitar o combatir este problema y reducir el riesgo de resistencias bacterianas, el Ministerio de Sanidad, a través de la Agencia Española de Medicamentos ha creado un grupo de expertos (Plan Nacional de la Resistencia a Antibióticos o PRAN), en el que participamos los farmacéuticos a través de representantes del Consejo General de Colegios Farmacéuticos.

Entre las principales recomendaciones, es fundamental no usar nitrofurantoína sin receta médica, ya que un antibiótico que te fue útil la última vez no tiene por qué serlo ahora. Además, es importante que tomes el medicamento a las horas y durante el periodo de tiempo que te haya indicado el médico. Y finalmente, desecha el tratamiento que te haya sobrado en el punto SIGRE de tu farmacia.

Generic name: Nitroglycerin.
Brand names: Nitro and Nitrolingual.
Available as spray, injection and patch.

How to use it?
- Spray: first take a rest. If your chest pain does NOT subsides use the spray. Do so by sitting down, press your tongue against the roof of your mouth and hold your breath. Then spray under the tongue and close your mouth. DO NOT inhale the spray. The drug should starts to work almost immediately. However if necessary, you can spray another puff after five minutes.
Contact a doctor immediately if the pain has not disappeared after three uses and after 15 minutes. This could indicate a heart attack.
- Patch: peel off the protective film and try not to touch the adhesive with your fingers. Apply the patch to a clean, dry, undamaged and not too hairy area on your chest or on the inside of the upper arm.

When to use it?
Nitroglycerin are part of a group of drugs called nitrates. These can cause vasodilation (which is the widening of your blood vessels), lowering the central venous pressure and decreases the supply of blood to the heart. That is why nitroglycerin can be used in the treatment of:
- Acute attacks of angina pectoris, which is a medical term for chest pain or heart cramp.
- And offlabel it is used in the treatment of acute heart failure and Acute coronary syndrome.

What is the correct dosage?
The precise dose will depend on your specific indication of use, the side effect you might experience and the severity of your symptoms. Therefore always use the precise amount and frequency your doctor prescribed you.
- However, when treating angina pectoris in adults with a spray your doctor might prescribe a dose of 1–2 sprays under the tongue. Which can be repeated after 5 minutes if necessary.
- When your doctor prescribed you nitroglycerin as a patch, the usual dose for an adult would be a patch of 5 mg per day. If necessary this can be increased up to a patch of 10 or 15mg per day.

What are the side-effects?
- Very common (10%): headache, nausea, vomiting.
- Common (1-10%): dizziness, low blood pressure, sleepiness, a fast heart rate.
- Uncommon (0.1-1%): Worsened symptoms of angina pectoris, collapse, allergic skin reactions.
- Rare (0.01-0.1%): transient redness of the face and neck.
- Very rare (0.01%): heartburn and exfoliative dermatitis.

Safety:
First of all nitroglycerin can cause dizziness. Which can make it dangerous to drive.
Furthermore if you combine it with alcohol, this might further increase some of the side effects you might experience.
In addition nitroglycerin can have interactions with other drugs your are taking. Always discuss this with your prescribing doctor.
Lastly, nitroglycerin can be dangerous for pregnant women and women who are breast feeding. Therefore always discuss this with your personal doctor.

MECHANISM OF ACTION
Nitroglycerin is converted to nitric oxide which then is converted to a protein that causes smooth muscle relaxation, causing vasodilation. It dilates coronary arteries increasing blood flow to the heart. This increases the blood flow to the ischemic areas which relieves the pain associated with angina pectoris. So by increasing the blood flow to the heart it relieves symptoms and pain associated with angina pectoris from ischemia. It also helps to increase cardiac output and reduces blood pressure from vasodilation.
 
SIDE EFFECTS
Hypotension
When the smooth muscles relax it causes vasodilation, so the vessels get wider, this reduces the pressure in the vessels. Reduced pressure in the blood vessels equals lower blood pressure.

Dizziness, headache, blurred vision, and syncope
These happen because of hypotension from the smooth muscle relaxation which causes vasodilation.

Tachycardia
Since there is vasodilation which causes hypotension, the heart tries to compensate and increase the heart rate to hopefully increase the blood pressure and compensate.

GI upset
This can happen because of the sudden vasodilation which can cause dizziness and hypotension.

constipation, nausea, vomiting;
dizziness, drowsiness, feeling tired;
headache;
or cold symptoms such as stuffy nose, sneezing, sore throat.

These drugs have a powerful effect on your brain. They’re both highly effective, but they work in different ways. These drugs also come with their own side effects. Here’s what to know about how tramadol and hydrocodone are similar and different.

How they work.

Both tramadol and hydrocodone attach to receptors in your brain to change your perception of pain. However, tramadol also allows the chemical messengers norepinephrine and serotonin to remain available to your brain for a longer period. It is believed that this helps block pain signals in your spinal cord.

