Understanding What is Labetalol and what is Labetalol used for...

In today's discussion, we'll delve into the medication known as labetalol, commonly sold under the brand names Normodyne and Trandate. We'll explore its purpose, specifically its role in managing hypertension (high blood pressure), and its relationship to another drug, carvedilol (Coreg).

Labetalol functions by obstructing the adrenergic nervous system's receptors. This system sends nerves from the spinal cord to arteries, releasing norepinephrine, which binds to receptors on the arteries, causing them to constrict. This constriction leads to increased blood pressure. However, when labetalol binds to these receptors, it causes the arteries to dilate, thereby reducing blood pressure.

We'll also look at the common side effects associated with labetalol, which include fatigue, dizziness, nausea, headache, diarrhea, edema (fluid retention), and shortness of breath. It's important to note that labetalol can sometimes cause postural hypotension, a condition characterized by a sudden drop in blood pressure upon standing, potentially leading to fainting or light-headedness. This effect is especially worth monitoring within a few hours of the first dose or after any dosage adjustments.

Other significant side effects can include sexual dysfunction, abnormal heart rhythms, slow heart rate, liver failure, and allergic reactions. Regarding dosage, the typical starting oral dose is 100 mg twice daily, which can be adjusted based on blood pressure response. Maintenance doses generally range between 200-400 mg twice daily, with some severe hypertension cases requiring up to 2.4 g daily. Side effects may be minimized when the medication is divided into three daily doses. It's crucial that any changes or cessation of the medication be done under a physician's guidance to avoid serious complications.

For intravenous administration, an initial dose of 20 mg over 2 minutes is standard, with subsequent doses of 40 or 80 mg every 10 minutes as necessary, up to a maximum of 300 mg. Alternatively, labetalol can be given via intravenous infusion at a rate of 1-2 mg/minute.

Furthermore, labetalol has interactions with various drugs. It can mask the symptoms of hypoglycemia, especially tremors and an increased heart rate, posing a challenge for diabetic patients who use insulin or oral antidiabetic medications. When combined with tricyclic antidepressants, such as amitriptyline, imipramine, or nortriptyline, labetalol may increase tremors, a common side effect of these antidepressants. If used with adrenergic stimulating drugs for asthma, like albuterol or pirbuterol, labetalol's blocking effects might reduce the effectiveness of these asthma treatments. Drugs like glutethimide and cimetidine may affect labetalol's effectiveness by altering its elimination or increasing its blood levels. Additionally, halothane anesthesia can enhance labetalol's blood pressure-lowering effects.

As for its safety during pregnancy and breastfeeding, labetalol's effects have not been adequately studied in pregnant women, and it is known to be excreted in breast milk. Therefore, caution is advised for nursing mothers. Lastly, labetalol is a prescription medication, and it should be stored at temperatures ranging from 20º Celsius to 30º Celsius (68º Fahrenheit to 86º Fahrenheit).

Nonsteroidal anti-inflammatory drugs or NSAIDs are drugs with analgesic, antipyretic and anti-inflammatory effects .i.e. reducing pain, fever and inflammation.

The most common traditional NSAIDs include:

· Aspirin

· Ibuprofen (Motrin, Advil, Motrin IB)

· Naproxen (Naprosyn, Aleve)

There are three main categories of NSAIDs, which include:

How do they work?

NSAIDs work by preventing an enzyme from performing its function. An enzyme is a protein that triggers changes in the body. The enzyme, known as COX (cyclooxygenase) has two forms, COX- 1 and COX-2.

COX-1 protects the stomach lining from harsh acids and digestive chemicals; it also helps maintain kidney function. Meanwhile, COX-2 is produced when joints are injured or inflamed.

Since traditional NSAIDs block the actions of both, COX-1 and COX-2, the chances of stomach disorders and bleeding really increase.

What is ketoconazole used to treat?
Ketoconazole topical (for the skin) is an antifungal medicine used to treat infections such as athlete's foot, jock itch, ringworm, and seborrhea (dry, flaking skin or dandruff).

Is ketoconazole harmful?
In a July 2013 Drug Safety Communication, we warned that ketoconazole tablets should not be used as a first-line treatment for any fungal infection because it can cause severe liver injury and adrenal gland problems, and advised it can lead to harmful interactions with other medicines.