Forms—Immediate-release tablets and extended-release tablets.

Strengths—Immediate release: 50 mg; extended release:
100 mg, 150 mg, 200 mg, 300 mg.

congestion.
sore throat.
headache.
itching.
weakness.
seizures.
Hydrocodone.

Generic available—No

Brand name— Zohydro ER, Hysingla ER

Forms—Extended-release tablets (Immediate-release hydrocodone is only available in combination products in which it’s combined with other drugs.)

Strengths—Extended release: 20–120 mg

Unique side effects:

confusion.
low blood pressure.
respiratory depression.
gastric obstruction.

Side effects
Common side effects of both drugs include:

dizziness.
drowsiness.
constipation.
loss of appetite.
nausea and vomiting.

However, tramadol may also cause:

flushing.
congestion.
sore throat.
headache.
itching.
weakness.

Most of these mild side effects will resolve within a few days.

Serious side effects.

More serious side effects of both drugs can include:

mood problems.
hypersensitivity reactions, including swelling of the tongue or throat, trouble. breathing, and skin rash.

Additionally, tramadol can cause seizures. Hydrocodone can also cause:

confusion.
low blood pressure.
respiratory depression.
gastric obstruction.
Get immediate medical attention or call 911 if you have any serious side effects. from either drug.

Risk factors.

Certain people may be more at risk of side effects. Side effects of both drugs are more likely or can be more intense if you are a senior. They can also be more intense if you have kidney or liver disease, chronic obstructive pulmonary disease, or other chronic diseases. People with depression who take tramadol may be at increased risk of suicide.

Interactions.

Tell your doctor and pharmacist about all the medications you take. Many drugs can interact. For example, alcohol and certain over-the-counter drugs that cause sleepiness, including cough or cold formulas, can increase the sedating effect of these drugs.

Warnings.

Hydrocodone comes with a black box warning about the potential for misuse. According to the Drug Enforcement Administration (DEA), hydrocodone is associated with more misuse than any other opioid.

It is possible to develop a tolerance to either drug, especially if you don’t take it exactly as your doctor prescribes it. Building a tolerance to a drug means that the same dosage no longer achieves the same effect. People who develop a tolerance often take more of the drug than recommended to get the same feeling.

Tolerance can often lead to dependence. You’re more likely to develop dependence on these drugs if you have a history of drug or alcohol misuse. If you feel yourself becoming dependent, contact your doctor right away.

Don’t stop taking the drug, especially if you have taken it for weeks or months. Your doctor will adjust your dosage to slowly taper you off the drug. This helps prevent withdrawal. Tell your doctor right away if you have withdrawal symptoms when you stop taking either of these drugs.

Deciding which one to take.

Before your doctor can recommend an opioid, it’s important that you discuss all underlying medical conditions. List all your over-the-counter and prescription medications and supplements. Tell your doctor if you’ve ever had a problem with alcohol or substance misuse.

Discuss the potential side effects and benefits of these and other opioids with your doctor. Together, you can choose the least potent treatment necessary to relieve your pain.

Norethisterone is a tablet that’s used for a few different things, one of which is to delay your period. Here Dr Daniel Atkinson, Clinical Lead at Treated, talks us through what it is and how it works.

Delaying your period can be really useful for you if you’ve got something coming up - like a holiday or a big event - that you don’t want your period getting in the way of. You might find it particularly useful if you tend to get some pretty inconvenient or uncomfortable symptoms during your period.

The normal dose is 3 tablets a day, and you can take it for however long you want to delay your period for (up to a maximum of 10 days). Norethisterone is a generic tablet, this means that it works in the same way as some other treatments for period delay (like Utovlan and Primolut N) but because it’s unbranded it’s usually a bit cheaper.

You wanted to know what side effects are associated with the mini pill, or one that has progestin only. When someone thinks of birth control pills, they may generally think of birth control pills that you take when you’re not breastfeeding, and these have 2 different hormones in them. But the mini pill only has 1, and it’s progestin, and this works in harmony with the hormones that a woman’s body produces while nursing to prevent ovulation from happening. The mini pill doesn’t cause headaches and it also doesn’t contribute to weight-gain, which is a really good thing after having a baby, because you’re trying to lose the weight.

Users of the mini pill are slightly more likely to have ovarian cysts, acne breakouts are more common while using it, and the most common side effect is unscheduled vaginal bleeding and spotting. This, by far, is the most common side effect and the reason why most women end up discontinuing its use. Some women actually experience shorter cycles or shorter periods in between times when they’re having periods and the periods may be longer, and it’s normal also to have breakthrough bleeding or bleeding between periods, and so if you’re experiencing this, talk with your doctor and they can help you determine if the birth control is the likely underlying cause, and if so, if another type of birth control would be better for your circumstances.