How long will it take for ketoconazole to work?
If you are using over-the-counter ketoconazole shampoo to treat dandruff, your symptoms should improve during the first 2 to 4 weeks of your treatment.

What fungus does ketoconazole kill?
It works by killing the fungus or yeast or preventing its growth. Ketoconazole cream is used to treat: Athlete's foot (tinea pedis; ringworm of the foot); Ringworm of the body (tinea corporis)

Can you use too much ketoconazole?
Do not use more of it, do not use it more often, and do not use it for a longer time than your doctor ordered. To do so may cause unwanted side effects or skin irritation.

How long should you take ketoconazole?
You may need to take ketoconazole for 6 months or longer to cure your infection completely. Continue to take ketoconazole until your doctor tells you that you should stop, even if you feel better. Do not stop taking ketoconazole without talking to your doctor.

Selenium sulfide may also be used as a single treatment; apply it at bedtime, leave it on overnight, and shower it off the next day. Repeat once a month until clear.

Do you need a prescription for ketoconazole?
Ketoconazole is a prescription drug. It's available as an oral tablet, topical cream, topical foam, shampoo, and topical gel. The oral tablet is only available as a generic drug.

How long does it take for ketoconazole cream 2 to work?
Clinical improvement may be seen fairly soon after treatment is begun; however, candidal infections and tinea cruris and corporis should be treated for two weeks in order to reduce the possibility of recurrence. Patients with tinea versicolor usually require two weeks of treatment.

Can ketoconazole shampoo damage liver?
The topical formulations of Nizoral have not been associated with liver damage, adrenal problems, or drug interactions. These formulations include creams, shampoos, foams, and gels applied to the skin, unlike the Nizoral tablets, which are taken by mouth

Does ketoconazole shampoo affect hair color?
FD&C Red No. 40 is the only colored ingredient in ketoconazole shampoo and the most likely cause of this patient's hair discoloration.

What is ketoconazole shampoo 2% used for?
Ketoconazole 2% shampoo is used to treat "sun fungus" (tinea versicolor; pityriasis versicolor). This medicine may also be used for other fungus infections of the skin as determined by your doctor.

Common side effects of Nizoral shampoo include:
mild skin itching or irritation,
dry skin,
abnormal hair texture,
scalp pustules,
oiliness and dryness of hair and scalp

Does Nizoral have side effects?
One of the most common side effects is skin irritation, which can take the form of pimple-like bumps where it's used. In some individuals, it may also cause either oiliness or dryness of the hair or scalp, abnormal hair texture, or discoloration

Can you use Nizoral everyday?
Usually, prescription ketoconazole is used at least twice a week for dandruff, and three times a week for hair loss. Over-the-counter versions can even be used daily.

Ketoconazole (Nizoral) is a topical anti-fungal treatment that can also be used to treat different types of hair loss. Ketoconazole is also used as a hair loss treatment for androgenetic alopecia (genetic pattern baldness) in both men and women.

According to the amendment, for drugs not covered by Schedule P-1, tablets and capsules should be packed as follows: if the count is less than 10, packaging should be in integral numbers; if more than 10, pack sizes should be multiples of five or seven.

If you have IBD, the Remicade (Infliximab) Infusion Center at the Hospital may be in your future as a possible treatment for Ulcerative Colitis (Pancolitis in my case) . In this video, PART 2 of my discussion on Remicade, I talk about my two experiences at an infusion center, one of which being less than enjoyable. The major side effect I experienced during the infusion was a headache, but as I explain in the video this is likely from being dehydrated and on an empty stomach, experienced side effects

Indication: Inflammatory disorders when patients do not respond to other medications

Action: Decreases pain and inflammation by inhibiting prostaglandin synthesis

Therapeutic Class: antirheumatics, ductus arteriosus patency adjuncts (IV only), nonsteroidal anti-inflammatory agents

Pharmacologic Class: none

Nursing Considerations:
 monitor for hepatitis and GI bleeding
 monitor for dizziness, drowsiness, and headache
 assess for anaphylactic reaction
 aspirin may decrease effectiveness
 monitor renal labs
 shake suspension before administration
 patient should wear sunscreen and protective clothing to protect against photosensitivity
